Auricular Acupuncture Effect on Facial Contraction via Middle Ear Muscles Reflexes.

IMPORTANCE: Auricular acupuncture (AA) regulates the function of systems or organs by stimulating specific regions of the body via efferent pathways in the central areas through the auricle. This case aims to investigate the causes of facial contractions observed during AA. CASE PRESENTATION: A female patient, diagnosed with depression and refusing medication, was referred for AA. She presented to our clinic with a complaint of right-sided facial contraction triggered by high-intensity sound during AA. CONCLUSIONS AND RELEVANCE: After evaluation, a decrease in acoustic reflex thresholds of 0

Mechanism of cyanotic spells in tetralogy of Fallot--the missing link?

The mechanism of cyanotic spells in patients with tetralogy of Fallot is not clear. Increases in infundibular contractility or hyperpnoea have been considered as the key factors, but this explanation appears inadequate. In this review, arguments are presented against these commonly held views. Preliminary evidence is synthesised in favour of another more plausible hypothesis that cyanotic spells may result from mechanoreceptor stimulation from the right ventricle. Increased contractility (due to catecholamines) and decreased right ventricular size (due to various factors) can trigger a reflex

Olivopontocerebellar atrophy presenting with hemiparkinsonian ocular motor signs.

A patient with olivopontocerebellar atrophy presented with an asymmetric parkinsonian syndrome consisting of right limb rigidity, bradykinesia, masked facies, and a hypophonic, monotonous voice. Right limb clumsiness suggested corticospinal tract involvement. Gait was slightly wide based, but no other cerebellar or brainstem signs were present. Eye movements were characteristic of parkinsonism with low-gain pursuit and hypometric rightward saccades. Saccadic velocity and the vestibular ocular reflex were within normal limits. There was no pathologic nystagmus or saccadic dysmetria. Magnetic re

Venous Outflow and Otosclerosis: An Unusual Case of Pulsatile Tinnitus.

We report resolution of right-sided pulsatile tinnitus in a 44-year-old male who underwent stapedectomy for fenestral otosclerosis. Initial workup revealed a mixed hearing loss and absent stapedial reflexes consistent with ossicular fixation. CT angiography demonstrated near complete stenosis of the left-sided transverse and sigmoid sinuses and dominant contralateral venous outflow. We hypothesized that the dominant right cerebral venous outflow tract created turbulent flow that was conducted to cochlea. Successful stapedectomy was performed, and the pulsatile tinnitus resolved. This case repo

Clinical Reasoning: A 51-Year-Old Woman With Diplopia and Headache.

A 51-year-old woman presented with a pressure-like headache behind her right eye and horizontal diplopia. On examination, she was unable to abduct or adduct the right eye but had intact vertical eye movements. Her deficits could not be overcome using the oculocephalic reflex. Imaging initially was interpreted as optic neuritis, but on careful review with radiology, a diffuse enhancing hyperintense signal within the orbital apex confirmed an orbital infiltrate. The focus of this case study is to review the localization approach for diplopia and build a differential diagnosis for orbital process

Brainstem infarct with pharyngeal dysmotility and paralyzed vocal cord: management with a multidisciplinary approach.

An unusual case of right posterior brainstem infarction with isolated deficits of severe dysphagia and ataxia is presented. Neurological examination revealed dysfunction of the pharyngeal and laryngeal branches of cranial nerves IX, X, and a paralyzed right vocal cord. The patient was unable to swallow 1/4 teaspoon of applesauce. Modified barium swallow revealed extremely sluggish pharyngeal peristalsis and absent swallowing reflex. Percutaneous esophageal gastrostomy tube was inserted and an intensive dysphagia rehabilitation program was initiated. Pharyngeal-phase-oriented protocol was used.

Reflex responses from the main pulmonary artery and bifurcation in anaesthetised dogs.

