The source of messages produced during facilitated communication with a boy with autism and severe mental retardation: a case study.

Assessed the source of messages produced through facilitated communication with a youth with autism and severe mental retardation using message-passing and picture-naming procedures. Results indicated that the facilitator was the source of the communications produced during the assessment; correct responding occurred only when the facilitator had knowledge of the correct answer. These findings are consistent with the results of other research demonstrating cuing, influence, or control of facilitated communications by the individuals providing assistance (facilitators). Implications for ethical

[Sexuality in acute psychiatric departments].

Sexual behavior of patients in psychiatric admission wards is still only seldom spoken about, despite its relatively frequent occurrence in various forms and the emotional impact it bears on the psychiatric staff. The sexual issues have to be discussed with every patient. Among other things, the patients should be provided with information and the communicative meaning of his or her sexual manifestations should be elucidated and worked through psychotherapeutically. In the ward, a prohibitive attitude toward genital sexuality is justified; otherwise a flexible attitude should be assumed, which

[Care in stillbirth; mothers' opinion].

Semi-structured interviews were conducted with 49 women who had experienced stillbirth in 1987-1989. Only 55% were satisfied with the manner in which the first suspicion had been communicated to them, while 85% were satisfied with the communication at the time of the diagnosis. Opinions about care in the labour ward, while largely positive, indicate that lack of tact from an individual may cloud the entire perception of care. Although only 40% had expressed a desire to see the baby, the others were happy to have been persuaded to do so and all were positive about the contact with the dead baby

[Psychotherapy and culture].

This is a follow-up study of a group of patients of Foreign origin for which an indication for analytic psychotherapy was found. The importance of mutual therapist-patient recognition is emphasized. It seems necessary that the patient masters the cultural and socio-economic environment in which he lives, in order to open himself up to an intra-psychic reality. Finally, we formulate the hypothesis that symptoms shown by the patient reveal indirectly adjustment disorders to the cultural environment.

Imposed termination of psychotherapy and its relation to death and mourning.

For the psychotherapist unilaterally to impose a termination date to therapy is a manoeuvre not often discussed; but with some patients it may be the only way of forcing critical issues into the open. The patient's position is then analogous to that of a bereaved mourner or someone with a terminal illness, and a sequence of responses can be observed identical to that which occurs in those cases. A detailed clinical example illustrates this. There appears to be a specific developmental task common to all such situations which is called forth by them. Subjecting a patient to this is a drastic me

Narcissism and adaptation to indignity.

Having initially briefly reviewed and then compared and contrasted the contributions of Kernberg and Kohut to the theory and treatment of narcissistic problems, the attempt was then made to evaluate critically their merits and possible short-comings. It was felt that in pushing back the frontiers of pathology to earlier and earlier developmental phases there has been an associated tendency to postulate "essentially preverbal unconscious communication' between therapist and patient where empathy tends to minimize or replace free association and other secondary process communication as the analy

[Outsiders in their own home: patients in a nursing home unit].

During a term of probation for the doctoral course in sociology at the university of Nijmegen a research was made for the relatinship between patients and staff on the ward of a nursing home. This relation was investigated with the use of Elias and Scotson's model of an established and outsiders figuration. In this figuaration are several sources of power, as fysical capacities, knowledge, key-positions and cohesion, important. These are considered more thoroughly. Some of the central problems of a nursing home are becoming more clear in the elaboratin of processes as exclusion and stigmatizat

The needs of parents with children suffering from lethal epidermolysis bullosa.

BACKGROUND: Some subtypes of the heterogeneous genetic blistering disease epidermolysis bullosa (EB) lead to lethality in childhood. The severity and extent of blistering leaves these patients living in excruciating pain and distress their entire lives. Parents of these patients experience some specific problems, such as the unfamiliarity of EB amongst healthcare professionals and the suffering and loss of their child. OBJECTIVE: To identify the needs of parents who have lost their child to lethal EB. METHODS: A qualitative study was performed, comprising semistructured, in-depth interviews

Identifying opportunities to support patient-centred care for ductal carcinoma in situ: qualitative interviews with clinicians.

BACKGROUND: Women with ductal carcinoma in situ (DCIS) report poor patient-clinician communication, and long-lasting confusion and anxiety about their treatment and prognosis. Research shows that patient-centred care (PCC) improves patient experience and outcomes. Little is known about the clinician experience of delivering PCC for DCIS. This study characterized communication challenges faced by clinicians, and interventions they need to improve PCC for DCIS. METHODS: Purposive and snowball sampling were used to recruit Canadian clinicians by specialty, gender, years of experience, setting, a

Communication patterns in audiologic rehabilitation history-taking: audiologists, patients, and their companions.

