Immune-Mediated Hemolytic Anemia and Clinically Suspected Acute Pancreatitis in Dogs, a Pilot Study.

Acute pancreatitis can be a complication of massive hemolysis, above all when intravascular in nature. Therefore, the aim of this study was to investigate the association between canine immune mediated hemolytic anemia (IMHA) and clinically suspected acute pancreatitis (CSAP) and the role of calculated free plasma hemoglobin (Hbfp) in CSAP occurrence/development. In this cohort study the records of 95 dogs with IMHA and 95 sick dogs with pathologies other than IMHA were compared for CSAP occurrence/development. At presentation, 12/95 dogs with IMHA met criteria for CSAP, while only 3/95 sick c

[Electrophysiological study].

The purpose of an electrophysiological study (EPS) is to determine the mechanism of arrhythmias and to decide the means of therapy for that arrhythmia. The electrode catheters are introduced percutaneously into the atria or ventricle to record the intracardiac electrogram or to stimulate those areas by electrical current. In bradycardia, atrioventricular block and sick sinus syndrome are the good indications of EPS. To determine the indication of pacemaker implantation, recording the block site of atrioventricular block is useful, and sinus node recovery time and sinoatrial conduction time in

[Stroke in pacemaker users for sinus node disease. Relevance of atrial function and clinical characteristics].

Several studies reported that the annual incidence of stroke in patients with sick sinus syndrome ranges from 6 to 10% while the incidence of stroke in patients with atrial fibrillation is about 2-4% and about 0.1% in the normal population. We evaluated the prevalence of cerebral ischemia and peripheral embolism and investigated the predictor factors in a population of 80 patients paced for sick sinus syndrome. The implanted pacemakers were 40 ventricular and 40 physiological stimulation mode was based on the physicians judgement. All patients had cerebral computed tomography scan at the time

Controlled drug release in periodontics: a review of new therapies.

Controlled drug release formulations are rapidly gaining importance in drug therapy as replacements for conventional systemic dosages. Examples include skin patches for treatment of angina pectoris and motion sickness, eye inserts for glaucoma and implants for prolonged release of contraceptives. This paper will review the current status of periodontal controlled release devices and look at their potential role in periodontal treatment.

Occupational and physical therapy for work-related upper extremity disorders: how we can influence outcomes.

Upper extremity disorders in the workplace continue to be an economic strain on society. Physical and occupational therapy plays a crucial role in the management of the disorders. Medical providers should use therapists whose treatments demonstrate improvement in function and provide superior outcomes. Skilled therapy intervention requires that a therapist be able to identify and treat an injured worker in a holistic manner by looking at the whole individual, including issues that involve mechanical dysfunctions, psychosocial issues that include job satisfaction, and other age-related organic

Explaining heterogeneity in disability associated with current major depressive disorder: effects of illness characteristics and comorbid mental disorders.

BACKGROUND: Although major depressive disorder (MDD) is associated with disability, some persons do function well despite their illness. Aim of the present study was to examine the effect of illness characteristics and comorbid mental disorders on various aspects of disability among persons with a current MDD episode. METHODS: Data were derived from 607 participants with a current MDD based on the Composite International Diagnostic Interview (CIDI). Severity was assessed via the Inventory of Depressive Symptoms self-report (IDS-SR). For disability three outcome measures were used: World Healt

Indicators of need for occupational therapy in patients with chronic pain: occupational therapists' focus groups.

This study explores occupational therapists' (OT) perceptions of indicators for occupational therapy interventions among patients with chronic pain. An exploratory design was applied to six focus groups of OTs (n = 25) for data collection. Analysis was performed using content analysis through identification of meaning units, codes, categories and themes. Limitations of occupational performance was a major theme that included participant restrictions caused by physical, emotional and environmental barriers. Five subthemes of need were identified: 1) pain behaviour that prevented engagement in a

The impact of family, peer, and school contexts on depressive symptoms in adolescents with spina bifida.

OBJECTIVE: Based on social ecological theory, this study examined the joint relations among adolescents' family, peer, and school contexts and depressive symptoms in youth with spina bifida using cumulative, protective, and specific effects models. METHOD: Sixty families of adolescents with spina bifida and 65 comparison families reported on adolescents' positive experiences within these contexts and on depressive symptoms when youth were 14-15 and 16-17 years old. RESULTS: Adolescents with spina bifida had fewer total positive contexts and less positive experience within peer and school con

Patients with worse mental health report more physical limitations after adjustment for physical performance.

OBJECTIVE: To determine whether mental health scores are associated with self-reported physical limitations after adjustment for physical performance. Patient-reported physical limitations are widely used to assess health status or the impact of disease. However, patients' mental health may influence their reports of their physical limitations. METHODS: Mental health and physical limitations were measured using the SF-36v2 mental health and physical functioning subscales in a cross-sectional study of 1024 participants. Physical performance was measured using tests of strength, endurance, dext

Do positive psychosocial factors predict disease progression in HIV-1? A review of the evidence.

