Lagophthalmos after facial palsy: current therapeutic options.

As the facial nerve carries sensory, motor and parasympathetic fibres involved in facial muscle innervation, facial palsy results in functional and cosmetic impairment. It can result from a wide variety of causes like infectious processes, trauma, neoplasms, autoimmune diseases, and most commonly Bell's palsy, but it can also be of iatrogenic origin. The main ophthalmic sequel is lagophthalmos. The increased surface exposure increases the risk of keratitis, corneal ulceration, and potentially loss of vision. Treatment options are wide; some are temporary, some permanent. In addition to gold st

Pseudomembranous colitis with fatal outcome in a 43-year-old man.

Pseudomembranous colitis is a life-threatening complication of broad spectrum antibiotic therapy caused by Clostridium difficile. Untreated, the disease can lead to severe and in many cases fatal complications such as peritonitis due to colonic wall perforation, shock as a consequence of volume depletion, toxic megacolon and massive lower gastrointestinal haemorrhage. Fatal complications mostly occur in elderly people with a high degree of comorbidity. We report the case of a 43-year-old patient with AIDS who was admitted to hospital with abdominal pain of unknown origin and died before the co

Carcinoembryonic antigen staining in choriocarcinoma.

Choriocarcinoma of the colon presented as life-threatening lower gastrointestinal bleeding and a pathologic femoral fracture in a 42-year-old man. Elevated serum carcinoembryonic antigen (CEA) levels and immunohistochemical positivity for CEA in neoplastic syncytiotrophoblasts were noted, as well as the expected positivity for the beta subunit of human chorionic gonadotropin (beta HCG). Additional cases, including one of two gestational choriocarcinomas and one of four testicular choriocarcinomas studied immunocytochemically for CEA also demonstrated positivity. Although CEA staining is common

[Wolff-Parkinson-White syndrome; important in childhood also].

In a cooperative, retrospective, study 120 children are reviewed with preexcitation pattern. 80 patients had tachycardias; 54 children were under 1 year, 49 under 3 months of age at their first attack. In 50% the preexcitation pattern disappeared in the first year of life, allthough intermittent preexcitation could be seen in some patients. In 12 patients a circusmovement tachycardia was proved on the surface ecg; in the group of 63 children with a tachycardia of unknown origin probably more may be caused by this mechanism. A beneficial effect of digoxin in childhood is noticed, with good resp

A Review of Pseudomyxoma Peritonei: Insights Into Diagnosis, Management, and Prognosis.

Pseudomyxoma peritonei (PMP) is a rare and complex clinical syndrome characterized by the accumulation of mucinous ascites within the peritoneal cavity, typically associated with mucinous tumours of appendiceal origin. Despite its rarity, PMP poses significant challenges in diagnosis and management due to its indolent yet locally aggressive nature. This comprehensive review provides insights into the diagnosis, management, and prognosis of PMP, synthesizing current evidence and emerging trends in the field. Challenges and opportunities in PMP management are discussed, along with recommendation

Incomplete Kawasaki Disease in an Adult Complicated by Stroke.

Kawasaki disease (KD) is a systemic vasculitis with an unclear origin primarily affecting young children. The sole difference between complete and incomplete KD upon presentation is the number of signs and symptoms. There is a concerning case of a 50-year-old male with incomplete KD who has additionally presented with a stroke. Cases of KD are uncommon in adults but well-known in children. Ischemic stroke is quite rare in people with incomplete KD. This appears to be the first documented case of incomplete KD linked to a stroke in an adult. Because of the potentially life-threatening consequen

Cancer prevention and control: Kaposi's sarcoma.

Kaposi's sarcoma (KS) is a vascular tumour of endothelial origin that is associated with human herpes virus-8 infection. In sub-Saharan Africa, AIDS-KS remains the most common HIV-associated malignancy, and hence it poses a huge burden to the already constrained health-care systems. KS has four clinical variants, namely, classic, endemic, iatrogenic and epidemic KS. The histopathology in these different KS forms is essentially identical; however, they have different clinical patterns. Expanding knowledge of KS biology increases hope for prevention, disease control, and hence better quality of

[Malaria].

Malaria is the most important infectious disease imported by travelers and migrants from tropical and subtropical areas. It is imported quite frequently. It is a life-threatening disease. Symptoms are nonspecific and cannot easily be distinguished from a wide range of other febrile conditions. Therefore, travel history must be taken in all patients with fever of unknown origin and malaria diagnostics must be performed immediately on suspicion of malaria. Uncomplicated falciparum malaria should be treated in the hospital with either atovaquone-proguanil or with an artemisinin-based combination

Case report: upper extremity deep venous thrombosis in a 19-year-old baseball player.

This report describes a case of a collegiate baseball player who presented with 3 weeks of worsening right shoulder pain and mild swelling and faint discoloration of the right arm and hand. He was found to have a nonocclusive clot on his brachial vein and was treated with anticoagulants. This was his second lifetime thrombosis. A hematologic workup revealed no evidence of a hereditary origin, and thrombosis was believed to be related to recent exertion (baseball throwing). Upper extremity deep vein thromboses are extremely uncommon. However, because they can have potentially life-threatening c

Haemophagocytic syndrome with disseminated intravascular coagulation associated with tuberculosis.

Haemophagocytic syndrome (HPS) is a clinical entity that combines non-specific clinical and biological features. The diagnosis is usually confirmed by a bone marrow examination. HPS may be primary or secondary to a malignancy or to an infectious or autoimmune disease. Early aggressive survey of the aetiology and optimal treatment of the underlying disease improve the outcome of life-threatening HPS. Infection-associated HPS occurs predominantly in immunocompromised patients and is usually fatal. Leading trigger agents are viruses, especially the Epstein-Barr virus  and cytomegalovirus. Mycobac

[Aortic regurgitation caused by quadri-cuspid aortic valve].

