NK cell lymphoblastic lymphoma in the masticator space: a case of non-Hodgkin lymphoma with challenging maxillofacial manifestation and immunophenotype.

Clinical and pathologic findings in extranodal non-Hodgkin lymphoma (NHL) often raise challenging problems in diagnosis. We demonstrate the first established case of lymphoma with precursor natural killer (NK) cell origin in the oral and maxillofacial region. An 81-year-old Japanese man had an enlarged facial mass mainly in the parotid region but hyposensitivity in the affected side of the mental and lingual region, and diagnostic imaging revealed the origin to be in the masticator space and indicated an advanced-stage lymphoma. The tumor cell immunophenotype was CD56(+)CD3(-)CD4(-), with no B

Oncocytic sialolipoma of the submandibular gland.

Sialolipoma, a rare tumor of the salivary gland, is a recently described variant of salivary gland lipoma. Oncocytic sialolipoma was first described by Pusiol et al. in 2009. We report the case of an oncocytic sialolipoma of the submandibular gland in a 43-year-old female. Excision of the tumor was performed with preservation of the submandibular gland. The tumor had a thin, fibrous capsule and consisted of abundant adipose tissue, an oncocytic nodule, and scattered normal glandular structures surrounded by adipose tissue. Four cases of sialolipoma of the submandibular gland, including the pre

Mammary analogue secretory carcinoma mimicking salivary adenoma.

Mammary analogue secretory carcinoma (MASC) is a recently described salivary gland tumor characterized by ETV6 translocation. It appears that prior studies have identified MASC by reviewing salivary gland carcinomas, such as acinic cell carcinoma and adenocarcinoma, not otherwise specified. To address the possibility of MASC mimicking benign salivary neoplasms we reviewed 12 salivary gland (cyst)adenomas diagnosed prior to the discovery of MASC. One encapsulated (cyst)adenoma of the parotid gland demonstrated features of MASC. The diagnosis was confirmed by fluorescence in situ hybridization w

Malignant inflammatory myofibroblastic tumor of the maxillary sinus.

Inflammatory myofibroblastic tumors (IMTs) are extremely rare neoplasms with a variable natural history and biologic behavior, ranging from completely benign to malignant tumors with fatal outcome. They have no common identifiable cause, although some authors have assumed that any inflammatory stimulus may cause these pseudotumors. They are most commonly found in the lungs. Extrapulmonary sites include abdomen, retroperitoneum and extremities. IMTs rarely affect the head and neck, but the most common subsites in this region include the orbit, larynx, mouth, tonsils, parapharyngeal space, thyro

Clonal relationship of extranodal marginal zone lymphomas of mucosa-associated lymphoid tissue involving different sites.

Patients with mucosa-associated lymphoid tissue (MALT) lymphoma often have multiple sites of disease at initial diagnosis or during the clinical course. The neoplasms at multiple sites are often presumed to be identical, indicating dissemination or relapse. However, evidence to support this presumption is usually not available. We compared IGH VDJ sequences in 4 patients with 2 sequential sites of MALT lymphoma. The specimen pairs were stomach and nasopharynx, stomach and lung, ocular adnexa and nasopharynx, and ocular adnexa and parotid gland. The median interval between biopsies was 4 months

Warthin's tumor at the Brazilian National Cancer Institute: additional evidence of homogeneous sex prevalence and association with other neoplasms.

PURPOSE OF THE STUDY: To report the largest Brazilian series of Warthin's tumor (WT). PROCEDURES: The medical files of 76 patients with WT treated in the Brazilian National Cancer Institute from 1996 to 2006 were reviewed. RESULTS: The male:female ratio was 2:1, with a predominance of white, old, and smoking patients. However, there were more women among the nonsmokers. One fifth of the patients presented with synchronous multiple lesions. Parotid lesions were prevalent, but there were patients with lesions in cervical lymph nodes and in the inferior lip. Most cases were treated by superfici

[Salivary gland neoplasms in the years 1991-2006 in the material of the Department of Otolaryngology, Medical University in Bialystok].

Complex structure of salivary glands, histological diversity of malignant salivary gland neoplasms and heterogeneous clinical image make therapeutic strategy difficult and controversial. The aim of this work was an epidemiologic analysis of 86 salivary gland tumour cases (in years 1991-2000) and 82 cases treated at the Otolaryngological Clinic of the Medical University of Bialystok (in years 2001-2006). Epidemiologic researches were conducted retrospectively. Age, sex, histological structure, location and local progression of tumour were analysed. The relationship between histological type, lo

Acinic cell carcinoma on the lower lip resembling a mucocele.

A 64-year-old woman presented with a 2-week history of an asymptomatic mass involving the lower lip. Histopathological examination showed a well-circumscribed tumour composed of many lobules separated by thin, fibrous connective tissue. Individual lobules were composed of round or polyhedral tumour cells, which had a characteristic finely granular and vacuolated cytoplasm and eccentric hyperchromatic nuclei. Positive staining was observed with Periodic acid-Shiff, and immunohistochemistry for cytokeratin, alpha-1 antitrypsin, and S-100 protein resulting in a final diagnosis of acinic cell carc

Unusual presentations of lymphoma of the head and neck in childhood.

