BACKGROUND: Allergic transfusion reactions (ATRs) are a common adverse reaction to transfusion therapy and can be potentially fatal. Washing blood products is the most effective strategy for preventing ATRs; however, washed products, especially platelets, are not available at many blood centers.
STUDY DESIGN AND METHODS: A 29-year-old female patient with an advanced myelodysplastic/myeloproliferative neoplasm, unclassifiable, developed severe ATRs after four platelet transfusions in a week. She showed no response to premedication with histamines and steroids and still had severe ATRs with the…

BACKGROUND: Extracorporeal membrane oxygenation (ECMO) is a mode of therapy initiated for patients with cardiac and/or respiratory failure. Blood transfusion plays a significant role in the management of ECMO patients. In this study, pediatric patients were all cyanotic and transfusion requirements of cyanotic patients differ from other critically ill patients. This study has been conducted to assess the daily transfusion requirements of such group of patients to frame future transfusion policies.
AIMS AND OBJECTIVES: This study was undertaken to assess the daily transfusion requirements of E

BACKGROUND/AIMS: Hemorrhagic complications during ECMO may affect a large proportion of the patients depending on the clinical setting. To guarantee optimal delivery of blood products to these patients, blood banks require updated information on the transfusion requirements. Few studies to date provide this information. This work assesses transfusion requirements in neonates and children during ECMO during the past 9 years.
METHODS: We reviewed blood bank and hospital records of patients who underwent ECMO at our institution between May 2003 and May 2012. Data obtained included age, weight, d

Platelets are a critical, cellular component of thrombosis and hemostasis. Too few platelets can lead to increased bleeding events, seen in conditions like immune thrombocytopenia. Therefore, understanding the mechanisms by which platelets are cleared from the body leads to strategies to maintain platelets in circulation, supplement patients with platelets through transfusions, or induce platelet production in vivo. Here, we discuss 2 major platelet clearance mechanisms, glycan recognition by hepatocytes and apoptosis, and how they can be modulated to improve patient outcomes.

To evaluate the parameters of the thrombin generation test (TGT) in coronary artery disease (CAD) patients on prolonged aspirin therapy during on-pump coronary artery bypass grafting (CABG) after donor platelet concentrate transfusion. A total of 148 patients with CAD on prolonged aspirin therapy (75-100 mg/day) who have undergone elective on-pump CABG were consecutively included in the study. Patients were divided randomly into two groups. Group 1 (n = 76) received donor platelet transfusions after cardiopulmonary bypass, whereas Group 2 (n = 72) did not. TGT parameters were measured using an…

The human population is ageing worldwide. The World Health Organization estimated that the world's population of people aged 60 years and older will increase to at least 30%, coinciding with a growing frequency of cognitive and cardiovascular disease. Recently, in preclinical studies platelet Factor 4 (PF4) was presented as a pro-cognitive factor. This molecule is released by platelets in the circulation and could be present in blood products destined for transfusion. We wondered if PF4 levels are correlated to the age of the blood donor or to the storage time of platelet concentrates (PCs) in

Transfusion of stored red blood cells (RBCs) is a life-saving intervention in trauma, surgery, and critical care. However, storage-induced alterations, collectively termed "storage lesions," lead to the accumulation of bioactive mediators, including mitochondrial DNA (mtDNA), which functions as a potent damage-associated molecular pattern (DAMP). mtDNA released from residual leukocytes, platelets, and extracellular vesicles during storage activates innate immune pathways via TLR9, cGAS-STING, and NLRP3 inflammasome signaling. This triggers neutrophil extracellular trap (NET) formation, monocyt

BACKGROUND: Hypotensive transfusion reaction (HyTR) is rare. It is characterized by a rapid onset of hypotension during transfusion, which usually resolves quickly upon cessation of transfusion. Information on the incidence and clinical characteristics of HyTR has been reported in only a few studies.
STUDY DESIGN AND METHODS: We retrospectively reviewed HyTR cases from 10-year hemovigilance data in a tertiary care hospital in Seoul, Korea.
RESULTS: We identified 37 HyTRs in 35 patients, and the overall incidence of HyTR was 0.50 per 10,000 transfused units. Among the blood components, the in

BACKGROUND: Several risk mitigation steps have improved the safety of platelets in regard to bacterial contamination, but this continues to be a concern today. A Food and Drug Administration (FDA) Guidance issued in December 2018 aims to further limit this risk. The guidance offers multiple pathways for compliance, and hospital blood banks will have to collaborate with blood donor centers to assess various factors before deciding which method is most appropriate for them.
METHODS AND MATERIALS: Our institution considered several factors before moving forward with pathogen reduction technology

BACKGROUND: Current guidelines advise against platelet transfusion prior to emergent esophageal variceal band ligation (EVL) in cirrhotic patients with platelet counts below 50 × 103/μL. However, recommendations for elective EVL remain unclear. This study evaluates the outcomes of cirrhotic patients undergoing outpatient EVL.
METHODS: Adult patients aged 18 years and older diagnosed with cirrhosis, with or without significant thrombocytopenia (<50 × 103/μL), were identified using the TriNetX database. Patients who received platelet transfusions within one week prior to or on the day of EVL we…

Blood transfusion and component therapy are an integral part of treatment for patients with certain life-threatening illnesses. Any benefit derived from transfusion must be weighed against the potential risk for complications. Patients who are chronically transfused present a unique challenge to practitioners. The presentation will focus on alloimmunization to red blood cell antigens, platelet refractoriness, transfusion-associated graft-versus-host disease, and cytomegalovirus infection. Prevention or management of these complications also will be discussed.

Recently, the leukocyte-derived cytokines interleukin (IL)-1 beta, IL-6, IL-8, and tumor necrosis factor(TNF)-alpha were found in considerable concentrations in platelet concentrates (PC) involved in nonhemolytic transfusion reactions (NHTR). We investigated transfusion reactions in two study periods over the last 3 years. In the 1st period, PC were leukocyte-depleted by bedside filtration. In the 2nd period we performed prestorage leukocyte filtration of PC. Recipients who experienced a transfusion reaction were tested for total and specific IgE, and reactions were analyzed with regard to the