OBJECTIVES: The nature of communication between patient and practitioner influences patient outcomes. Specifically, the history-taking phase of a consultation plays a role in the development of a relationship and in the success of subsequent shared decision making. There is limited research investigating patient-centered communication in audiology, and this study may be the first to investigate verbal communication in an adult audiologic rehabilitation context. This research aimed, first, to describe the nature of verbal communication involving audiologists, patients, and companions in the his

Patient-provider communication and low-income adults: age, race, literacy, and optimism predict communication satisfaction.

OBJECTIVE: To assess whether literacy, numeracy, and optimism are related to low-income adults' satisfaction with their healthcare provider's communication skills. METHODS: Low-income adults (N=131) were recruited from seven counties in Indiana through University extension programs. To achieve research triangulation, participants were surveyed and interviewed about their communication satisfaction with health providers. RESULTS: Survey data revealed that four variables significantly predicted satisfaction: age, race, literacy, and optimism. Low-income adults in the current study were more cr

Expressed emotion in staff-patient relationships: the professionals' and residents' perspectives.

BACKGROUND: Expressed emotion (EE) is a well-established, important predictor of the relapse rate of patients suffering from schizophrenia and other severe psychiatric disorders. EE measures the quality of the social interaction between a patient and his most important (in)formal caregiver. The aim of this study was to investigate the quality of the relationship in the staff-patient dyad as measured by the concept of EE. METHODS: EE was assessed using the Camberwell Family Interview (CFI, professionals) and the Perceived Criticism Scale (PCS, residents and professionals form) in a sample of 5

Helping patients overcome their fear of mammography.

Research shows that between 50% and 60% of U.S. women do not follow established guidelines on mammography. This article examines some of the reasons women cite for not undergoing the examination. These reasons include cost, absence of breast cancer symptoms and lack of family history of breast cancer. Women also express fear of breast compression, radiation and an unfavorable diagnosis. These fears are a challenge to the professional mammographer, because imaging an uncooperative or apprehensive patient may result in suboptimal films or repeats. This article suggests ways mammographers can hel

Burnout among healthcare professionals in high-emotion work environments: A qualitative study from the burn unit in Albania.

This study explores the experiences of burnout among physicians and nurses working in the Burn Unit at the University Hospital Center "Mother Teresa" in Tirana. Using a qualitative case study methodology, ten semi-structured interviews were conducted with professionals who have over five years of experience in this service. Thematic analysis, following a template analysis approach, identified four main themes: ongoing emotional and physical workload, lack of institutional support, emotional connection with patients, and individual coping strategies. The findings show that burnout in this conte

Love Yourself as a Person, Doubt Yourself as a Therapist?

OBJECTIVE: There are reasons to suggest that the therapist effect lies at the intersection between psychotherapists' professional and personal functioning. The current study investigated if and how the interplay between therapists' (n = 70) professional self-reports (e.g., of their difficulties in practice in the form of 'professional self-doubt' and coping strategies when faced with difficulties) and presumably more global, personal self-concepts, not restricted to the professional treatment setting (i.e., the level of self-affiliation measured by the Structural Analysis of Social Behaviour (

Potential clients' beliefs about the relative competency and caring of psychologists: implications for the profession.

To assess the extent to which the results reported by Warner and Bradley (1991) can be generalized beyond the population of undergraduate psychology students, 132 adults in the metropolitan Harrisburg area were asked to evaluate the competency and traits of clinical psychologists, licensed professional counselors, and psychiatrists. Results indicated that the general public believed counselors to be more caring than psychologists and psychiatrists. All three professional groups were perceived to be comparable in their ability to treat the least severe disorders. Licensed counselors were percei

Self-reported halitosis and emotional state: impact on oral conditions and treatments.

BACKGROUND: Halitosis represents a common dental condition, although sufferers are often not conscious of it. The aim of this study was to examine behavior in a sample of Italian subjects with reference to self-reported halitosis and emotional state, and specifically the presence of dental anxiety. METHODS: The study was performed on Italian subjects (N = 1052; range 15-65 years). A self-report questionnaire was used to detect self-reported halitosis and other variables possibly linked to it (sociodemographic data, medical and dental history, oral hygiene, and others), and a dental anxiety sc

Timing of the medical examination.

The medical examination of the sexually abused child may have evidentiary, medical, and therapeutic purposes, and the timing of the examination requires consideration of each of these objectives. In cases of acute sexual assault, emergent examinations may be needed to identify injury, collect forensic evidence, and provide infection and pregnancy prophylaxis. Alternately, most sexually abused children are not identified immediately after assault, and the timing of the examination needs to balance physical and emotional issues with the availability of qualified examiners. In all cases, the best