Adding to a traditional stress perspective, behavioral medicine has been focusing increasingly on investigating the potential impact of positive psychosocial factors on disease course in HIV. Dispositional optimism, active coping, and spirituality show the most evidence for predicting slower disease progression, although the data are not entirely consistent. Findings for the role of social support are mixed, although indications are that it may be particularly helpful at later stages of illness. Many of the other constructs (positive affect, finding meaning, emotional expression/processing, op

Occult PTSD with panic attacks in a patient post-TIA: case report.

Affective disorders are common sequelae of cerebrovascular events. A myriad of evidence demonstrates that clinically significant depression can often follow a stroke. However, less is known about the extent to which anxiety disorders present after these experiences, and in particular, post-traumatic stress disorder (PTSD) with panic attacks. To our knowledge, the association between transient ischemic attacks (TIAs) and PTSD with panic attacks has not been described in the literature. Here we describe a patient with depression and PTSD with panic attacks following a transient ischemic attack.

Teachers' perceptions of and responses to adolescents with chronic pain syndromes.

OBJECTIVE: To examine factors that influence teachers' perceptions of and responses to chronic pain in students. METHODS: Two-hundred and sixty classroom teachers responded to a vignette describing a student with limb pain. The 2 x 2 x 2 factorial design included conditions that varied by (a) the presence or absence of documented organic evidence for the pain, (b) cooperative vs. confrontational parent-teacher interactions, and (c) the presence or absence of communication from the medical team. Teachers rated pain severity and impairment, relief from classroom responsibilities, extent of acco

Idiopathic physical symptoms: a common manifestation of psychiatric disorders in primary care.

Worldwide, patients with common mental disorders, such as depression and anxiety, have a tendency to present first to primary care exhibiting idiopathic physical symptoms. Typically, these symptoms consist of pain and other physical complaints that remain medically unexplained. While in the past, traditional psychopathology emphasized the relevance of somatic presentations for disorders, such as depression, in the last few decades, the "somatic component" has been neglected in the assessment and treatment of psychiatric patients. Medical specialties have come up with a variety of "fashionable"

[Employment after the first myocardial infarct and rehabilitation].

Which are the determinants of return to work in middle aged myocardial infarction patients? This question was analysed by means of a follow-up study on men between 1986 and 1992 (group 1: n = 64, age = 46.6 (+/- 5.4), 5-6 years follow-up; group 2: n = 36, age = 51.2 (+/- 7.5), 2 years follow up), all of whom underwent a cardiac rehabilitation program in our hospital. Our aim was to determine the predictive power of social and psychological as compared to cardiologic factors. These factors were operating under two relevant conditions: 1. relative social homogeneity of the samples; 2. rehabilita

Medication, chronic illness and identity: the perspective of people with asthma.

The issue of compliance with prescribed medication has traditionally been dominated by the perspective of the health professional although increasingly sociologists, using qualitative methods, have begun to present the patients' point of view. However, little has been published on asthma, despite the numbers of people suffering from this chronic condition and the amount of medication regularly prescribed. This paper focuses on the perspective of a sample of S. Wales (U.K.) asthma patients who have all been prescribed prophylactic medication in the last 12 months and explores their attitudes to

[Personality structure and subjective illness concepts of phobic anxiety neurotic patients. Qualitative comparison of 12 individual analyses of initial psychotherapy interviews].

Verbatim transcripts of 12 psychotherapeutic interviews with patients suffering from phobia and panic disorders were examined, focussing on subjective theories of illness, biography, and descriptions of the own personality. The results of our qualitative content analysis allow the reconstruction of some central features of these patients, like characterizing the own personality as normal and without any problems in a stereotype way (1), at the same time feeling wrong understood and exploited from other persons (2), feeling as an outsider and left alone (3), and being no more able to compete wi

'Compensation neurosis' revisited: outcome studies of the effects of litigation.

Although the term 'compensation neurosis' has been frequently used to describe personal injury litigants, its clinical validity--and the assumptions about individuals described by this label--have not been critically examined. This article reviews the concept of 'compensation neurosis' and follow-up studies of claimants for compensation following the finalisation of their legal proceedings. A follow-up study of 760 litigants found that, of the 264 subjects who were not working at the time of conclusion of litigation and who could be traced, 198 (75%) were not working after an average of 23 mon

Age and health bias in practicing clinical psychologists.

Although ageism is widely cited as a problem in mental health delivery, it is unknown whether practitioner biases are related to factors such as physical health. A randomly drawn national sample of experienced practicing doctoral-level psychologists (N = 371) responded to detailed vignettes of a client presenting with symptoms of depression, in which age (35 years or 70 years) and health (unremarkable or poor) were manipulated. Respondents completed ratings involving professional and interpersonal judgments about the hypothetical client. Results revealed some evidence for age bias, but much st
