Factors Contributing to Variability in Longitudinal Pain Scores in Osteoarthritis Randomised Clinical Trials: A Systematic Review and Meta-Analysis.

BACKGROUND AND OBJECTIVES: Osteoarthritis is a prevalent joint disease causing pain and functional decline, but high variability in patient-reported treatment outcomes in clinical trials challenges detection of treatment effects. This systematic review aimed to evaluate variability in pain reporting and identify trial characteristics, placebo comparator attributes, and participant clinical characteristics associated with this variability. DATABASES AND DATA TREATMENT: A systematic review of placebo-controlled RCTs for pharmacological interventions in knee osteoarthritis was conducted, adherin

Reporting Completeness of Usual Care Comparator Groups in Exercise-Based Trials for Knee Osteoarthritis: A Meta-Research Systematic Review.

BACKGROUND: Exercise-based randomized controlled trials are essential for guiding the management of knee osteoarthritis, but their interpretability depends on transparent reporting of both intervention and comparator groups. The term usual care (UC) is frequently used as a comparator, yet its reporting quality remains unclear. OBJECTIVE: To evaluate the reporting completeness of UC comparator groups in randomized controlled trials (RCTs) of exercise-based interventions for knee osteoarthritis. METHODS: This systematic review included RCTs of exercise-based interventions for adults with knee

Misleading at Best: A Critical Appraisal of "Nurse-Physician Substitution in Hospital Settings".

BACKGROUND: A recent Cochrane review concluded that nurse-led care results in little to no difference in outcomes compared with physician-led care in hospital settings. These findings have been widely interpreted as evidence supporting substitution of nurses for physicians. OBJECTIVE: To evaluate the studies included in the Cochrane review and determine whether the evidence supports independent nurse substitution for physicians in hospital settings. METHODS: We conducted a structured analysis of the 82 randomized controlled trials included in the review, examining clinical setting, patient a

The association between remnant cholesterol and cardiovascular events: A systematic review and meta-regression.

BACKGROUND: Despite extensive efforts to lower low-density lipoprotein cholesterol (LDL-C), cardiovascular disease remains highly prevalent. Remnant cholesterol (RC) has been proposed as a potentially more relevant therapeutic target. OBJECTIVE: To evaluate the association between RC reduction and cardiovascular outcomes in randomized controlled trials (RCTs) of cholesterol- and triglyceride-lowering therapies, assessing whether RC independently influences cardiovascular risk. METHODS: Cochrane Central and Embase were searched for RCTs with ≥1000 participants and ≥2 years' planned duration t

Interventions to Prevent Postinflammatory Hyperpigmentation After Laser and Energy-Based Device Treatments: A Systematic Review and Network Meta-Analysis.

BACKGROUND: Postinflammatory hyperpigmentation (PIH) is the most common adverse effect following laser treatments, yet the relative efficacy of proposed prophylactic measures remains uncertain. OBJECTIVES: To compare the effectiveness of available interventions for preventing laser-induced PIH in randomized controlled trials (RCTs). METHODS: PubMed, Embase, Scopus, Cochrane Library, and ClinicalTrials.gov were searched through February 2025. RCTs reporting PIH incidence after laser or other energy-based treatments were eligible. Stereotactic body radiotherapy for oligometastatic disease: current evidence and future perspectives.

Oligometastatic disease (OMD) represents an intermediate state between locoregionally advanced and widespread polymetastatic disease, where local therapy may alter disease progression and improve survival. Metastasis-directed therapy (MDT), particularly stereotactic body radiotherapy (SBRT), has been evaluated alongside systemic therapy, yet OMD definitions and criteria for selecting patients who benefit from local therapy remain inconsistent. This review summarizes recent clinical trials on SBRT for OMD across cancer types. The SABR-COMET trial suggested that MDT may improve survival in OMD a

Role of wound protectors in preventing surgical site infection in patients undergoing abdominal surgery: a meta-analysis of randomized controlled trials.

OBJECTIVE: To investigate the role of wound protectors (WPs) in surgical site infection (SSI) prevention in patients undergoing abdominal surgery. BACKGROUND: Despite practitioners having pursued the ultimate aim of "getting to zero" for centuries, SSI is still a global healthcare burden. WPs are thought to be able to prevent the incision from bacterial contamination, however, contradictory results have been reported. METHOD: A meta-analysis was performed to compare the SSI rate in patients with and without WPs after abdominal surgery. It includes subgroup analysis of the SSI rate in differe

Creatine supplementation for treating symptoms of depression: a systematic review and meta-analysis.