Lymphoma of the head and neck in children can pose a significant diagnostic problem, especially when histologic analysis indicates non-Hodgkin's lymphoma and the initial site of involvement is extranodal. This report describes 15 pediatric cases of lymphoma seen from 1981 to 1987 with an initial presentation in the head and neck. Cervical lymph nodes represented the initial site of involvement in 10 of the cases. The other five cases presented with disease in the tonsillar fossa; maxillary sinus and mandible; parotid; pharyngeal wall; trachea and thyroid gland; and ethmoid sinus, sphenoid sinu

The neck mass. 2. Inflammatory and neoplastic causes.

Several inflammatory processes can cause nodules or swelling in the neck. A complete physical examination and, usually, laboratory testing are required to establish the diagnosis. Common infections include cervical lymphadenitis and tuberculous lymphadenitis, cat-scratch disease, infection in the neck spaces, infectious mononucleosis, and syphilis. Primary or metastatic cancer may also be the cause. Cervical metastasis often presents as a neck mass. Although a primary tumor may not be found immediately when a neck mass is being evaluated, one is often discovered later. Other types of malignanc

Mandibular Metastasis from Undiagnosed Hepatocellular Carcinoma - A Case Report.

RATIONALE: Cancer metastatic to the oral cavity accounts for 1%-4% of all oral malignancies, spreading from a distant primary site. Nevertheless, oral metastasis is often the first presentation of primary cancer. PATIENT CONCERNS: A 53-year-old male was referred to our institution because of painful swelling of the left parotid region that did not respond to medications. A firm mass extending towards the mandbular angle had caused trismus; furthermore, the patient complained about hypoesthesia in the V3 area. No noticeable findings could be seen intraorally. DIAGNOSIS: Computed tomography im

Ancient schwannoma in the parotid gland: A case report and review of the literature.

Schwannomas are uncommon neoplasms of neurologic origin that are rare in the salivary glands. A schwannoma that persists for a long time is referred to as an ancient schwannoma if it is accompanied by degenerative changes on histology. The case described herein involved a 37-year-old man with an ancient schwannoma that had persisted for 20 years in his right parotid gland. Clinically, the lesion presented with swelling and pain. Computed tomography revealed a well-defined, multilocular enhanced lesion. T2-weighted magnetic resonance images displayed multilocular hyperintensity, while T1-weight

Endoscope-assisted transoral approach to parapharyngeal space tumors.

BACKGROUND: Parapharyngeal space (PPS) tumors are rare tumors for which the surgical management differs widely. The purpose of this study was to discuss the advantages and disadvantages of the endoscope-assisted transoral approach over the traditional approaches. METHODS: This prospective study included 4 patients who were diagnosed with PPS tumors and treated with the endoscope-assisted transoral approach. RESULTS: The endoscope-assisted transoral approach described here safely allowed for the resection of benign neoplasms with high surgical view to the parapharyngeal neurovascular structur

Lymphoepithelial-like carcinomas of the head and neck.

Lymphoepithelial-like carcinomas (LELC) of the head and neck represent malignant neoplasms that are histologically similar to nasopharyngeal carcinoma (NPC), nonkeratinizing undifferentiated type but arise in locations other than the nasopharynx. The most common location for LELC in the head and neck is the salivary glands, in particular the parotid gland. However, LELC may arise in other sites including the oropharynx (tonsils, base of tongue), sinonasal tract, larynx, and middle ear/temporal bone. Unlike the nonkeratinizing undifferentiated type of NPC which is etiologically linked to Epstei

Palatal plasmacytoid myoepithelioma.

Myoepitheliomas are benign neoplasms of salivary glands derived from myoepithelial cells. These tumors can occur at any age but are most common in young adults. This tumor is usually located in the parotid gland and the minor salivary glands of the soft palate and represents less than 1% of all salivary gland tumors. The myoepithelioma is classified in the follow cells types: spindle, plasmacytoid, reticular, epitheliod, and clear, additionally, mixed histological forms are described. The plasmacytoid myoepithelioma from palate salivary glands is considered as a rare entity. A 45-year-old lady

Perineurioma of the parotid gland: first case report.

Neurogenic neoplasms account for approximately 1% of salivary gland tumors. A 45-year-old woman presented with a slowly growing unilateral parotid gland mass that was excised completely by lateral parotidectomy. No recurrence was detectable 6 months after surgery. The resection specimen contained a 1.7 × 1.2 × 1-cm whitish nodular lesion that was firm, well circumscribed, and completely surrounded by compressed normal glandular tissue. The tumor was composed of spindle cells with bipolar tapering, wavy nuclei, and palely stained cytoplasm. The tumor cells were arranged in storiform, lamellar,
