A stepped care approach to reduction of anterior shoulder dislocation in the prone position.

There are various methods of reducing anterior shoulder dislocations. The traditionally used methods like Kocher's and Hippocratic are associated with procedural complications and frequently require sedation/muscle relaxants which have their own complications and are associated with longer stay and monitoring requirement in the emergency department. We describe a stepped care approach to shoulder dislocation reduction in the prone position involving four steps and two new manipulation methods taking into consideration the pathological deforming forces in a dislocated shoulder and addressing th

Entrapment of the acetabular labrum following reduction of traumatic hip dislocation in a child.

In traumatic hip dislocation, concentric reduction can be prevented by various causes. Soft-tissue interposition, such as entrapment of the acetabular labrum, is a rare but important cause of failed reduction of a hip. Early diagnosis of incomplete reduction due to interposition of soft tissue is important, because delayed treatment is associated with a greater incidence of avascular necrosis of the femoral head and early onset of osteoarthritis. This report describes a case of acetabular labral entrapment following reduction of traumatic hip dislocation in a child. The importance of CT and MR

Pyogenic infections in orthopedic surgery. Prevention and management.

A problem that confronts surgeons in clinical practice is that a patient may acquire new infections while in the hospital. When such infections occur they are predominantly staphylococcal and these bacteria are often, but not always resistant to penicillin, streptomycin and the tetracycline antibiotics. They are often but neither completely nor uniformly sensitive to the newer or less frequently used antimicrobial agents. The extension of antibiotic usage from proven situations to "routine" prophylaxis has been a widespread practice. There are many reasons to discourage and to reexamine the va

Transfusion practice in elderly patients across surgical disciplines in Germany - a secondary data analysis of over 21 million patients.

BACKGROUND: Clinicians are increasingly confronted with elderly patients across nearly all surgical disciplines. Advanced age is an independent risk factor for complications, prolonged hospitalization, and particularly for red blood cell (RBC) transfusion. The elderly suffer more frequently from anemia, which is associated with higher perioperative transfusion requirements, increased complication rates, and mortality Surgical disciplines differ not only in primary diagnoses but also in the demographics of their typical patient population. MATERIAL AND METHODS: Hospitalized surgical patients i

Patterns of Bacterial Infections Following Orthopedic Surgeries: A Retrospective Analysis.

Background and objective Surgical site infections (SSIs) following orthopedic procedures are associated with significant morbidity and continue to pose an ongoing clinical challenge. Pathogen patterns vary by surgical category and may not be adequately covered by standard perioperative antibiotic prophylaxis, which has remained largely unchanged for decades. This study aimed to characterize pathogen distribution patterns across different orthopedic surgical categories and to evaluate whether current prophylactic regimens align with the observed microbiological profile. Methods This retrospecti…

First experiences of immersive virtual reality as an additional learning tool in curricular teaching for orthopedic and trauma surgery.

INTRODUCTION: Immersive virtual reality (iVR) simulators have been introduced for skills training in various medical disciplines. While iVR simulators have been shown to improve technical skills, they are currently not well established in curricular teaching for medical students. The aim of this study was to demonstrate the implementation of an iVR operating theater in addition to conventional bedside teaching in orthopedic and trauma surgery and to evaluate user feedback regarding expectations, acceptance and limitations. METHODS: Medical students must complete a week of bedside teaching in

Amino Acid Supplementation May Help Prevent Muscle Wasting After Orthopedic Surgery, but Additional Studies Are Warranted: A Systematic Review of Randomized Clinical Trials.

Background: Essential amino acid (EAA) supplementation, including conditionally essential amino acid (CEAA) and branched-chain amino acids (BCAA) supplementation, has been suggested as a mechanism to optimize patient outcomes by counteracting the atrophy associated with orthopedic procedures. Purpose: We sought to investigate the effect of EAA supplementation in the perioperative period on patients undergoing orthopedic and spine surgery, specifically whether it is associated with (1) reductions in postoperative muscle atrophy and (2) improved postoperative function including range of motion, …

Standardized Multimodal Pain Protocol Minimizes Inpatient Opioid Administration in Pediatric Orthopedic Surgery Population.

BACKGROUND: Optimal management of post-operative pain is a critical component of orthopedic surgical care. There is a heightened awareness of narcotic prescribing habits given the current "opioid epidemic." The lack of standardized protocols has led to increased errors, delayed access to prescribed medications, and excessive narcotic prescribing. The purpose of this study is to assess the current trends in opioid use and document the prescribing patterns in the pediatric population before and after the implementation of a standardized protocol at our Institution. METHODS: A multimodal postoper

Extremity soft tissue coverage in the combat zone: use of pedicled flap transfers by the deployed orthopedic surgeon.

