Status of COPD in Saudi Arabia: challenges, gaps, and opportunities for health system transformation.

Chronic obstructive pulmonary disease (COPD) is a serious public health problem and unfortunately, policies and health system structures in Saudi Arabia remain inadequate to address its rising healthcare burden. This review aims to map and critically analyze the existing evidence on COPD management in Saudi Arabia and to propose a framework to guide clinical practice and improve patient outcomes. Globally and particularly in Saudi Arabia, COPD remains significantly underdiagnosed despite the availability of well-established clinical guidelines. Based on the current analysis, this is primarily

An Overview of Appropriate Medical Practice and Preparedness in Radiation Emergency Response.

Radiation emergencies involving high doses of nuclear radiation pose significant risks from exposure to ionizing radiation in various scenarios. These situations include transportation accidents involving radioactive materials, occupational exposure, nuclear detonations, dirty bombs, and nuclear power plant accidents. In addition to the immediate risks of acute radiation syndrome (ARS) and related diseases, long-term exposure can increase the risk of other health issues such as cardiovascular disease and cancer. Vulnerable populations, including pregnant women and children, face particular con

Artificial intelligence in dementia care: challenges, controversies, and policy implications.

Artificial intelligence (AI) is rapidly expanding into dementia-related health and social care, with proposed applications ranging from early risk detection and monitoring to care coordination and service planning. While these technologies may support independence, reduce caregiver burden, and improve efficiency in overstretched systems, dementia care is a uniquely high-stakes context for digital innovation. Cognitive decline can affect consent and agency, care often occurs in private domestic settings, and individuals may become increasingly dependent on others to interpret and act on algorit

Cancer research in South Asian Association for Regional Cooperation (SAARC) countries.

Cancer is a major global health threat, with 35 million new cases projected by 2050, predominantly in low-income and middle-income-countries. Within South Asian Association for Regional Cooperation (SAARC) countries, a notable gap in cancer research investment and output compared with high-income countries highlights the need to strengthen research capacity. The rising cancer incidence across SAARC countries is not being matched by local research, particularly in clinical trials in molecular biology, targeted therapy, immunotherapy, and cancer vaccines. This paucity of research is problematic

Leptospirosis-clinical review and updates on therapeutics.

Leptospirosis is a globally distributed zoonotic bacterial disease caused by Leptospira spp. Mortality and morbidity burdens are greatest in low-to-middle-income tropical countries, where frequent natural disasters and poor infrastructure drive outbreaks that disproportionately affect marginalised communities. Leptospirosis is a common but overlooked cause of acute undifferentiated fever. This neglect is augmented by limited diagnostics, insufficient treatment evidence, and the absence of a coordinated global health policy. Diagnosis requires clinical vigilance and timely testing, although acc

Recurrent outbreaks of cholera in Nigeria: a narrative review on the role of conflict and climate change.

Cholera remains a major public health problem in Nigeria, with recurrent outbreaks linked to weak water and sanitation services. We conducted a narrative review of published studies and reports and examined how conflict and climate variability contribute to cholera risk and how these drivers interact. Evidence shows that conflict increases cholera transmission mainly through population displacement, overcrowding, and the breakdown of water, sanitation and hygiene (WASH) systems, surveillance, and health services. Climate hazards, especially heavy rainfall, flooding, and drought, increase expos

A Protected Class, An Unprotected Condition, and A Biomarker - A Method/Formula for Increased Diversity in Clinical Trials for the African American Subject with Benign Ethnic Neutropenia (BEN).

Expanding on previous industry guidance relative to increased clinical trial diversity, while honing more exacting treatments and better ways to fight diseases that have often disproportionately impacted people of color, is a topic being discussed by multidisciplinary public health experts across the nation. This writing draws attention to the African American demographic, which is continually subject to health care disparities. Any glimpses of knowledge or medical discovery that could potentially help to redress harm or reinforce a weakened familial-cultural infrastructure should be emphasized

The effects of racism, social exclusion, and discrimination on achieving universal safe water and sanitation in high-income countries.

Drinking water and sanitation services in high-income countries typically bring widespread health and other benefits to their populations. Yet gaps in this essential public health infrastructure persist, driven by structural inequalities, racism, poverty, housing instability, migration, climate change, insufficient continued investment, and poor planning. Although the burden of disease attributable to these gaps is mostly uncharacterised in high-income settings, case studies from marginalised communities and data from targeted studies of microbial and chemical contaminants underscore the need …

Waterborne Infectious Diseases Associated with Exposure to Tropical Cyclonic Storms, United States, 1996-2018.

In the United States, tropical cyclones cause destructive flooding that can lead to adverse health outcomes. Storm-driven flooding contaminates environmental, recreational, and drinking water sources, but few studies have examined effects on specific infections over time. We used 23 years of exposure and case data to assess the effects of tropical cyclones on 6 waterborne diseases in a conditional quasi-Poisson model. We separately defined storm exposure for windspeed, rainfall, and proximity to the storm track. Exposure to storm-related rainfall was associated with a 48% (95% CI 27%-69%) incr

Perceived Benefits and Challenges of Ebola Preparation Among Hospitals in Developed Countries: A Systematic Literature Review.

The 2014-2016 Ebola epidemic in West Africa provided an opportunity to improve our response to highly infectious diseases. We performed a systematic literature review in PubMed, Cochrane Library, CINAHL, EMBASE, and Web of Science of research articles that evaluated benefits and challenges of hospital Ebola preparation in developed countries. We excluded studies performed in non-developed countries, and those limited to primary care settings, the public health sector, and pediatric populations. Thirty-five articles were included. Preparedness activities were beneficial for identifying gaps in

Determinants Shaping Healthcare Experiences Among Vulnerable Populations in Hong Kong: A Qualitative Descriptive Study.

BackgroundIn many urban Asian cities, healthcare experiences are situated at the intersection of infrastructure-dictated living, multilingual and aging populations, and pluralistic medical practices. In Hong Kong, these dynamics are intensified by socioeconomic inequality and long wait times for subsidized care within the public-private healthcare system. Few qualitative studies have examined how social, environmental, and healthcare system factors shape the experiences of vulnerable populations in this context against a multilevel framework. The study explored these determinants and their imp

Antimicrobial resistance and social inequity: Why the one health approach must centre poverty.

Antimicrobial resistance (AMR) is one of the most consequential threats to global public health. In 2021, bacterial AMR was associated with an estimated 4.71 million deaths globally, including 1.14 million directly attributable to resistance. Yet, despite its designation as a global emergency, policy responses remain heavily focused on surveillance, antimicrobial stewardship, diagnostics, and pharmaceutical innovation. These interventions are necessary but insufficient without sustained action on the social and structural conditions shaping infection exposure, access to care, and antibiotic us
