Patients with wrist pain commonly present with an acute injury or spontaneous onset of pain without a definite traumatic event. A fall onto an outstretched hand can lead to a scaphoid fracture, which is the most commonly fractured carpal bone. Conventional radiography alone can miss up to 30 percent of scaphoid fractures. Specialized views (e.g., posteroanterior in ulnar deviation, pronated oblique) and repeat radiography in 10 to 14 days can improve sensitivity for scaphoid fractures. If a suspected scaphoid fracture cannot be confirmed with plain radiography, a bone scan or magnetic resonanc

Clinical outcomes of the dorsal-retrograde headless screw-fixation technique in 15 patients with proximal scaphoid nonunion are presented. In this technique, screws are inserted from the dorsal rough surface of the scaphoid, located between the dorsal ridge and scaphoid-trapezium-trapezoid joint. Fifteen patients underwent osteosynthesis with this technique with iliac bone graft. Seven patients required primary surgery, and eight patients with a history of failed operation required revision surgery. Among 15 patients, 13 achieved union and two with persistent nonunion were asymptomatic with av

Background This article reviews the results of a surgical technique using three iterations of drilling , autologous cancellous bone grafting ( filling ), and use of an intraosseous compression screw for the treatment of nondisplaced or minimally displaced scaphoid delayed unions or nonunions. Methods Part 1-Cadaveric study: Three cadaveric scaphoids underwent stained cancellous bone graft packing and headless cannulated compression screw placement using a single iteration of drilling and graft packing. Three additional scaphoids were allocated to the triple "drill and fill" group, and underw

Herbert's classification of scaphoid fractures provides the underlying rationale for treatment according to the fracture type. A CT bone scan in the long axis of the scaphoid is the best means of differentiating between stable and unstable fractures. This is difficult from conventional X-rays due to the particular three-dimensional anatomy of the scaphoid. To avoid long-term plaster immobilization and to diminish the risk of a nonunion, unstable fractures of type B should be fixed operatively. With headless screws such as the Herbert screw, which are now available in a cannulated shape, the ma

BACKGROUND: Scapholunate interosseous ligament (SLIL) injuries alter carpal stability and cause abnormal load transmission due to changes in carpal bone position. Carpal stability under load is dependent on healthy carpal bone alignment, yet current imaging methods cannot evaluate three-dimensional carpal bone position during functional loading. Therefore, the purpose of this study was to characterize carpal bone position changes under load and to develop semi-automated three-dimensional carpal alignment measurement techniques, including two approaches to measure dorsal scaphoid translation (d

BACKGROUND: Surgically, it is difficult to achieve union of a scaphoid nonunion that is associated with osteonecrosis of the proximal pole, and those with carpal collapse are especially difficult to treat. A variety of vascularized bone grafts can be used. The purpose of this study was to compare the effectiveness of two types of vascularized bone graft -- a distal radial pedicle graft and a free vascularized medial femoral condyle graft -- in the treatment of scaphoid waist nonunions associated with proximal pole osteonecrosis and carpal collapse.
METHODS: A retrospective review was conducte

Preiser disease is a rare condition of avascular necrosis of the scaphoid occurring in the absence scaphoid fracture or trauma. While the etiology of Preiser disease remains unknown, it has been associated with steroid use, chemotherapy, and infrequently with systemic diseases. No reports have associated Preiser disease with hemoglobinopathy. Due to the rarity of Preiser disease, management remains controversial and evidence is limited. Here, we describe the case of a 32-year-old right-hand dominant male with sickle cell anemia and a 4-year history of bilateral wrist pain. Radiographs and gado

Scaphoid fractures account for 50 to 70% of all carpal bone fractures. Percutaneous scaphoid osteosynthesis can use the dorsal or volar approach, both with good results, and is most commonly performed under general anesthesia or regional nerve block. The wide-awake local anesthesia no tourniquet (WALANT) technique is already considered a safe and cost-effective technique in hand surgery around the world. Local anesthesia with epinephrine causes vasoconstriction, which obviates the need for tourniquet and, consequently, the need to use patient sedation. Thus, the possibility of testing fixation

This retrospective study evaluates the functional and radiological results of a series of acute trans-scaphoid dorsal perilunate fracture-dislocations treated operatively by minimally invasive technique. Twenty-one patients had surgery performed by one of three surgeons between 1994 and 2006, and all cases involved: (1) closed reduction of the carpus; (2) closed reduction and percutaneous screw fixation of the scaphoid fracture; and (3) closed reduction and multiple Kirschner wire stabilisation of the carpal dissociation. Seventeen males and four females with an average age of 29 years were ev

Whether operative treatment is a better option than nonoperative treatment for acute nondisplaced or minimally displaced fractures of the sc … er operative treatment is superior to nonoperative treatment for an acute proximal pole fracture of scaphoid bones. There is insufficient evidence to determine which type of cast should be used in nonoperative treatment of nondisplaced scaphoid fractures.

