Horizons in therapy for corneal angiogenesis.

Corneal neovascularization can lead to a devastating disease process that involves the breakdown of the limbal barrier and the formation of blood vessels in the cornea, leading to severe visual impairment. This review discusses the delicate balance between antiangiogenic and angiogenic factors that govern the antiangiogenic privilege of the cornea. Current treatment methods, clinical trials, and future prospects in the management of corneal neovascularization also are discussed.

Endoscopic treatments for Barrett's esophagus: a systematic review of safety and effectiveness compared to esophagectomy.

BACKGROUND: Recently, several new endoscopic treatments have been used to treat patients with Barrett's esophagus with high grade dysplasia. This systematic review aimed to determine the safety and effectiveness of these treatments compared with esophagectomy. METHODS: A comprehensive literature search was undertaken to identify studies of endoscopic treatments for Barrett's esophagus or early stage esophageal cancer. Information from the selected studies was extracted by two independent reviewers. Study quality was assessed and information was tabulated to identify trends or patterns. Result

Effects of photodynamic therapy on the structural integrity of vertebral bone.

STUDY DESIGN: This study investigates the effects of photodynamic therapy (PDT) on the structural integrity of vertebral bone in healthy rats. OBJECTIVE: To determine the short-term (1 week) and intermediate term (6 weeks) effects of a single PDT treatment on the mechanical and structural properties of vertebral bone. SUMMARY OF BACKGROUND DATA: Spinal metastasis develops in up to one-third of all cancer patients, compromising the mechanical integrity of the spine and thereby increasing the risk of pathologic fractures and spinal cord damage. PDT has recently been adapted to ablate metastati

A patient with neviod basal cell carcinoma syndrome treated successfully with photodynamic therapy: case report and review of the literature.

Nevoid basal cell carcinoma syndrome (NBCCS) is a genetic disorder characterized by multiple basal cell carcinomas (BCCs) in addition to skeletal abnormalities and other neoplasms. Difficulty lies in treating the large number of BCCs that develop in these patients. The authors report a case of a patient with NBCCS treated successfully with photodynamic therapy (PDT) using 20% 5-aminolevulinc acid and blue light. The authors also review the literature and summarize past case reports and series using PDT to treat patient with NBCCS. Based on their experience and the reports of others, the author

Vitiligo therapy.

Vitiligo can be treated using various new and experimental therapies, such as narrow-band ultraviolet B microphototherapy (NB-UVB), narrow-band ultraviolet B excimer laser and monochromatic excimer light. The first is recommended for generalized vitiligo, whereas the latter two are indicated in patients affected by segmental and bilateral symmetrical vitiligo with < 20% cutaneous involvement. Medical treatments include topical corticosteroids and other topical treatments, such as antioxidants, tacrolimus and pimecrolimus, prostaglandin E, and vitamin D derivatives. Excellent therapeutic result

Methyl aminolaevulinate photodynamic therapy for the treatment of hidradenitis suppurativa and pilonidal cysts.

Hidradenitis suppurativa (HS) and pilonidal cysts are chronic, inflammatory skin conditions involving apocrine gland-bearing skin. Treatment is limited and unsatisfactory. Photodynamic therapy (PDT) has been reported to be safe and effective in the treatment of several off-label skin conditions. We report the case of a 29-year-old man affected by HS and pilonidal cysts since the age of 21. In the past, the patient was treated with antibiotics, corticosteroids and retinoids, without significant clinical improvement. Treatment with methyl aminolaevulinate (MAL)-PDT was started. A topical MAL cre

Dramatic clearance of a recalcitrant acral viral wart using methyl aminolevulinate-red light photodynamic therapy.

A 20-year-old man presented with a 3-month history of a viral wart located on his right thumb. Cryotherapy was administered weekly to the wart over 5 months without any significant improvement. A decision was made to treat the wart with red light photodynamic therapy (PDT) using topical methyl aminolevulinate (MAL) administered as three treatments 1-week apart. The patient was reviewed 4 weeks after the third treatment and the wart was found to have cleared completely. Three months after the last treatment, there remained no clinical evidence of recurrence of the wart. Pain was the main proble

Photodynamic therapy in dermatology--an update 2008.

Photodynamic therapy (PDT) is used for the prevention and treatment of non-melanoma skin cancer. Until recently, clinically approved indications have been restricted to actinic keratoses, nodular and superficial basal cell carcinoma, and--since 2006--Bowen disease. However, the range of indications has been expanding continuously. PDT is also used for the treatment of non-malignant conditions such as acne vulgaris and leishmaniasis, as well as for treating premature skin aging due to sun exposure. Here, PDT is used for the stimulation of immunomodulatory effects in contrast to the induction of

Photodynamic therapy of onychomycosis caused by Trichophyton rubrum.

A combination of topical and oral antifungals is widely used to treat onychomycosis, but treatment failure is common and oral drugs may cause toxicity and potential drug interactions. For this reason, new approaches and strategies should be considered. The following case shows that photodynamic therapy (PDT) may represent an alternative noninvasive approach to treatment of onychomycosis.

Performance of a dedicated light delivery and dosimetry device for photodynamic therapy of nasopharyngeal carcinoma: phantom and volunteer experiments.

UNLABELLED: The objective of this study was to develop a light delivery and measurement device for photodynamic therapy (PDT) in the nasopharyngeal cavity, which achieves a homogeneous and reproducible fluence rate distribution to a target area and provides proper shielding of predefined risk areas. MATERIALS AND METHODS: A flexible silicone applicator was developed, incorporating light delivery and dosimetry fibers. The applicator can be inserted through the mouth and fixed in the nasopharyngeal cavity. Tissue optical phantoms were prepared on the basis of optical properties measured in vivo

New photodynamic therapy protocol to treat AIDS-related Kaposi's sarcoma.

OBJECTIVE: The aim of this study was to evaluate the efficiency of photodynamic therapy (PDT) with phenotiazinium compounds (methylene blue and toluidine blue) and excitation by a non-coherent light source (RL50) to treat AIDS-related Kaposi's sarcoma (Sk-AIDS). BACKGROUND DATA: Sk-AIDS is a malignant disease that is recurrent in AIDS patients. Laser-based PDT protocols have been applied to treat Sk-AIDS with relative success. METHODS: A single patient with multiple lesions who had undergone chemotherapy without success was treated with several applications of PDT, and the patient was closel

Levulan: the first approved topical photosensitizer for the treatment of actinic keratosis.

Actinic keratosis (AK) is a very common skin problem found in patients over 50 years of age, representing an in situ keratinocytic neoplasm that can progress to invasive squamous cell carcinoma of the skin. One Food and Drug Administration-approved treatment for AK of the face and scalp is photodynamic therapy (PDT) with 20% aminolevulinic acid (ALA). This advanced technology has been demonstrated in clinical trials to be effective and well tolerated by patients.
