[Sleep-disordered breathing and cardiac resynchronization therapy].

Patients with progressive heart failure often suffer from sleep-disordered breathing (SDB). Upon receiving cardiac resynchronization therapy (CRT), there is an improvement of cardiac function and central sleep apnea syndrome (CSA) with Cheyne-Stokes respiration; however, effects of CRT on obstructive sleep apnea syndrome seemed to be without clinical relevance. Likewise, additional atrial overdrive pacing did not improve CRT effects relevantly in CSA patients. During CRT, there is an improvement in sleep parameters, sleep quality by reduction of depressive syndromes, and in long-term survival.

Oculocutaneous albinism accompanied by minor morphologic stigmata and reduced number and function of NK cells. A new variant of NK cell defect?

We describe the case of a 7-year-old girl with an apparently new genetic disorder characterized by oculocutaneous albinism, microcephaly, low-set helices, a prominent nose with a broad bridge, a long philtrum, a thin upper lip, a short neck, brachydactyly of the hands and syndactyly between the second and third toes of both feet, thrombocytopenia, and granulocytopenia. In addition, she had extremely low amounts of natural killer cells that were phenotypically normal but lacking cytotoxic activities. Clinically this defect was associated with recurrent and severe respirator-dependent pneumonia

320-detector CT imaging of the upper airway structure of patients with obstructive sleep apnea-hypopnea syndrome.

Obstructive sleep apnea-hypopnea syndrome (OSAHS) is one of the most common sleep disorders that have significant associated health costs. It is caused by recurrent occlusion of the upper airway during sleep. Recently, many researchers have provided evidence that patients with OSAHS tend to have a large tongue, a comparatively prolonged soft palate, and a narrow upper airway. There are many therapeutic methods for OSAHS, although determining the obstructive points is very important. Several methods to study the morphology of the upper airway in patients with OSAHS are available: acoustic reflec

Anomalies of the tracheobronchial tree in patients with esophageal atresia.

To examine associated anatomic anomalies of the tracheobronchial tree and structural abnormalities of the trachea, and their clinical consequences, bronchoscopic examination and follow-up studies were performed in 32 patients with esophageal atresia (EA) and tracheoesophageal fistula (TEF) over a 5-year period. To evaluate structural abnormalities of trachea, the ratio of circumferential length of cartilaginous trachea to circumferential length of membranous trachea (C:M ratio) was determined before surgical repair. In patients examined before and after surgical repair, the motion of the poste

The role of ultrasound lung artifacts in the diagnosis of respiratory diseases.

Thoracic ultrasound is employed for the diagnosis of many thoracic diseases and is an accepted detection tool of pleural effusions, atelectasis, pneumothorax, and pneumonia. However, the use of ultrasound for the evaluation of parenchymal lung disease, when the organ is still aerated, is a relatively new application. Areas covered: The diagnosis of a normal lung and the differentiation between a normally aerated lung and a lung with interstitial pathology is based on the interpretation of ultrasound artifacts universally known as A and B-Lines. Even though the practical role of lung ultrasound

Giant Left Atrium in a 14-Month-Old Child With Compression of Airways.

Giant left atrium (GLA) is a rare entity in the pediatric population. GLA carries a significant mortality risk; once its existence is established, it needs to be evaluated with intention to treat. We report a 14-month-old boy with GLA presenting with symptoms of cough and stridor because of compressed airways. The child underwent successful surgical resection for the same.

Oxygen therapy for management of periodic breathing : A theoretical approach.

A generalized framework for the generation of Periodic Breathing (PB), caused by delay variation, hypocapnia and sleep along with its management with oxygen therapy is presented. For this, a minimal model of respiratory regulation with cardiovascular component and two delays is proposed. This model is linearized and analyzed for stability by the proposed algorithms using Lyapunov-Krasovskii functional. Oscillation in this model is produced by the increase of delays, an increase of chemoreceptor gains (hypocapnia) and a decrease in minute ventilation (sleep induced PB). For delay variation, it

Acute Respiratory Events in Connective Tissue Disorders.

Subacute-acute, hyperacute, or even catastrophic and fulminant respiratory events occur in almost all classic connective tissue disorders (CTDs); they may share systemic life-threatening manifestations, may precipitously lead to respiratory failure requiring ventilatory support as well as a combination of specific therapeutic measures, and in most affected patients constitute the devastating end-of-life event. In CTDs, acute respiratory events may be related to any respiratory compartment including the airways, lung parenchyma, alveolar capillaries, lung vessels, pleura, and ventilatory muscle

Hypersensitivity to hypercapnia: definition/(s).

Empirical evidence indicates that panic disorder (PD) patients experience hypersensitivity to hypercapnia, a condition in which the blood level of carbon dioxide exceeds the normal value. The importance of this research line is substantial and indeed, hypercapnic hypersensitivity has been advanced as a possible endophenotype of panic. Definitions of "hypersensitivity," however, have varied. The purpose of this brief review is to delineate and critique different definitions of hypercapnic hypersensitivity. Several definitions - panic attack rate, panic symptoms including dyspnea, subjective anx

When wheezing may not mean asthma. Other common and uncommon causes to consider.

Establishing that wheezing is not due to asthma requires attention to the patient's history, physical examination results, and response to therapy. If the diagnosis of asthma is questionable, appropriate pulmonary function tests should be performed to firmly establish or refute that the wheezing is due to asthma. If asthma is not the cause, other entities need to be considered so that inappropriate therapy is not prescribed.

[The differential diagnosis of sarcoidosis of the respiratory organs using mathematical methods and computers].

A system for detecting sarcoidosis has been proposed on the basis of the results of a primary examination, which comprises a combination of programs for determining whether sarcoidosis may occur in various X-ray syndromes (enlarged intrathoracic lymph nodes, disseminated process, infiltrative or pneumonia-like shadow and cavity formation). The system has been developed with the help of a program-algorithm complex which enables one to make the dialogue system of the decision-make support on the basis of statistical methods of "image recognition" containing basic structural elements of the exper
