[Perifascicular necrosis and regeneration in a case of adult polymyositis].

We report a case of adult polymyositis with peculiar muscular pathology of innumerable muscle fiber necrosis and regeneration accentuated in the perifascicular area. A 51-year-old woman developed generalized weakness of the extremities, trunk and bulbar muscles subacutely for two months. Anterior tibial muscle biopsy showed numerous tube-like necrotic/regenerative muscle fibers predominantly in the perifascicular area. The diameters of the muscle fibers were smaller in the periphery of the fascicles. Small arteries at the center of the fascicles occasionally showed marked perivascular cuffing,…

Asymmetrical weakness in polymyositis associated with neuropathic involvement.

Peripheral nerve involvement is rare in polymyositis and dermatomyositis. We describe a patient in whom the clinical appearance was suggestive of a markedly asymmetrical presentation of polymyositis. Nerve conduction studies, however, were consistent with multiple neuropathies and suggest that neurological involvement may have been the basis of the asymmetrical weakness. Both the muscle weakness and the neurological abnormalities resolved with corticosteroid therapy. This case illustrates a previously undescribed neurological complication of polymyositis.

[A case of polymyositis associated with primary biliary cirrhosis].

We studied a 67-year-old female suffered from polymyositis associated with primary biliary cirrhosis. She was pointed out liver dysfunction by screening test. Alkaline phosphatase, transaminase, and IgM were increased. Antimitochondrial antibody and antinuclear antibody were positive. Liver biopsy showed cell infiltrations in Glisson's capsules and destruction of cholangioles, being diagnosed as primary biliary cirrhosis (Scheuer Stage I). Four years later she showed a muscle weakness of four extremities and admitted to our department. Neurological examination revealed a severe weakness and at…

[Acute circulatory disorders in the lower extremities in polymyositis].

A 32-year-old women with a definite diagnosis of polymyositis is described. The disease began acutely with two episodes of acute ischaemia of one lower extremity without evidence of organic occlusion of the arteries. Further on, signs predominated suggestive of deep thrombophlebitis of the lower extremities. The diagnosis was based on electromyography, histological examination of muscle biopsy material and rapid improvement after treatment with corticosteroids.

[An unusual association: polymyositis, myasthenia gravis and thymoma].

Based on the inexistence of references in Brazilian and International literature of the association polymyositis, myasthenia gravis and thymoma in a young male adult, the authors propose to describe one case presenting the associated syndrome. The disease began when the patient was 22 years old, being the climax of its clinical expression four years later. It is emphasized the difficulty on the differential diagnosis between polymyositis and myasthenia gravis, once these diseases frequently have their clinical manifestations well individualized. In this study we call attention to the existence…

[Anatomoclinical expressions of polymyositis in the child. 23 cases].

Childhood polymyositis is generally believed to exhibit specific characteristics including involvement of multiple systems, absence of malignancies, severe outcome, cortico-resistance, perforations of the bowel, and inflammatory vasculitis. On the basis of our experience with 23 cases and a review of the literature, we divide childhood polymyositis into five groups: 1) Severe acute dermatomyositis (11 cases) that fits the description above; 2) subacute or chronic polymyositis (4 cases) that resembles the chronic forms seen in adults; 3) infantile polymyositis (3 cases) whose inflammatory origi…

Polymyositis in pregnancy. A case report and literature review.

Polymyositis is a myositic disease that is rarely seen as a complicating condition of pregnancy. As with other connective tissue diseases, previously reported cases of polymyositis have been associated with increased perinatal mortality and maternal morbidity. We treated a patient in whom two pregnancies had been complicated by polymyositis. In the second of these gestations, aggressive management, including maintenance therapy with oral corticosteroids, contributed to a successful outcome.

Pathology of eosinophilic fasciitis and its relation to polymyositis.

The anatomical substrate of eosinophilic fasciitis (EF) was studied in 15 muscle biopsy specimens of this disease, six of which included the … of EF in the literature do not even describe muscle. A comparative study of 15 biopsy specimens of polymyositis and dermatomyositis with those of EF revealed only quantitative differences in the histopathological changes of muscle and mysia, inflammatory infiltrate and eosinophilia. We suggest that the diseases are more closely related than previously recogni …

Abnormalities of gastric and esophageal emptying in polymyositis and dermatomyositis.

Gastric and esophageal emptying were assessed using scintigraphic techniques in 13 patients with polymyositis or dermatomyositis and in 13 normal volunteers. Esophageal emptying was significantly delayed in patients, with 8 of 13 patients being outside the normal range. Gastric emptying was also markedly slower in patients than in controls, with 8 patients being outside the normal range for solid emptying and 8 patients beyond the normal range for liquid emptying. The 5 patients with dysphagia all had delayed esophageal emptying, but both gastric and esophageal emptying were delayed in some as…

Clinical and serologic features of patients with polymyositis or dermatomyositis.

The clinical and serologic features of 36 patients with polymyositis (PM) or dermatomyositis (DM) were observed over a 5-year period. The mean age of the patients at the time of diagnosis was 48.5 years, and 61% were female. According to widely accepted diagnostic criteria 50% had PM (group I), 14% DM (group II), 11% PM or DM associated with malignant disease (group III) and 25% PM or DM associated with a connective tissue disorder (group V). None of the patients had childhood PM or DM associated with vasculitis (group IV). All the patients had muscle weakness, and 94% of the patients tested h…
