When the inheritance of two heterozygote states become a diagnostic problem: misdiagnosis of the sickle cell trait.

BACKGROUND: The sickle cell trait is a benign asymptomatic condition that should not ordinarily be associated with clinical manifestations of a haemoglobinopathy. METHOD: This is a case control study of sickle cell trait patients who presented with symptomatology of a haemoglobinopathy. HbA2, HbF and HbS levels as well as the haematocrit and the peripheral film pictures of 10 symptomatic individuals (patients) with the sickle cell trait were compared with those of 20 asymptomatic individuals (controls) with the sickle cell trait. RESULTS: The mean HbA, of the cases was 4.9% compared to the m…

Albuminuria in Afro-Caribbeans with Type 2 diabetes mellitus: is the sickle cell trait a risk factor?

AIMS: To determine the prevalence of albuminuria [raised albumin-creatinine ratio (ACR)] in an out-patient population of Afro-Caribbeans with Type 2 diabetes mellitus (DM) and to determine if the possession of the sickle cell trait (SCT) is a risk factor. PATIENTS AND METHODS: The ACR in a morning urine sample was determined in each of 181 Afro-Caribbeans with Type 2 DM attending the out-patient clinic at King's Diabetes Centre of King's College Hospital, London. The subjects were genotyped for the SCT, Haemoglobin AS (HbAS). RESULTS: Although a raised ACR was demonstrated more frequently in…

Alterations in sensitivity to calcium and enzymatic hydrolysis of membranes from sickle cell disease and trait erythrocytes.

Normally, human erythrocytes display several responses to elevated intracellular calcium levels. These include a shape transition from discocyte to spherocyte, shedding of microvesicles into the extracellular fluid, and enhanced susceptibility to the hydrolytic action of secretory phospholipase A(2). These responses to elevated intracellular calcium were all blunted in erythrocytes containing hemoglobin S. The reduction of both the shape transition and the shedding of microvesicles were greater than the impairment of phospholipase susceptibility, and both correlated strongly with the intracell

Effect of sickle cell trait and B-Thalassemia minor on determinations of HbA1c by an immunoassay method.

OBJECTIVE: Glycated hemoglobin determination is being used worldwide to monitor the efficiency of blood glucose control and to plan treatment in diabetes mellitus patients. Several methods are used for measuring glycated hemoglobin, but a possible interference by hemoglobin variants is a major concern. The use of immunoassay methods with glycated hemoglobin-specific antibodies is supposed to overcome this problem. We are evaluating the effect of the most prevalent hemoglobinopathies in the region (sickle trait hemoglobin and B-Thalassemia) on the immunoassay method used in determining glycated

Beta-thalassaemia unlinked to the beta-globin gene interacts with sickle-cell trait in a Portuguese family.

An autosomally transmitted hypochromic microcytic mild anaemia with elevated haemoglobin (Hb) A2 and globin chain imbalance has been observed in a three-generation family of Portuguese origin. Extensive DNA analysis of the beta-globin gene cluster, including the complete sequencing of the beta-globin gene and flanking regions, failed to reveal any genetic alteration. The co-segregation of sickle-cell trait in this family enabled us to postulate a defective erythroid trans-acting factor was playing a role in the down-regulation of both beta A- and beta S-globin genes. Among the transcription fa

COVID-19 as a trigger for splenic infarction in a patient with sickle cell trait: A case report.

Sickle cell trait (SCT) is the carrier state for sickle cell disease (SCD) and is usually perceived as a mild condition; however, previous studies have shown that hypoxemia may trigger sickle-cell related complications in these patients, including splenic infarction. Hypoxemia is a common finding in patients with COVID-19 pneumonia. We present the case of a 19-year-old male with a history of epilepsy who presented to the emergency room due to abdominal pain in the left flank that appeared after presenting generalized tonic-clonic seizures and fever. SARS-CoV-2 RT-PCR testing in nasopharyngeal …

Association of sickle cell trait with β-cell dysfunction and physical activity in adults living with and without HIV in Tanzania.

This study aimed to investigate sickle cell trait (SCT) associations with physical activity, markers of insulin secretion and resistance, and glucose among people living with HIV infection (PLWH), both antiretroviral therapy (ART) naive and experienced, and HIV-uninfected adults. This was a cross-sectional study conducted in Mwanza, Northwestern Tanzania. We used data of 668 participants attained from two sub-studies of CICADA study. Mean age was 40 (SD 11.5) years, 402 (61.7%) were females and 157 (24.1%) had SCT. PLWH were 422 (64.7%), of these, 80 (18.9%) were on ART. People with SCT had hi…

Frequencies of peripheral blood myeloid cells in healthy Kenyan children with alpha+ thalassemia and the sickle cell trait.

The high frequencies of both alpha+ thalassemia and the sickle cell trait (hemoglobin AS [HbAS]) found in many tropical populations are thought to reflect selection pressure from Plasmodium falciparum malaria. For HbAS, but not for alpha+ thalassemia, protection appears to be mediated by the enhanced phagocytic clearance of ring-infected erythrocytes. We have investigated the genotype-specific distributions of peripheral blood leukocyte populations in two groups of children living on the coast of Kenya: a group of healthy P. falciparum parasite-negative children sampled at cross-sectional surv…

Factors affecting marriage decisions among couples with sickle cell or beta-thalassemia traits.

OBJECTIVE: This study assessed the prevalence of marriage among genetically incompatible couples at risk of having children with either sickle cell disease (SCD) or beta-thalassemia (β-thalassemia) in Jazan province, KSA, and the factors influencing these couples' decisions to marry. METHODS: An analytical cross-sectional research design was used. From the National Health Premarital Screening System database, couples whose premarital screening tests indicated genetic incompatibility and a risk of having children with either SCD or β-thalassemia were randomly selected, with a sample size of 38

Comorbidity of sickle cell trait and albinism: a cross-sectional survey in the Democratic Republic of the Congo.

INTRODUCTION: Sickle Cell Disease (SCD) and albinism are both recessive hereditary diseases in human kind with a high prevalence in sub-Saharan Africa. This study aimed to determinate the prevalence of sickle cell trait in people living with albinism (PLA). METHODS: a cross-sectional descriptive survey was conducted in PLA attending the "Hôpital du Cinquantenaire de Kisangani". In total, by non-probabilistic convenience sampling, 82 albinos and 139 non-albinos and without any antecedents of albinism in their family were included, selected from students in the Faculty of Medicine and Pharmacy …

[Risks and benefits of genetic screening: the sickle cell trait as a model in a Brazilian population group].

Population screening of asymptomatic heterozygotes for genetic guidance purposes is a very controversial procedure due to the risk of labeling, discrimination, stigmatization, loss of self-esteem, and invasion of privacy. Effects of genetic counseling supplied to blood donors with the sickle cell trait (AS heterozygotes) were investigated. Ninety-two AS donors were invited by letter to a free, individual orientation session; 66 of them (72%) accepted the invitation. Some 50% of the individuals brought their wives and children for examination, thus allowing identification of several at-risk cou…
