Atypical Migrating Nummular Retinal Lesions in an Immunosuppressed Patient With Whipple Disease.

Whipple disease is a rare systemic infection of Tropheryma whipplei with nonspecific clinical features. Ocular involvement is uncommon but has been reported in both anterior and posterior segments of the eye. We present the case of a 70-year-old man with a history of cutaneous T-cell lymphoma, who developed unilateral ocular inflammation with hypopyon, vitritis, and migrating nummular retinal lesions. Despite intravitreal antimicrobial therapy, inflammation persisted. Four months later, he developed systemic symptoms and findings on duodenal biopsy consistent with Whipple disease. This case hi

Detailed Outer Retinal Assessment in Parkinson's Disease Using Directional OCT.

PURPOSE: This study aimed to compare outer retinal thickness and reflectivity using directional OCT between people living with Parkinson's disease (PD) and age-matched controls. METHODS: Fourteen neurologist-confirmed people living with PD and 18 age-matched healthy controls participated. To measure the true outer nuclear layer (ONL) (composed of photoreceptor cell bodies) rather than the conventional presumed or standard ONL (ONL plus Henle fiber layer [HFL], which includes photoreceptor processes), off-axis (directional) OCT scans were acquired by altering the incident beam angle. Combined

Levodopa in retinal disease: Dopamine pathways, neuroprotective mechanisms, and clinical evidence.

We review the role of levodopa (L-DOPA), a dopamine precursor used to treat Parkinson disease, as a candidate for retinal neuroprotection, focusing on retinal dopamine biology, mechanisms of action, and evidence from preclinical and clinical studies. Selected experimental and clinical studies indicate that L-DOPA and related dopaminergic interventions influence multiple retinal pathways relevant to degeneration. L-DOPA, synthesized in the pigmented retinal pigment epithelium, activates GPR143 signaling and promotes the release of neuroprotective factors. In animal studies, L-DOPA supplementati

Hyperspectral retinal imaging to detect Alzheimer's disease in a memory clinic setting.

BACKGROUND: Previous literature indicate retinal hyperspectral imaging as a non-invasive method with the potential for identifying amyloid-beta (Aβ) protein deposits. Current diagnostic methods, such as cerebrospinal fluid analysis or positron emission tomography, are costly, invasive, and non-scalable. Hyperspectral imaging offers a potentially accessible alternative for early detection of Alzheimer's disease. The aim of this study is to investigate the potential of retinal hyperspectral imaging in identifying Aβ-positive patients within a clinical cohort from a memory clinic. METHODS: A pro

OCT5k: A dataset of multi-disease and multi-graded annotations for retinal layers.

Publicly available open-access OCT datasets for retinal layer segmentation have been limited in scope, often being small in size, specific to a single disease, or containing only one grading. This dataset improves upon this with multi-grader and multi-disease labels for training machine learning-based algorithms. The proposed dataset covers three subsets of scans (Age-related Macular Degeneration, Diabetic Macular Edema, and healthy) and annotations for two types of tasks (semantic segmentation and object detection). This dataset compiled 5016 pixel-wise manual labels for 1672 OCT scans featur

Surfactant proteins A and D nucleotide variants: association with retinal vascular disease.

BACKGROUND: Retinopathy of prematurity (ROP) is associated with systemic inflammation. Surfactant proteins A and D (SP-A and SP-D) play an immunomodulatory role. We previously reported the impact of SP-A on retinal angiogenesis. This study investigates SP-A and SP-D single-nucleotide polymorphisms (SNPs) with risk of ROP. METHODS: Subjects were infants with gestational age (GA) of <32 weeks and/or birth weight <1500 grams. DNA from blood was used to genotype the SNPs. Statistical analysis used logistic regression for the association of ROP with genetic and clinical factors, including bronchop

An explainable transformer model for Alzheimer's disease detection using retinal imaging.

Alzheimer's disease (AD) is a neurodegenerative disorder that affects millions worldwide. In the absence of effective treatment options, early diagnosis is crucial for initiating management strategies to delay disease onset and slow down its progression. In this study, we propose Retformer, a novel transformer-based architecture for detecting AD using retinal imaging modalities, leveraging the power of transformers and explainable artificial intelligence. The Retformer model is trained on datasets of different modalities of retinal images from patients with AD and age-matched healthy controls,
