The frequency of non-motor symptoms in SCA3 and their association with disease severity and lifestyle factors.

BACKGROUND: Non-motor symptoms (NMS) are a substantial burden for patients with SCA3. There are limited data on their frequency, and their r … lifestyle factors alcohol consumption, smoking and physical activity. RESULTS: Sleep disturbance, restless legs syndrome, mild cognitive impairment, depression, bladder dysfunction and pallhypesthesia were frequent among SCA3 patients, while mainly absent in pre-ataxic mutation carriers. Except for restless legs syndrome, NMS correlated significantly with disease seve …

[Why pregnant women sleep poorly compared to non-pregnant women? Cross-sectional study].

AIMS: To evaluate causes/reasons that fragment sleep related to physiological changes in the different trimesters of pregnancy and compare t … groups we find: ME / MNE: Snoring: p = 0.001. Apnea: p = 0.89. In NPW, the following predominated: Restless legs syndrome: p = 0.01. Bruxism: p = 0.00. CONCLUSIONS: The PW reported more fragmented sleep due to almost all physiological causes, related to pregnancy, who were questioned, but the only sleep disorders found, by survey, were mainly respiratory disorders, ni …

[Sleep disorders and mental health].

Sleep disorders occur in almost one out of three people and are therefore among the most common disorders that physicians encounter in their practice. They often also present in the context of mental disorders. While it was previously assumed that they were merely a symptom of mental illness, current literature emphasizes the bidirectional connection between sleep and mental disorders. Even in the most common sleep disorders, namely sleep-related breathing disorders, insomnia, and restless legs syndrome / periodic limb movement disorder, reciprocal and sometimes complex interactions between sl…

Neurological Sleep Disorders and Blood Pressure: Current Evidence.

Hypertension is a major determinant of cardiovascular morbidity and mortality and is highly prevalent in the general population. While the relationship between sleep apnea and increased blood pressure has been well documented, less recognized is emerging evidence linking sleep-related movement disorders such as restless legs syndrome/periodic limb movements of sleep and sleep-related bruxism with blood pressure (BP) dysregulation and hypertension. There is also recent literature linking narcolepsy-cataplexy with elevated BP and altered pressor responses, and there are data suggesting abnormal …

Defining the boundaries of the response of sleep leg movements to a single dose of dopamine agonist.

STUDY OBJECTIVES: To define which leg movements (LM) associated with restless legs syndrome (RLS) respond to dopamine-agonist treatment and verify if they fall within current diagnostic criteria for periodic LM during sleep (PLMS). DESIGN: Single-blind placebo-controlled study. SETTINGS: Sleep laboratory. PATIENTS: 43 consecutive untreated patients with idiopathic restless legs syndrome. INTERVENTIONS: Patients underwent clinical and neurophysiological evaluation, hematological screening, and 2 consecutive full-night polysomnographic studies. Before the second polysomnographic study, all p…

[A case of narcolepsy in adolescent].

A description of the clinical observation of the teenager with narcolepsy is presented. This case demonstrates specific clinical and polysomnographic characteristics of narcolepsy with cataplexy as well as a unique combination of paroxysmal forms of diseases affecting the state of sleep and wakefulness (narcolepsy, epilepsy and other phenomena related to sleep medicine). These forms include sleep-associated eating disorder and restless legs syndrome. The current approaches to treatment of narcolepsy are presented.

Unilateral RLS with predominantly ipsilateral PLMS and variable response to dopaminergic drugs: a variant of idiopathic RLS?

BACKGROUND: Restless legs syndrome (RLS) is characterized by sensorimotor symptoms that usually are localized in both legs, but may present considerable asymmetry. Patients with strictly and persisting unilateral manifestations have not yet been reported. METHODS: We describe the clinical and electrophysiological characteristics of three RLS patients with unilateral symptoms. RESULTS: All essential RLS criteria were fulfilled in each patient. Neuroimaging and electrophysiological studies did not reveal structural lesions. All patients showed a predominance of periodic limb movements in sleep…

The cerebellum and sleep.

