This study evaluates the impact of the severity of myopia and the type of visual correction in presbyopia on vision-related quality of life (QOL), using the refractive status and vision profile (RSVP) questionnaire. A total of 149 subjects aged 41-75 years with myopic presbyopia were recruited: 108 had low myopia and 41 had high myopia. The RSVP questionnaire was administered. The article reviews the findings on the anatomy, morphological and biomechanical features of the accommodation apparatus. Modern methods of imaging and biometry confirm the validity of the Helmholtz lenticular theory of accommodation, according to which its mechanism involves three main components: the ciliary body, the zonular fibres and the lens capsule, the lens itself. Based on this, there is certain interest in studying the degree of participation of each of these components in the development of age-related changes in accommodation (presbyopia).

An analysis of patients presenting with headache shows that in very few cases is the headache related to refractive error. Similarly in presbyopia and hypermetropia headaches are infrequent. A significant proportion (up to 50%) of those patients presenting with a close relationship of headache to accommodation difficulty can be helped by glasses. It is postulated that ciliary muscle contraction per se is effortless and symptomless and that any headaches produced are due to associated contraction of the scalp muscles.

BACKGROUND: According to the Helmholtz theory of accommodation the loss of accommodation amplitude is caused by the growing sclerosis of the crystalline lens, whereas the ciliary muscle and the lens capsule are mainly uneffected by age. The genetic and environmental contributions to physical aging (hair graying, balding, presbyopia) and longevity (age at death) were examined by within-pair comparison in monozygotic (MZ) and dizygotic (DZ) twins in later adulthood.
