Ryanodine receptor 2 mutations in catecholaminergic polymorphic ventricular tachycardia: From molecular mechanisms to precision medicine.

Catecholaminergic polymorphic ventricular tachycardia is a classic example of the successful transfer of genetic cardiology from gene discovery to implementation of precision medicine. This inherited arrhythmia syndrome induces potentially lethal ventricular arrhythmias by catecholaminergic stress in normally structured hearts and is most commonly due to ryanodine receptor 2 (RyR2) mutations in 60%-70% families. Pathophysiology involves gain-of-function mutations forming "leaky" calcium channels with increased sensitivity to catecholaminergic stimulation. Store overload-induced calcium release…

Precision Medicine in Neurodegeneration with Brain Iron Accumulation (NBIA) Disorders: An Update on Emerging Treatments.

BACKGROUND: Neurodegeneration with Brain Iron Accumulation (NBIA) is a heterogeneous group of heritable, mostly recessive, progressive neurodegenerative diseases characterized by iron deposition in the basal ganglia and brainstem. There are no solid global epidemiological data on prevalence and incidence of NBIA subtypes, but registry data and expert opinion suggest PKAN, BPAN, PLAN, and MPAN are the most common subtypes. NBIA disorders present with a wide spectrum of clinical symptoms, including movement disorders (dystonia, parkinsonism, chorea), pyramidal involvement (eg, spasticity), speec

Advances in body composition imaging for diagnosis and monitoring: From radiologic assessment to precision medicine.

Body composition (BC) is a key determinant of metabolic risk, treatment tolerance, and long-term outcomes. The distribution of skeletal muscle, visceral adipose tissue (VAT), and ectopic fat provides greater prognostic information than total body weight alone. However, conventional anthropometric measures, such as body mass index, cannot distinguish among different tissue compartments, including skeletal muscle, VAT, and ectopic fat, potentially obscuring clinically relevant phenotypes such as sarcopenia, myosteatosis, and sarcopenic obesity. This review addresses the "BMI paradox" by evaluati

Challenges and prospects of CYP2C19 genotype-guided clopidogrel therapy from the perspective of precision medicine.

In the era of precision medicine, dual antiplatelet therapy with aspirin and a P2Y12 receptor inhibitor is the standard strategy for patients after percutaneous coronary intervention (PCI). Clopidogrel, as a widely used representative drug, has its efficacy significantly influenced by CYP2C19 genetic polymorphisms, leading to poor metabolism in some patients, insufficient exposure to active metabolites, and increased risk of ischemic events. In theory, CYP2C19 genotyping to guide individualized antiplatelet therapy could help optimize the clinical selection of P2Y12 inhibitors and improve prog…

Brain heart crosstalk after ischemic stroke toward a unified framework for pathophysiology and precision medicine.

BACKGROUND: Acute ischemic stroke and cardiac dysfunction engage in a complex bidirectional interplay that critically determines patient outcomes. Despite growing recognition of the brain-heart axis, a comprehensive mechanistic framework integrating traditional and emerging pathways remains lacking. Moreover, existing reviews have predominantly focused on descriptive mechanisms without systematically evaluating the strength of evidence, addressing knowledge gaps, or proposing testable hypotheses for precision medicine. RECENT ADVANCES: This review systematically delineates the pathophysiology…

Precision Medicine in Pediatric Nephrology: From Shared Clinical Phenotypes to Genotype-Guided Diagnosis and Management.

Pediatric nephrology is shifting from broad phenotype-based labels toward molecularly defined, genotype-guided diagnosis and management. Childhood kidney disorders are enriched for monogenic causes, yet persistent microscopic hematuria, bilateral kidney cysts, steroid-resistant nephrotic syndrome, and thrombotic microangiopathy may represent shared endpoints of biologically distinct mechanisms. Using these four scenarios, this narrative review illustrates how structured phenotyping, pedigree analysis, biochemical evaluation, and appropriately selected genomic testing can establish etiology, re

The Inflammation-Thrombosis Genetic Axis in Abdominal Aortic Aneurysm: A Shared Roadmap to Precision Medicine.

Abdominal Aortic Aneurysm (AAA) is characterized by persistent inflammation, extracellular matrix loss, and intraluminal thrombus formation, yet the genetic links among these processes remain incompletely defined. Here, we examine evidence that genetic variation can influence inflammatory and thrombotic responses at the same time. Findings from genome-wide association studies are considered alongside transcriptomic, proteomic, single-cell, epigenetic, and experimental data. Particular attention is given to candidate genes and signaling networks relevant to AAA susceptibility, enlargement, and

Toward Precision Oral Medicine in Pemphigus Vulgaris: A Conceptual AI Framework for Rituximab Response Prediction.

Background/Objectives: Rituximab has improved outcomes in pemphigus vulgaris (PV), but treatment response remains heterogeneous and no clinically validated biomarker currently supports reliable individual-level prediction. Oral lesions frequently represent the earliest manifestation of PV and contribute substantially to disease burden and longitudinal assessment. This perspective aimed to synthesize candidate biomarkers and organize them within a conceptual, task-specific artificial intelligence (AI) framework for future rituximab outcome modeling. Methods: A structured literature search and n

Post-hepatectomy liver failure remains a major clinical challenge. Objective: This review summarizes metabolic mechanisms underlying liver regeneration and explores metabolic vulnerabilities in patients with HCC, along with emerging therapeutic strategies to enhance regenerative capacity. CYP2C19-Guided Voriconazole Therapy: A Precision Medicine Approach to Mitigate Adverse Effects in Japanese Patients.

Voriconazole (VRCZ) is a triazole antifungal agent with a broad antifungal spectrum. It is metabolized by hepatic cytochrome P450 (CYP) isozyme CYP2C19, whose genetic polymorphism causes significant variability in drug efficacy and safety. Poor metabolizer alleles of CYP2C19 are more common in Asian populations, increasing the risk of supratherapeutic VRCZ levels. We have developed a novel nomogram based on CYP2C19 genetic polymorphisms. This study aimed to evaluate whether CYP2C19 genotype-guided VRCZ therapy reduces toxicity in Japanese patients. This retrospective study included 64 patients

Decoding neuroinflammation in Alzheimer's disease: a multi-omics and AI-driven perspective for precision medicine.

Alzheimer's disease (AD) is a common neurodegenerative disease, which is characterized by β-amyloid (Aβ) deposition, Tau hyperphosphorylation, synaptic dysfunction and chronic neuroinflammation. Despite significant advances in research in recent years, effective therapeutic options remain limited. The development of single-cell RNA sequencing (scRNA-seq) has made it possible to analyze cellular heterogeneity in AD brain tissues at high resolution, breaking through the limitation of signal averaging in traditional large-scale tissue analysis. This technology has led to the discovery of novel di
