[Role of arthroscopy in complex sports injuries].

Shoulder lesions caused by accident are often of complex nature. In most cases, different structures are involved, which cannot be distinguished and individualized clinically with certainty. Arthroscopy permits far more differentiated diagnosis with systematic revision of traumatized structures. Besides the possibility of comprehensive repair (e.g. in part arthroscopic, in part open), the faster rehabilitation attained by minimal exposures is also of importance.

Pathophysiologic Origins of Brachial Plexus Injury.

OBJECTIVE: To analyze the origins and consequences of cases of brachial plexus injuries and their relationship to shoulder dystocia. METHODS: We conducted a retrospective cohort study and identified all neonates with brachial plexus injury delivered at our institution between March 2012 and July 2019. A review was performed of the maternal and neonatal records of each neonate to identify obstetric antecedents, including the occurrence of shoulder dystocia and persistence of the injury. Experience of the delivering clinician was also examined. Statistical analysis was performed with the Fisher

Risk factors and interventions for injury in competitive swimmers: a scoping review.

Competitive swimming involves repetitive, high-load movements that predispose athletes to overuse injuries, particularly affecting the shoul … sion criteria and included only competitive swimmers. Among 46 risk-factor studies, most focused on shoulder injuries, with fewer examining the lumbar spine or knee. The most frequently reported risk factors were training volume and intensity, injury history and muscle imbalance. All 12 intervention studies were exercise-based and targeted the shoulder or back. The …

Results of Acute Treatment of Reverse Hill-Sachs Lesion Associated With Locked Posterior Shoulder Dislocation With Modified McLaughlin Procedure: A Case Report and Literature Review.

This case report evaluates the clinical outcomes of two patients with locked posterior shoulder dislocation associated with a reverse Hill-Sachs lesion who were operated on in the acute phase with a modified McLaughlin procedure. Both patients had dislocated posterior shoulder fractures affecting approximately 40% of the articular surface of the humeral head and could not be reduced in the emergency department. Both patients were operated on the same day of hospitalization. We analyzed the clinical and radiological results after at least three years of follow-up. Care should be taken not to ov

Luxatio erecta humeri: Report of a swimming injury with analysis of the mechanism of the injury and associated injuries in literature.

Inferior shoulder dislocation also referred to as luxatio erecta is an unusual and rare type of shoulder dislocation. Its incidence is about 0.5% among all shoulder dislocations. After an exhaustive search of all the available literature we were unable to find a swimming accident case that did not have other associated injuries and an uneventful reduction. The mechanism of the injury was mostly related to direct axial loading and indirect hyperabduction lever arm. We would like to emphasize the importance of this being a swimming accident, a type of accident that requires awareness of the poss

The double fascicular nerve transfer for restoration of elbow flexion.

BACKGROUND: Injuries to the upper trunk of the brachial plexus are debilitating, affecting primarily shoulder abduction and elbow flexion. Treatment is aimed at restoring shoulder stabilization, shoulder abduction, and elbow flexion and may be accomplished by nerve grafting, nerve transfer, or functional muscular transfer. OBJECTIVE: To describe the double fascicular nerve transfer with the goal of restoring elbow flexion. METHODS: The double fascicular nerve transfer involves transferring an ulnar nerve fascicle to the musculocutaneous nerve innervating the biceps muscle and a median nerve

Acute massive rotator cuff tear and biceps tendon dislocation following posterior shoulder dislocation: A case report.

BACKGROUND: Posterior shoulder dislocation is a rare injury. It accounts for only 1%-4% of all shoulder dislocation cases. However, this injury is often underdiagnosed. Massive rotator cuff tears associated with posterior shoulder dislocation are exceptionally rare. Early diagnosis and surgical management are crucial for restoring shoulder function and preventing long-term disability. CASE SUMMARY: A 60-year-old male with no previous shoulder injuries presented to our hospital with severe right shoulder pain and immobility after a motorcycle accident. He reported that he braced his fall with …

Associated Pathologies following Luxatio Erecta Humeri: A Retrospective Analysis of 38 Cases.

Inferior shoulder dislocation in fixed abduction, also known as luxatio erecta humeri (LEH), is a rare injury with little data available. Therefore, the primary aim of this study was to evaluate and present our case series of this type of injury with special emphasis on associated pathologies; the secondary aim was to present diagnostic recommendations to detect for potential associated pathologies typically seen with this injury. A total of 38 patients (13 females, average age 72.8 years and 25 males, average age 41.4 years), who have been treated for inferior shoulder dislocation between 199

Advances in the management of traumatic anterior and atraumatic multidirectional shoulder instability.

Dislocation of the shoulder is a common and often disabling injury to an athlete. Most shoulder dislocations are traumatic in origin, occur in the anterior direction and result in stretching and detachment of the anterior capsule and labrum. The most frequent adverse sequel of shoulder dislocation is recurrence--an event that occurs most commonly in active individuals and less frequently with age. In the past, many operative procedures failed to address the anatomical disruptions of shoulder instability. Recently, an enhanced understanding of shoulder instability pathoanatomy and significant t

Neurovascular trauma in athletes.

Injuries to neurovascular structures are not the most common injuries seen in athletes and for this reason may often be overlooked. Additionally, diagnosis and management may be more difficult because of inexperience with these injuries. The majority of acute sports-related neurovascular injuries are associated with contact sports such as rugby, wrestling, ice hockey, and especially football. These injuries most commonly occur about the shoulder girdle and brachial plexus, with "burners" syndrome being the most common. Less common injuries include thoracic outlet syndrome, effort-induced throm

Injury differences between youth male and female American football players.

OBJECTIVES: American football continues to be a popular and rapidly growing sport among both males and females, with relatively little resea … le American football players had a greater likelihood of sustaining concussions and head, knee, and shoulder injuries, whereas females were more likely to have sprains/strains, finger, and ankle injuries. Future studies are warranted to further explore how these differences may be attributed to play style and sex-based physiologic and development differences.

Axillary nerve injuries in contact sports: recommendations for treatment and rehabilitation.

Axillary nerve injuries are some of the most common peripheral nerve injuries in athletes who participate in contact sports. Resulting deltoid muscle paralysis is secondary to nerve trauma which occurs following shoulder dislocation or a direct blow to the deltoid muscle. Compression neuropathy has been reported to occur in quadrilateral space syndrome as the axillary nerve exits this anatomic compartment. The axillary nerve is also extremely vulnerable during any operative procedure involving the inferior aspect of the shoulder, and iatrogenic injury to the axillary nerve remains a serious co

Single-Portal Arthroscopic Treatment of Posterior Shoulder Instability With Labral Tear With Reverse Hill-Sachs Remplissage Using MGHL.

Posterior glenohumeral dislocations are uncommon shoulder injuries accounting for up to 2% to 5% of all shoulder dislocations. They are caused by high-energy traumas, seizures, or electrocution as well as repetitive microtraumas related to sports or other activities. They can cause reverse Hill-Sachs lesions and in this Technical Note we describe a technique to treat this pathology.

Overuse syndromes in adult athletes.

Approximately 50% of all sports participants will be injured at some point, and at least half of these injuries will be attributed to overuse. This article provides an in-depth review of the presentation, diagnosis, and treatment of the most common overuse injuries encountered in the adult population: epicondylitis, shoulder impingement/rotator cuff tears, patello-femoral dysfunction, and Achilles tendinitis. Stress fractures, the "ultimate" overuse injuries, are also discussed.
