We report the case of an 8-year-old girl who, having been reported missing, was found while concealed in a bed storage compartment in her neighbour's house. Despite medical intervention she died 13 hours later. Autopsy revealed sexual and physical assault with multiple incised and blunt force injuries, signs of possible restraint, torture and evidence of concealed trauma. Our report discusses the forensic interpretation of findings and the contributory roles of haemorrhagic shock and positional asphyxia. It demonstrates the pivotal function of forensic autopsy in unravelling what occurred in s

PURPOSE: To examine the use of the PARO robotic pet seal for hospitalized older adults with dementia and compare the effects of PARO use with visits by a human on various outcomes, including physical restraint use, psychotropic medications, bedside sitters, and delirium.
METHOD: Complete data for 38 participants were analyzed using Mann-Whitney U and chi-square tests.
RESULTS: Participants interacting with PARO spent less time in their interactions; however, they received fewer psychotropic medications and had fewer instances of delirium than those who interacted with human visitors only.

Previous research indicates that acute physical stress (restraint) raises core temperature in growing pigs via a prostaglandin-dependent mechanism. This study investigated whether transport stress affects body temperature in pigs and whether any such changes might involve endogenous prostaglandins. Pre-pubertal boars (n = 7) were implanted with venous catheters and biotelemetry devices for the measurement of core temperature. They were transported for two hours, with and without indomethacin (IND) pre-treatment, and blood samples were taken at 15 minute intervals for the determination of plasm

BACKGROUND: The patella is a sesamoid bone that comprises a vital component of the extensor mechanism of the knee. It has static and dynamic restraints that differ throughout range of motion of the knee. Tracking, and potentially maltracking, are impacted additionally by the bony anatomy of the pelvis, femur, and tibia.
INDICATIONS: Evaluation of patients with patellofemoral instability warrants consideration of each of these nuanced arenas of tracking. The diagnostic workup must be based on a thorough understanding of anatomy, which guides focused physical examination maneuvers that are not

Seclusion and restraint (S/R) is a controversial topic in the field of psychiatry, due in part to the high rates of childhood physical and sexual abuse found among psychiatric inpatients. The trauma-informed care perspective suggests that the use of S/R with previously abused inpatients may result in retraumatization due to mental associations between childhood trauma and the experience during S/R. Thus, though one would expect to see efforts on the part of inpatient psychiatric facilities to limit S/R of previously abused inpatients, research suggests that trauma victims may be more likely to

A 36-year-old pregnant woman with a prior history of depression and recent gunshot wounds presented with sudden deterioration in her mental status. Clinical examination revealed psychosis, hallucinations, and lack of orientation, with an otherwise normal neurological and cardiorespiratory examination. Computed tomographic scan of her head was normal, and she was diagnosed with acute psychosis and excited delirium. She did not respond to supraphysiologic dosages of antipsychotic therapy and needed physical restraints for combativeness and agitation. Her cerebrospinal fluid analysis was negative

In this introductory chapter we describe the ethological basis of defensive behavior, including tonic immobility (TI). The defensive repertoire activated in response to threatening stimuli, both in natural and experimental conditions, consists of a system of interrelated behaviors influenced by two main dimensions, as distance from the threat and escapable/inescapable context. When the active strategy of escape is not feasible, passive immobility forms are adopted, the latter representing substitutes of actual escape. In an inescapable context, and at very short distance or in contact with the

Lymphoedema is a long-term chronic condition that results from lymphatic insufficiency and may cause skin changes, which can cause discomfort and impact patient' quality of life. The four cornerstones of lymphoedema management are exercise, lymphatic massage, compression and skin care. This article critically examines self-management in lymphoedema focusing on skin care. Patients may initially demonstrate enthusiasm to undertake the required skin care independently. However, psychosocial factors, such as financial burden and physical restraints, can affect motivation and behaviours over time.

