Hypercalcemia upon recurrence of renal cell carcinoma producing parathyroid hormone-related protein.

We established a new renal carcinoma cell line that produces parathyroid hormone-related protein (PTHrP) and interleukin-6 in culture. The cellular production of PTHrP was confirmed by Northern blot analysis and immunofluorescence examination. Bone and lung metastases occurred simultaneously 3.5 years after surgery. The patient did not show hypercalcemia at this time, despite the presence of multiple osteolytic metastases. About 7 months after bone metastasis was first shown, serum PTHrP was detected by means of an immunoradiometric assay and the calcium level was found to be elevated to 3.29 …

Normocalcemia with elevated parathyroid hormone levels after surgical treatment of primary hyperparathyroidism.

BACKGROUND: Thirty percent of patients who undergo successful parathyroidectomy for primary hyperparathyroidism show unexplained elevated postoperative serum parathyroid hormone (PTH) levels despite normocalcemia. METHODS: PTH levels were measured monthly in 97 patients for 6 months after parathyroidectomy. Renal function, 25-OH-vitamin D levels, serum alkaline phosphatase levels, osteocalcin, and bone densitometry were evaluated before and 6 months after surgery. PTH reactivity to calcium loading was tested at the sixth month. RESULTS: Thirty patients had elevated PTH levels despite normoca…

[Activating mutations of calcium-sensing receptor cause insufficient secretion of parathyroid hormone].

Calcium-sensing receptor (CaSR) was cloned as an essential receptor for regulating secretion of parathyroid hormone (PTH) by extracellular Ca. Sensing of Ca by CaSR activates several intracellular signal transduction systems and suppresses secretion of PTH. Autosomal dominant hypocalcemia (ADH) with insufficient secretion of PTH was shown to be caused by activating mutations of CaSR. Clinical spectrum of ADH is broad from asymptomatic patients to severe hypocalcemia with tetany soon after birth. Some patients formerly believed to have idiopathic hypoparathyroidism may actually have activating …

Spatial and temporal expression of parathyroid hormone-related protein during wound healing.

Parathyroid hormone-related protein is produced by many normal tissues including the skin, where it regulates growth and differentiation of keratinocytes. To define better the role of parathyroid hormone-related protein in the skin, we investigated the spatial and temporal expression of parathyroid hormone-related protein and mRNA by immunohistochemistry and in situ hybridization during the healing of skin wounds, and the effects of topical administration of a parathyroid hormone-related protein agonist [parathyroid hormone-related protein (1-36)] and a parathyroid hormone-related protein anta…

Transient rise in intact parathyroid hormone concentration after surgery for primary hyperparathyroidism.

Two-site immunometric assays for determination of the level of intact parathyroid hormone (PTH) in serum have enabled detailed clinical studies of PTH secretion. Fifty-one patients with primary hyperparathyroidism were studied before surgery and 2 years after operation. PTH levels dropped to low values after operation. A rebound increase in PTH concentration occurred from 10 days to 3 months after surgery. Abnormally raised postoperative PTH concentrations were noted in 31 per cent of the patients, who had lower serum concentrations of ionized calcium (Ca2+) than the rest (P < 0.003). Before o…

Protamine and acute depletion of magnesium limit bone response to parathyroid hormone.

The effect of protamine on calcium homeostasis was studied in nine pediatric patients undergoing cardiopulmonary bypass. Total serum calcium decreased from 8.44 mg/dL to 7.49 mg/dL (P < 0.05) after protamine. Ionized calcium decreased from 1.39 to 1.31 mmol/L (P < 0.05). A bioassay determined the etiology of this response. Bone disks were placed in sera, protamine, parathyroid hormone, parathyroid hormone antibody, or magnesium-depleted solutions, then were incubated in solutions with known calcium content. The change in the media's calcium concentration reflects the bone's response to the ini…

[Diagnostic significance of parathyroid hormone-related protein in tumor patients with hypercalcemia].

The clinical utility of measuring parathormone-related protein (PTHrP) was investigated in 94 patients (48 men, 46 women, mean age 56 [18-82] years) with tumour-associated hypercalcaemia, using a radioimmunoassay directed against the middle portion (53-84) of PTHrP (reference range < 5-21 pmol/l). Increased levels of the middle portion of PTHrP were found in 44 of the 73 patients (60%) with hypercalcaemia associated with solid tumours (median 49 [22-333] pmol/l). There was a positive correlation between the serum calcium concentration and the PTHrP (P = 0.018). The frequency of a raised PTHrP,

The relationship between serum intact parathyroid hormone and calcium in idiopathic hypoparathyroidism.

We studied the relationship between serum calcium (Ca) and parathyroid hormone (PTH) in patients with idiopathic hypoparathyroidism (IHP) to investigate the diversity of the degree of parathyroid insufficiency and a regulation of PTH secretion by Ca in IHP. Serum intact PTH concentrations ranged from < 1.0 to 10.3 pg/ml in 20 patients with IHP and were above the detection limit in 18 (90%) patients. Serum total Ca levels in IHP were positively correlated with serum intact PTH (r = 0.824, P < 0.001), in contrast with a negative correlation between serum intact PTH and Ca in 91 normal subjects (…

Parathyroid hormone and calcitriol changes in normal and insulin-dependent diabetic pregnancies.