Nutraceuticals are increasingly of interest in nutritional psychiatry, where creatine has been investigated in several randomised trials for its effects on depressive symptoms. However, these findings have not yet been systematically synthesised. We conducted a systematic review to assess the effects of creatine supplementation on symptoms of depression. Four databases were searched up to February 2025 for trials comparing creatine with placebo in individuals with or without depression. Study selection, data extraction and risk of bias assessment (RoB 2) were conducted independently, and certa

Effects of sedatives on diaphragm activity monitored by ultrasound: a systematic review and meta-analysis.

BACKGROUND: Current research lacks comprehensive evaluations on the inhibitory effects of sedatives on diaphragm activity under ultrasound monitoring. This meta-analysis aims to launch this problem by systematically analyzing the available evidence. METHODS: The EMBASE, PubMed, and Web of Science databases were searched. Original studies that explored the effects of sedative agents on human diaphragm activity via ultrasound were eligible. The quality of the included studies was evaluated using the Revised Cochrane Risk-of-Bias tool for randomized trials (RoB 2) and the Newcastle-Ottawa Scale

Exploring the potential of acupuncture as a complementary treatment for Alzheimer's disease: Pushing the boundaries forward.

Acupuncture has emerged as a promising adjunctive symptomatic therapy for Alzheimer's disease, demonstrating potential benefits in cognitive function and neuroprotection. Recent meta-analyses and randomized clinical trials have investigated the potential of acupuncture to improve cognitive assessments, signaling pathways, and gut regulation in Alzheimer's disease, underscoring its potential clinical application. However, the limited number of studies, small sample sizes, and lack of detailed mapping of acupuncture's core targets must be considered when interpreting these positive results.

Risk of serious infection with JAK inhibitors in immune-mediated inflammatory skin diseases: a meta-analysis of randomized clinical trials.

BACKGROUND: Emerging research suggests that Janus kinase-signal transducer and activator of transcription (JAK-STAT) inhibitors may be associated with a higher risk of serious infection for patients with rheumatoid arthritis. However, there is no consensus on whether JAK inhibitors increase the risk of serious infection in patients with Immune-mediated inflammatory skin diseases (IMISDs). OBJECTIVES: To ascertain the correlation between JAK inhibitor use and the risk of serious infection in patients with IMISDs. METHODS: PubMed, Cochrane Central Register of Controlled Trials (CENTRAL), and r

Brief Drug Interventions Delivered in General Medical Settings: a Systematic Review and Meta-analysis of Cannabis Use Outcomes.

There is consistent evidence that brief interventions can be effective in preventing and reducing alcohol use, but support for their effects on illicit drug use is more limited. This meta-analysis expands prior research by testing whether brief drug interventions (BDIs) delivered in general medical settings reduce cannabis consumption and severity across post-intervention follow-up periods and explores potential heterogeneity in these effects. Findings from 17 randomized controlled trials were synthesized to compare short- and long-term cannabis use outcomes between intervention and control gr

Potential placebo bias in current trials of delirium prevention: a network meta-analysis of 86 randomized controlled trials.

AIM: Numerous network meta-analyses (NMAs) have reported inconsistent findings on the efficacy of various pharmacologic treatments for delirium prevention in high-risk patients. A potential confounder was the study design-using a placebo control or standard-of-care (SoC) control. We reexamined the incident delirium between SoC plus placebo and SoC alone in randomized controlled trials (RCTs) on delirium prevention. METHODS: We systematically searched for relevant RCTs on pharmacotherapies for delirium prevention from the inception of electronic databases through 30 November 2023. The primary

Elucidating some common biases in randomized controlled trials using directed acyclic graphs.

Although the ideal randomized clinical trial is the gold standard for causal inference, real randomized trials often suffer from imperfections that may hamper causal effect estimation. Stating the estimand of interest can help reduce confusion about what is being estimated, but it is often difficult to determine what is and is not identifiable given a trial's specific imperfections. We demonstrate how directed acyclic graphs can be used to elucidate the consequences of common imperfections, such as noncompliance, unblinding, and drop-out, for the identification of the intention-to-treat effect

Placebo Response in Acute and Prophylactic Treatment of Migraine: A Systematic Review and Meta-Analysis Covering 36 Years of Research.

The authors identified and meta-analyzed 84 acute and 42 prophylactic systematically selected RCTs on antimigraine drugs between 1988 and 2024, with a total of 25,542 placebo recipients, analyzed separately for episodic and chronic migraine in adults and children. Placebo response rates in adults were 13% for "Pain Free" and 35% for "Pain Relief" in acute RCTs, and a reduction of -2.83 "monthly migraine days" and -3.64 "monthly headache days" in prophylactic RCTs was observed. The "≥50% responder rate for monthly migraine days" was 28%. In children, placebo responses were almost twice as high.