Scapholunate dissociation or scaphoid pseudarthrosis may lead to wrist osteoarthritis. When osteoarthritis concerns the midcarpal joint, proximal row carpectomy is not possible. Only 4-corner or capitolunate arthrodesis may be indicated. In this procedure, pseudarthrosis was frequently described in literature. However, in these series, fixation was performed with pins or staples. Type and position of the device are important to obtain carpal bones fusion. The efficiency of compression screws has been validated in scaphoid fracture or pseudarthrosis. Moreover, the axial position of the screws,

The difficulty in healing scaphoid nonunions is challenged further by the dynamic, unstable nature of the fracture-fragment interface. Recently, several investigators have introduced a minimally invasive technique for scaphoid nonunion repair, which has the advantages of minimal morbidity and accurate articular reduction, resulting in less postoperative stiffness and increased functional outcomes. However, failure to recognize the critical steps during minimally invasive surgery can result in incorrect treatment or limit any chances for successful bone repair. We reviewed the selected literatu

This is a preliminary report of eight cases in which an allograft was used to replace half of the scaphoid. The indications for the procedure include the following: (1) Severe necrosis with fragmentation of the proximal pole, (2) Very proximal pole nonunion with small (less than 20% of the bone), unreconstructable proximal fragments, and (3) One case of severely comminuted intra-articular fracture of the scaphotrapezial joint and basal joint of the thumb caused by a gunshot wound. The Herbert scaphoid screw was used to provide rigid fixation. Follow-up ranged from 8 to 30 months. The result wa

Background Scaphoid fractures are relatively uncommon in children, especially below the age of 10 due to the ossification of the scaphoid bone, which starts around 4 to 6 years of age and continues until 13 to 15 years of age, where pediatric scaphoid fractures peak. This makes the diagnoses challenging in this age group. Methods The primary aim of this study was to analyze prospectively collected data in managing scaphoid fractures. All cases in children up to the age of 10 years, treated in a tertiary pediatric hand and upper limb from January 2014 to June 2018 were included. Parameters st…

PURPOSE OF THE STUDY: Fractures of the scaphoid must be diagnosed quickly to avoid persistent nonunion and the risk of osteoarthritis. Despite meticulous physical examination and adequate x-ray detection, numerous occult fractures still go unrecognized. The aim of this prospective study was to analyze the pertinence of quantitative radio-scintigraphy (QRS) presently used for the diagnosis of occult wrist fractures.
MATERIAL AND METHODS: Quantitative radio-scintigraphy (QRS) is a new imaging technique associating quantitative bone scan and numerical fusion between bone scan images and x-ray im

INTRODUCTION: Volar locking plate fixation with cancellous bone grafting is an effective treatment for scaphoid non-union. However, the plate can impinge on the volar rim of the radius, causing pain or restricted movement that often necessitates plate removal. The primary aim of this study was to evaluate the effect of hardware removal on wrist function and patient-reported outcomes after plate fixation for scaphoid non-union.
METHODS: This prospective study assessed 49 of 113 patients who underwent plate fixation for scaphoid non-union and later required plate removal due to functional impai

PURPOSE: There are many controversies in the literature regarding the morphology of the scaphoid ligaments. The aim of this study was to provide a more accurate description by quantitatively describing the 3-dimensional, geometrical aspects of the scaphoid ligaments and their attachments, using cryomicrotome images of cadaveric wrists.
METHODS: Eight fresh-frozen human cadaver wrists were examined with computed tomography (CT) and an imaging cryomicrotome. A series of 2-dimensional cryoimages created a 3-dimensional anatomical data set of each test specimen. Detection of ligaments and their s

Presented is a case report of a 19-year-old man who developed a giant cell tumor in the distal ulna following trauma to the carpus. Roentgenograms and bone scan revealed a fracture of the scaphoid and no other bony abnormalities. On year later, after the scaphoid fracture had healed, the same investigations demonstrated a giant cell tumor of the distal ulna. Circumstantial evidence suggests a possible role for trauma in the pathogenesis of giant cell tumor.