Authors report the surgical treatment of a rare malformation of the aortic valve associated with coronary artery stenosis. The aortic valve had a supernumerary cusp which was revealed intraoperatively. The supernumerary cusp appeared to be similar in size to the three other. Surveying the literature it is stated, that the number of similar entities are about 50 in number. Autopsy findings note, that the prevalence is about 0.008-0.012%. During the past 25 years, in aortic valve replacement cases such entity could have not been explored. The possible significance of the entity lays in--that wit

The many faces of atypical sinusitis.

The physician must be alert to the possibility of unsuspected sinusitis when evaluating a patient with chronic cough, sore throat, fever of unknown origin, supraglottitis, pneumonia, or headache. This article presents four cases in which atypical or asymptomatic sinusitis was discovered that could have caused significant or potentially life-threatening complications. In each case, the sinusitis was initially unsuspected. A complete nasal evaluation is warranted following decongestion of the nasal cavity when conditions are present. A screening sinus computed tomography scan may be indicated wh

Proarrhythmic effect of adenosine in a patient with atrial flutter.

Adenosine has been proposed as a useful diagnostic agent in patients with narrow complex regular tachycardia of uncertain origin. Its effects are usually transient owing to its extremely short plasma half life and, as a consequence, it is thought to be safer than other drugs used in the acute treatment of such arrhythmias. However, adenosine had a proarrhythmic effect when administered to a patient in order to confirm the diagnosis of atrial flutter. As expected, a transient increase in atrioventricular block was seen but this was followed by a doubling of the ventricular rate and haemodynamic

Pediatric HIV infection in its second decade--the changing pattern of lung involvement. Clinical, plain film, and computed tomographic findings.

The changing pattern of pediatric HIV infection is illustrated in this article with plain films and computed tomographic scans. Today, vertical transmission from infected mothers results in HIV infection of exposed infants in about one third of pregnancies. Although the high mortality with catastrophic illnesses such as Pneumocystis carinii pneumonia seen in early life appears to be decreasing, chronic illness with pulmonary involvement due to diffuse lymphocytic infiltrative syndromes continues. Lymphocytic interstitial pneumonitis (LIP) has evolved in some patients to cystic lung disease and

A case of cutaneous metastases of gastric carcinoma showing peculiar clinical features.

A 52-year-old male patient presented with multiple cutaneous nodules on the face, trunk, and upper extremities. Examination of a skin biopsy specimen disclosed numerous signet ring cells throughout the dermis. Histopathologic examination of the stomach, along with gastroscopy, revealed that the cutaneous metastases were of gastric origin. The metastatic skin lesion on the shoulder and the back of the patient grew relatively rapidly, showing an erysipelas-like lesion followed by prominently large tumor masses. The clinical appearance seen in our case is thought to be rare. Radiotherapy and chem

Complex Congenital Heart Malformation Including Transposition of the Great Vessels, Presenting as Sudden Unexplained Infant Death.

Transposition of the great vessels (TGV) is a common congenital heart defect that is difficult to diagnose before birth. Antenatal diagnosis is associated with increased survival. Unusual features such as anomalous pulmonary artery origin may delay cyanosis, decreasing clinical suspicion. A three-week old female infant who had never been cyanotic presented for forensic autopsy due to onset of unresponsiveness at home. History included risk factors for TGV and signs of heart enlargement that were not recognized during life. Cardiac pathology consultation identified D-type TGV with additional ra

Fibroid growth and medical options for treatment.

Although fibroids are common benign tumors, their impact on women's quality of life can be considerable. The most frequent symptoms are uterine bleeding, resulting in anemia, and pelvic pain. Fibroids can be of genetic or hormonal origin or arise from intrauterine events. Current options for medical treatment include control of estradiol and progesterone production or action and are discussed in this review. Although curative treatment of fibroids relies on surgical strategies, the current trend is for uterine-sparing treatment to preserve fertility and avoid unnecessary surgery. Currently app

The placenta is simply a neuroendocrine parasite.

This paper will document the early scientific observations that kindled my neuroendocrinological interest in pre-eclampsia, a life-threatening disease that affects both mother and baby. My interest in this subject started with the placental origin of melanotrophin activity, moving on, through corticotrophin-releasing factor and its binding protein, to a tachykinin modified specifically in the placenta by phosphocholine, a post-translational moiety normally used by parasites to avoid immune surveillance and rejection. This work may finally have led to an understanding of the identity of the elu

Polymorphic corneal abnormalities.

Alterations of the deep corneal layers, which are manifest primarily as Hassall-Henle bodies in older patients, are a relatively frequent finding in the normal population. Changes which occur in Descemet's membrane and the endothelium earlier in life can be ascribed to various causes, including dystrophy, herpetic infections, and birth trauma. Six cases in which clear vesicles and scalloped lesions were found exclusively at the level of Descemet's membrane and the endothelium are reported. Although the polymorphic changes were in some cases extensive, visual acuity and, apparently, corneal phy

A Case Report of an Unusual Presentation of Epiglottic Cyst Causing Airway Obstruction in an Adult.

A laryngeal cyst is usually benign. The most common site origin of a cyst in the larynx is the lingual surface of the epiglottis. Epiglottic cysts are rare. Usually, it is asymptomatic in patients and can be treated conservatively if the size is small. Airway obstruction is very rare but could be life-threatening. If there is airway obstruction, the cyst should be removed immediately. Early detection and immediate management lead to favorable outcomes. A multidisciplinary-team approach with the ENT and anesthetic inputs are essential when dealing with the current condition. This case report hi