The use of seclusion and restraints (SR) in acute hospital settings remains a controversial practice. Despite the focus on SR in the psychiatric services literature, data on SR use in pediatric day hospital settings is lacking. A case-control retrospective analysis for children admitted into a pediatric psychiatry day hospital in a 2-year span examined predictors of SR use. Demographic and clinical descriptors were examined in relation to SR events using univariate and multivariate regression models. Significant univariate risk factors for SR use were psychiatric morbidity, history of physical

Delirium is characterized by disturbances of consciousness, attention, cognition, and perception and is the most common reason for acute cognitive dysfunction in hospitalized elderly patients. Causes of delirium can be multifactorial, and a careful medical and medication history can help determine the underlying cause of behavioral disturbances. A 65-year-old patient with a history of chronic pain, insomnia, and multiple medical problems, who presented with altered mental status and aggressive behavior, is described. The patient had taken an overdose of zolpidem prior to admission, and she req

This research study utilizes indicators from federal and state surveys to evaluate variation in outcomes in 438 Medicare certified skilled nursing care facilities in Pennsylvania. First, a standardization function adjusting for patient characteristics known to influence outcomes was developed and estimated. The relationships between organizational and environmental characteristics and the chosen outcome indicators (i.e., differences between the actual and expected rate of mortality, pressure ulcers, urethral catheterization and physical restraints) were then analyzed by weighted least squares

The behavioural symptoms in a 10-year-old boy with Lesch-Nyhan syndrome were effectively ameliorated by the behavior therapy techniques of systematic desensitization and extinction. Therapy was undertaken in a highly controlled environment. The hypothesis that the self-destructive behaviours in this syndrome were voluntary and maintained through continuous reinforcement was confirmed. Characteristic biting and other maladaptive behaviours were extinguished. Over a period of time it was possible to remove all the physical restraints previously used to prevent the boy injuring himself. During tr

Under stressful condition, reproductive function is impaired due to the activation of various components of the hypothalamic-pituitaryadrenal (HPA) axis, which can suppress the activity of the hypothalamic-pituitary-gonadal (HPG) axis at multiple levels. A hypothalamic neuropeptide, gonadotropin-inhibitory hormone (GnIH) is a key negative regulator of reproduction that governs the HPG axis. Converging lines of evidence have suggested that different stress types and their duration, such as physical or psychological, and acute or chronic, can modulate the GnIH system. To clarify the sensitivity

Management of the unstable shoulder after a failed stabilization procedure can be difficult and challenging. Detailed understanding of the native shoulder anatomy, including its static and dynamic restraints, is necessary for determining the patient's primary pathology. In addition, evaluation of the patient's history, physical exam, and imaging is important for identifying the cause for failure after the initial procedure. Common mistakes include under-appreciation of bony defects, failure to recognize capsular laxity, technical errors, and missed associated pathology. Many potential treatmen

The history of Icelandic psychiatry is in many ways comparable with neighboring countries. Mentally ill people were badly treated in the country and received no medical treatment until at the beginning of the 20th century. Kleppur Hospital for the mentally insane was built in 1907; hence the development was approximately 150 years later than in comparable countries. Only two doctors contributed significantly to policy making in Icelandic psychiatry until 1959 and they were very different personalities. Dr Thórdur Sveinsson was famous for his interest in hydrotherapy and Dr Helgi Tómasson for h

Several physical and psychological stresses frequently become triggers for gastrointestinal disorders such as ulcer. In this study, we tried to identify serum proteins as potential biomarkers for the evaluation of stress-induced gastric ulcer. By proteomic analysis using rats with gastric ulcer induced by water immersion and restraint (WIR) stress as an animal model, we found quantitative changes in several serum proteins, including creatine kinase muscle M chain (CK-M) and apolipoprotein A-IV (ApoA4) in the stressed rats. On western blotting and enzyme-linked immunosorbent assay (ELISA), we c

Pitchers are prone to elbow injuries because of high and repetitive valgus stresses on the elbow. The anterior bundle of the medial ulnar collateral ligament (MCL) of the elbow is the primary restraint and is often attenuated with time, leading to functional incompetence and ultimate failure. Pitchers with a history of medial elbow pain, reduced velocity, and loss of command may have an MCL injury in evolution. Physical examination and imaging can confirm the diagnosis. Treatment begins with rest and activity modification. All medial elbow pain is not MCL injury. Surgery is considered only for