The importance of the cerebellum in sleep disorders, and vice versa, is only beginning to be understood. Advanced neuroimaging modalities have revealed cerebellar changes in both common and rare sleep disorders. Sleep disorders in those with genetic cerebellar disease, such as spinocerebellar ataxia, Friedreich ataxia, Joubert syndrome, and ataxia-telangiectasia, include excessive daytime sleepiness, restless legs syndrome, periodic limb movements of sleep, obstructive apnea, central apnea, and rapid eye movement behavior disorder. Sleep medicine is an important and under-recognized part of th…

Sleep disorders in the elderly.

Geriatric patients often complain about sleep disorders, but many of the typical sleep disturbances in the elderly are thought to be normal consequences of old age and go underdiagnosed and undertreated. Sleep disorders are estimated to affect nearly 50% of older persons. Most frequently the elderly suffer from Sleep Disordered Breathing (SDB), Periodic Limb Movements in Sleep (PLMS), Restless Legs Syndrome (RLS), morning headaches, circadian rhythm disorders, excessive daytime sleepiness, Obstructive Sleep Apnea Syndrome (OSAS), and insomnia. This review describes all these common sleep probl…

Pediatric sleep-wake disorders.

Sleep-wake problems are common during childhood and adolescence. They are of diverse cause, and can contribute significantly to alterations in behavior, cognition, and learning. Obstructive sleep apnea, central hypoventilation syndrome, narcolepsy, periodic hypersomnia, delayed sleep phase syndrome, restless legs syndrome, parasomnias, and sleep disruption consequent to psychiatric disorders are some of the commonly encountered conditions. Some aspects of sleep architecture and its organization change with age and maturation. Diagnostic criteria and sleep laboratory techniques and findings for…

The prevalence of restless sleep disorder among a clinical sample of children and adolescents referred to a sleep centre.

Restless sleep disorder (RSD) is a newly described sleep disorder in children characterized by large body movements and repositioning that lasts all night with at least five body movements per hour and a significant impact on daytime behaviours. The authors have previously identified and described the syndrome and compared the sleep parameters and sleep-related movements to those in children with restless legs syndrome, normal controls and snorers. The current study is a retrospective review of the sleep diagnosis in 300 consecutive children seen and evaluated in a single sleep disorders centr…

A Case of Restless Arm Syndrome and the Effective Use of Gabapentin.

Restless arm syndrome (RAS) is an uncommon neurological condition that is characterized by an irresistible urge to move the upper limbs, often worse at rest and transiently relieved by movement. RAS is an upper limb variant of restless legs syndrome (RLS), a more common neurological condition, also known as Willis-Ekbom disease. In view of its atypical presentation, RAS is often underdiagnosed. RAS is reported to share the same risk factors as RLS with its prevalence increasing with age, in the presence of certain conditions, and associated with the use of medications like antipsychotics and a…

Immediate resolution of acute, choreatic hyperkinesias following intravenous fentanyl.

Acute hyperkinesia after discontinuation of tramadol in a patient with chronic pain using citalopram and pramipexole for restless legs syndrome (RLS) has not been reported. An 81-year-old female was admitted for increasing hyperkinesias of the whole body after she had discontinued tramadol 300 mg (taken during 3 months) without tapering 4 days earlier. In addition, she was on treatment with pramipexole (0.18 mg) for RLS for years, citalopram 10 mg/day for ~4 years, and fentanyl 75 μg/day for 1 year. Hyperkinesias did not respond to benzodiazepines, quetiapine, biperiden, or valproic acid. Surp…

Sleep and Parkinson Disease.

Sleep disorders are prevalent in Parkinson disease (PD), a disease with well recognized motor dysfunction. Sleep related problems received little attention until the last three decades. Sleep disorders seen in PD patients include insomnia, excessive sleepiness, restless legs syndrome, REM sleep behavior disorder. Some of these can have significant impact and lower the quality of life in these patients. An understanding of sleep issues in PD can help identify them early and result in optimal management.