In pregnancy, an increase in serum calcitriol and parathyroid hormone concentrations has been reported in several studies, though the increase in parathyroid hormone remains controversial. In magnesium deficiency states, parathyroid hormone and calcitriol secretion may be decreased. Because magnesium deficiency may occur in insulin-dependent diabetic patients, mainly because of urinary magnesium losses, we hypothesized that serum parathyroid hormone and calcitriol do not increase in the diabetic pregnancy. We studied, in a prospective longitudinal manner, 35 nondiabetic and 199 insulin-depende…

Insulin secretion in uremia: effect of parathyroid hormone and vitamin D metabolites.

Insulin resistance is almost universal among uremic patients. There are, however, two different subgroups of uremic patients with regard to … evidence that hyperparathyroidism or vitamin D deficiency may inhibit insulin secretion in uremia. Parathyroid hormone (PTH) concentrations correlated inversely with glucose tolerance in adolescents with chronic renal insufficiency. Insulin hypersecretion in response to glucose loads occurred only in uremic patients with normal PTH concentrations. Surgical correctio …

Chronic renal failure, parathyroid hormone and fatty acids oxidation in skeletal muscle.

Fatty acids are an important source of skeletal muscle energy, and certain data suggest oxidation of long-chain fatty acids (LCFA) may be impaired in uremia. This abnormality may in part be responsible for uremic myopathy. Uremia is associated with hyperparathyroidism and PTH affects muscle metabolism; PTH enhances muscle proteolysis and impairs muscle bioenergetics, and it is possible that PTH also affects fatty acids oxidation. The present study examined in rats the effects of 4 days administration PTH and of 21 days of chronic renal failure (CRF) with and without excess PTH on oxidation of

Parathyroid hormone-related protein purified from a human lung cancer cell line.

A protein with biological activities similar to parathyroid hormone (PTH) has been purified from serum-free culture medium obtained from a human lung cancer cell line (BEN). A major protein band of 18 kDa was obtained on NaDodSO4/polyacrylamide gels, with faint bands at 35 kDa and 67 kDa. Biological activity was associated only with the 18-kDa band. Amino acid sequence analysis of the material purified by HPLC revealed that 8 of the 16 residues were identical with those of human PTH. Antibody raised to a corresponding synthetic peptide recognized the PTH-related material but showed less than 1…

[Sensitivity of bone to parathyroid hormone in type I pseudohypoparathyroidism. 6 cases].

The sensitivity of bone to parathormone in pseudoparathyroidism is not well known. Six patients with Type I pseudohypoparathyroidism (4 with Albright's osteodystrophy) had increased alkaline phosphatase levels (5 patients) and radiological signs of periosteal resorption in the hand in one case. All patients had histological signs of increased surfaces of resorption and periosteocytic lacunae, increased osteoid surfaces and relative osteoid volume with no change of the index of osteoid thickness. These changes are identical to those observed in hyperparathyroidism which leads on to the discussi

Calcium potentiates the cyclic nucleotide and phosphaturic response to parathyroid hormone infusion.

To determine whether calcium modulates the action of PTH, we measured the cyclic nucleotide and phosphaturic response to PTH following a 4-h infusion of glucose (day 1) and calcium (day 2). The 12 subjects were selected to provide a range of low, normal, and high endogenous PTH function. PTH stimulated nephrogenous cAMP [185 +/- 31 nmol/100 ml glomerular filtrate (GF)], cyclic guanosine monophosphate (0.44 +/- 0.09 mumol/g creatinine), and phosphate (367 +/- 59 mg P/g creatinine) excretion. Calcium infusion stimulated nephrogenous cAMP excretion in the hypoparathyroid subjects (1.42 +/- 0.35 n

Adult-onset vitamin D-resistant osteomalacia with the unresponsiveness to parathyroid hormone.

Serum immunoreactive parathyroid hormone (PTH) levels were increased in a 50-yr-old man with a unique variant of adult-onset vitamin D-resistant osteomalacia, presenting with high phosphate clearance, mild hypocalcemia, and blunted hypercalcemic and phosphaturic responses to exogenous parathyroid extract. After normalization of his serum calcium level with a large amount of 1 alpha-hydroxycholecalciferol, a synthetic vitamin D analog, serum PTH levels and phosphate clearance returned to normal, and hypercalcemia as well as phosphaturia appeared when parathyroid extract was administered. Furthe…

Transient neonatal hypocalcemia, hypomagnesemia, and high serum parathyroid hormone with maternal hyperparathyroidism.

Findings in a case of neonatal tetany in a child whose mother had elevated blood parathyroid hormone levels because of a parathyroid adenoma are given. The child was found to have hypocalcemia, hypomagnesemia, and hyperphosphatemia. Parathyroid hormone in the infant was found to be elevated. It has been postulated that the pathophysiologic mechanism of the hypocalcemia in this syndrome is transient hypoparathyroidism. The present case suggests a different mechanism: hypomagnesemia and end-organ refractoriness to parathyroid hormone, a state analogous to pseudohypoparathyroidism.
