We describe a healthcare worker with a previously positive tuberculin skin test result who developed active tuberculosis. An investigation revealed 280 potential contacts, 3 of whom had positive tuberculin skin test results. Our experience demonstrates the potential benefits of therapy for latent tuberculosis infection as a component of a tuberculosis-control program.

In a 750-bed community-teaching hospital with 3,200 employees, throat cultures were routinely done in hospital personnel complaining of a sore throat. During a 3-month period, 323 employees had throat cultures; only 20 (6.2%) of these throat cultures grew group A streptococcus. The prevalence of positive throat cultures was similarly low in employees (6.2%) and adult patients (7.3%). There was no evidence that employees either had higher prevalence of group A streptococcal pharyngitis or that they spread the infection to patients. This report describes a severely retarded adult patient whose self-injurious behavior became acutely life threatening. Electroconvulsive therapy (ECT) was prohibited by law, but it was administered after the law was judged unconstitutional. The patient's response to ECT was excellent, and there were no adverse consequences.

Patients with generalized joint hypermobility (JHM) may experience excessive bruising/bleeding, with heavy menstrual bleeding (HMB) commonly reported. We performed a retrospective review of 30 adolescents seen in a Young Women's Hematology Clinic with both HMB and JHM. The purpose of this study was to examine pediatricians' use of nicotine replacement therapy (NRT) and the 5 A's counseling method with adolescent smokers. Coronary artery anomalies were found in 1,686 patients (1.3% incidence) undergoing coronary arteriography at the Cleveland Clinic Foundation from 1960 to 1988. Two hundred five children with a temperature greater than or equal to 104 degrees F seen in a pediatric emergency department were studied over a six-month fall-winter period to determine the patterns of illness and hospitalization. Children over five years composed a large group (38%) of patients admitted.
