Data condensed synthesis regarding kinesiotherapeutic procedures used in spasticity therapy.

Spasticity represents an important feature of the upper motoneuron syndrome (UMNS). The clinical signs, such as the abnormal movement models, the unwanted muscular co-contractions, the muscular and joint rigidity with a consecutive deformity can be signs of spasticity and, also of upper motoneuron lesion. The different therapeutic options applied in the management of spasticity are a basic component of UMNS treatment scheme. This article presents the main kinesiotherapeutic procedures used in spasticity therapy.

Development of an assist controller with robot suit HAL for hemiplegic patients using motion data on the unaffected side.

Among several characteristics seen in gait of hemiplegic patients after stroke, symmetry is known to be an indicator of the degree of impairment of walking ability. This paper proposes a control method for a wearable type lower limb motion assist robot to realize spontaneous symmetric gait for these individuals. This control method stores the motion of the unaffected limb during swing and then provides motion support on the affected limb during the subsequent swing using the stored pattern to realize symmetric gait based on spontaneous limb swing. This method is implemented on the robot suit H

Timely manner application of hand orthoses to patients with amyotrophic lateral sclerosis: A case report.

BACKGROUND: In patients with amyotrophic lateral sclerosis, the atrophy of hand muscles often causes impairment of thumb/finger motion and disabilities in their activities of daily living. Hand orthoses are effective for such impairment and disabilities; however, there are some difficulties in their application. CASE DESCRIPTION AND METHODS: In this case report, we present the timely application of hand orthoses and introduce our clinical algorithm for the application of hand orthoses to patients with amyotrophic lateral sclerosis. FINDINGS AND OUTCOMES: Our clinical algorithm was applied to

Functional walking ability of paraplegic patients: comparison of functional electrical stimulation versus mechanical orthoses.

BACKGROUND: Spinal cord injury (SCI) can cause partial or complete paralysis of the lower extremities, impairing the ability of individuals to stand and walk. Various types of mechanical orthoses, functional electrical stimulation (FES) systems and hybrid orthoses that incorporate FES have been designed to restore the ability of individuals with SCI to stand and walk. Standing and ambulation performance of SCI subjects using these different systems have been previously evaluated using energy consumption analysis, stability analysis and by quantitative gait analysis. Though FES-based systems ar

Simple sling for hand infection.

In the absence of a Bradford sling, I constructed a simple sling to elevate an infected hand from medical equipment that was readily available on the ward. The results were so successful and the experience so comfortable for the patient that the technique should be explained and made clear for other doctors. In this particular case, a serious side effect was avoided namely infection of nearby indwelling metalwork. The patient contacted us afterwards to say how well he felt, he had been looked after and how comfortable the equipment had been to use during his stay in hospital.

A portable powered ankle-foot orthosis for rehabilitation.

Innovative technological advancements in the field of orthotics, such as portable powered orthotic systems, could create new treatment modalities to improve the functional out come of rehabilitation. In this article, we present a novel portable powered ankle-foot orthosis (PPAFO) to provide untethered assistance during gait. The PPAFO provides both plantar flexor and dorsiflexor torque assistance by way of a bidirectional pneumatic rotary actuator. The system uses a portable pneumatic power source (compressed carbon dioxide bottle) and embedded electronics to control the actuation of the foot.

The role of bracing in Legg-Calve-Perthes disease.

BACKGROUND: Treatment methods in Legg-Calve-Perthes disease (LCPD) have varied during the 100-year history of this disorder. This is a review of the present practice of bracing in LCPD. METHODS: Published articles from the last 35 years were reviewed including primary analyses of bracing, meta-analysis, and summaries of present opinion. The recent literature was also evaluated to determine the present bracing practices. RESULTS: Studies performed regarding specific braces failed to show that they offer any advantage over other methods of management, including no treatment. Similarly, meta-an

Gait changes with the use of Heelys: a case study.

Heelys shoes are a novel athletic shoe with a concealed wheel. They have been popular among youths since their introduction in 2000. This case study serves as a first look into the biomechanical implications of Heelys shoes on gait. Pressure readings of the forefoot, midfoot, and rearfoot during ambulation in regular athletic-shoe walking, Heelys without the wheel walking, Heelys with the wheel walking, and Heelys skating with the wheel were recorded on a single subject using the Pedar X System. A visual gait analysis was also performed on the subject. The resulting data show increased forefoo

Customized dynamic splinting: orthoses that promote optimal function and recovery after radial nerve injury: a case report.

Radial nerve injury is a relatively common occurrence and recovery depends on the level of injury and extent of connective tissue damage. Orthoses (splints) are often provided to compensate for lost motor power. This article chronicles the recovery, over 27 months, of a 76-year-old woman who sustained a high radial nerve injury of her dominant arm during surgery for total shoulder replacement (Delta Reverse). Customized, low-profile dynamic splints, unlike any previously published design, were developed to address her goals for functional independence and the biological needs of the tissues. D

Shoe adaptation after amputation of the II - V phalangeal bones of the foot.

In The Netherlands, about 50% of all amputations of the lower limb are toes and forefoot amputations. Traumata of toes and mid-foot are rare. Preservation of the foot is the primary goal for treatment. Crush injuries of the foot may be associated with prolonged morbidity. This case study presents an insole solution for the solitary first phalangeal bone after amputation of the phalangeal bones II - V. The normal adaptation for forefoot amputations is stiffening of the sole of the shoe and a rocker bar to improve the toe off phase with load reduction of the forefoot. Because the patient had to

Use of a lumbosacral orthosis in the management of septic arthritis of the hip.

INTRODUCTION: Septic arthritis can lead to loss of joint motion and permanent mobility deficits because of infection and inflammatory response damaging articular structures. METHODS: A 13-year-old boy underwent five left hip surgeries after diagnosis of septic arthritis. He participated in 14 months of physical therapy, including hip stretching and manipulation. A lumbosacral orthosis (LSO) was applied to immobilize the lumbar region and isolate motion at the left hip. Treatment was divided into three phases: initial treatment, after manipulation under anesthesia, and after application of the