This study was undertaken to determine the reflex cardiovascular and respiratory responses to discrete stimulation of pulmonary arterial baroreceptors using a preparation in which secondary modulation of responses from other reflexes was prevented. Dogs were anaesthetised with -chloralose, artificially ventilated, the chests widely opened and a cardiopulmonary bypass established. The main pulmonary arterial trunk, bifurcation and extrapulmonary arteries as far as the first lobar arteries on each side were vascularly isolated and perfused through the left pulmonary artery and drained via the ri

Foot dystonia and lumbar canal stenosis.

We describe a patient who developed involuntary, painless, dystonic contraction of the toes of the right foot on standing or walking. The development of this abnormal movement had been preceded by sensory disturbance on the soles of both feet, triggered by dorsiflexion of the feet. Examination showed that weight bearing on the right foot and walking brought on clawing of the toes of the right foot, which was relieved within seconds of taking pressure off the right foot. There was sensory and reflex evidence of bilateral S1 root disturbance confirmed by electrophysiology. Magnetic resonance ima

[Unusual mechanism of reciprocating tachycardia in a case of ventricular preexcitation resistant to preventive anti-arrhythmia therapy].

A patient with a previously asymptomatic W.P.W. syndrome had, at the age of 60, frequent runs of paroxysmal tachycardia, which occurred particularly after meals. The paroxysms were easily controlled by pharmacological therapy in the acute stage, but could not be prevented by prophylactic therapy because of a large oesophageal diverticulum which, through right vagal reflexes, caused sinus arrests with junctional escapes that triggered the re-entry mechanism. Resection of the diverticulum abolished the tachycardia spells.

A Case of Carotid Sinus Reflex Caused by Manual Aspiration Thrombectomy Using a Balloon Guide Catheter.

When starting a mechanical thrombectomy, manual aspiration with balloon guide catheters inserted into the internal carotid artery (ICA) is an efficient method for thrombus aspiration. However, no complications associated with this procedure have been reported. This study describes the case of a 76-year-old man who presented to our hospital with total aphasia and complete right-sided paralysis due to chronic atrial fibrillation and left occlusion of the ICA. When the balloon guide catheter was inserted and inflated at the origin of the left ICA, the patient's systolic blood pressure suddenly de

Voluntary teeth clenching facilitates human motor system excitability.

OBJECTIVE: Voluntary contraction of the teeth is a common maneuver used to facilitate peripheral monosynaptic reflexes. It was the aim of this study to determine the site along the neuraxis where this effect occurs. METHODS: Focal transcranial magnetic stimulation (TMS) was used to measure recruitment curves, motor thresholds and intracortical inhibition and facilitation from the right first dorsal interosseus (FDI) and tibialis anterior (TA) muscles in seven normal volunteers. Changes in excitability in subcortical structures during teeth clenching were studied using F waves, H reflexes, and

The acute effects of dynamic stretching on the neuromuscular system are independent of the velocity.

This study examined the acute effects of dynamic stretching at different velocities on the neuromuscular system. Fourteen participants underwent four experimental sessions in random order: (1) control (lying at rest with the ankle in a neutral position); (2) slow velocity dynamic stretching (50 beats/min; SLOWDS); (3) moderate velocity dynamic stretching (70 beats/min; MODDS); and (4) fast velocity dynamic stretching (90 beats/min; FASTDS). The stretching protocols consisted of four sets of 10 repetitions and targeted the plantar flexor muscles of the right ankle. Assessments included corticos

Right-sided dominance of the bilateral vestibular system in the upper brainstem and thalamus.

MRI diffusion tensor imaging tractography was performed on the bilateral vestibular brainstem pathways, which run from the vestibular nuclei via the paramedian and posterolateral thalamic subnuclei to the parieto-insular vestibular cortex. Twenty-one right-handed healthy subjects participated. Quantitative analysis revealed a rope-ladder-like system of vestibular pathways in the brainstem with crossings at pontine and mesencephalic levels. Three structural types of right-left fiber distributions could be delineated: (1) evenly distributed pathways at the lower pontine level from the vestibular