BACKGROUND: In theaters of operation, military orthopedic surgeons have to deal with complex open extremity injuries and perform soft-tissue reconstruction on local patients who cannot be evacuated. Our objective was to evaluate the outcomes and discuss practical issues regarding the use of pedicled flap transfers performed in the combat zone on local national patients. METHODS: A retrospective study was conducted on data from patients treated by a single orthopedic surgeon during four tours in Chad, Afghanistan and Mali between 2010 and 2017. All pedicled flap transfers performed on extremit

Microsurgery and external fixation in orthoplastic reconstruction of tibial injuries.

BACKGROUND: "Orthoplastics" is a relatively new approach to lower limb reconstruction, where an integration of both plastic and orthopedic expertise is required, together with the availability of well-equipped facilities. Acute shortening and long-term frames for lengthening are generally considered alternatives to length preservation and soft tissue microsurgical reconstruction, but an integration of external fixation and reconstructive microsurgery is gaining an increasing role with refinements of joint approaches. MATERIAL AND METHODS: Data on sixteen patients who underwent microsurgical l

Comparison of ESwab and Wound Fiber Swab Specimen Collection Devices for Use with Xpert SA Nasal Complete Assay.

Paired nasal swab specimens were collected from patients who were undergoing routine methicillin-resistant Staphylococcus aureus (MRSA) screening prior to elective cardiac or orthopedic procedures. Each patient was swabbed using a traditional wound fiber liquid Stuart swab and an ESwab device, a flocked swab with a modified liquid Amies microbiology transport medium. The two specimens were tested using the Cepheid Xpert SA Nasal Complete assay. Results demonstrated a 95.5% agreement between the ESwab and the FDA-cleared wound fiber swab collection device.

Median nerve (anatomical variations) and carpal tunel syndrome - revisited.

Carpal tunnel syndrome belongs to the most common causative factors of surgical interventions in the wrist region. Anatomy of carpal tunnel and median nerve is a subject of current revision. Authors paid attention to etiology of the syndrome based on review of literature and their own anatomical studies. They remind basic knowledge on the median nerve and indicate that only based on number of dissections a good orthopedic surgeon may acquire experience necessary to perform procedures in a most appropriate way.

Exploring versatile applications of a vacuum-assisted bone harvester in orthopedic surgery.

BACKGROUND: Orthopedic procedures often require removing bone or pathological tissue, with traditional methods involving instruments like curettes and rongeurs. However, these methods can be time-consuming and lead to increased blood loss. To mitigate these side effects, vacuum-assisted tools have been developed to aid in tissue removal. These devices enable surgeons to suction tissue without discarding it, potentially improving outcomes in conditions such as osteomyelitis or tumor removal while enabling collection of the material for downstream applications. Despite limited research, vacuum-a…

Is antibiotic prophylaxis necessary in elective soft tissue hand surgery?

Antibiotic prophylaxis for clean soft tissue hand surgery is not yet defined. Current literature focuses on overall orthopedic procedures, traumatic hand surgery, and carpal tunnel release. However, a paucity of data exists regarding the role of antibiotic prophylaxis in a broader variety of soft tissue hand procedures. The goal of the current study was to evaluate the rates of surgical site infection following elective soft tissue hand surgery with respect to administration of prophylactic antibiotics.A multicenter, retrospective review was performed on 600 consecutive elective soft tissue ha…

Critical-sized bone defects: essential concepts for the higher orthopaedic examination.

PURPOSE: To provide a practical review of the assessment and management of critical-sized bone defects (CSDs) for candidates preparing for higher orthopaedic examinations. METHODS: This narrative review considers definitions and classification, clinical assessment, acute management, reconstructive techniques and salvage options. RESULTS: A critical-sized bone defect (CSD) is unlikely to heal without intervention because of its size and location, although no numerical threshold defines criticality. Assessment should consider skeletal stability, infection, vascularity, soft tissues, host facto

Allograft reconstruction of the medial patellofemoral complex in skeletally immature patients: a technical note.

Patellofemoral instability (PFI) is a common disorder in children and adolescents. Surgical stabilization of the patella poses challenges in skeletally immature patients due to anatomical peculiarities at this stage, leading to the absence of an established standard reconstruction technique. Recently, there has been a notable interest in the medial patellofemoral complex (MPFC), encompassing the medial patellofemoral ligament (MPFL) and the medial quadriceps tendon femoral ligament (MQTFL), owing to their role in restricting lateral patellar displacement. This paper aims to describe the surgic
