A case of primary malignant lymphoma of the skull base is reported. A 62-year-old man was admitted with headache and diplopia. Computed tomography and magnetic resonance imaging showed an abnormal mass in the right parasellar region. The final diagnosis after surgery was primary malignant lymphoma of the skull base. Primary malignant lymphomas sometimes occur in the central nervous system but those in the skull base are exceptional. The present paper presents just such a rare case of malignant lymphoma of the skull base. Diagnosis and combined therapy consisting of surgery, radiotherapy and ch…

A ring-shaped electrode was developed and used in 20 patients to record evoked electromyographic responses directly from the extraocular muscles during skull base surgery. Intra-operative monitoring with this electrode helps the surgeon to localize the nerves that innervate the extraocular muscles precisely and to refrain from disturbing important neural structures during operations. Such monitoring also provides some insight into the pathophysiology of the dysfunction of these nerves resulting from skull base lesions.

Three dimensional computed tomography was performed in 150 children suffering from a bone lesion of the base of the skull and face. These lesions were either malformative, neoplastic, infectious or traumatic. 3D CT provided an essential anatomical approach to the lesions facilitating the choice of the most appropriate treatment. 3D CT is also indicated in the follow-up of these operated children, particularly of the growth of the operated bone segment and its relations with adjacent structures.

In a prospective study of 262 consecutive patients with nasopharyngeal carcinoma (NPC), using computed tomography (CT) as their baseline evaluation, erosion of the base of the skull and intracranial extension into the middle cranial fossa were found in 31.3% and 12.2% of patients, respectively. Thirty-four of these patients had cranial nerve involvement at presentation; 30 of them had involvement of one or more of the third to sixth cranial nerves. Most cases of intracranial extension of tumor were accompanied by erosion of the base of the skull, but the reverse was not true. All patients with

18 patients presenting a circumscribed cerebrospinal fluid (CSF-) leak in the region of the ethmoidal roof, the olfactory cleft or the sphenoidal sinus, were operated on via the endonasal route. Dural laceration could be attributed to paranasal sinus surgery (iatrogenic CSF-leak, N = 11) and to other traumas (N = 6). In one case, no obvious cause was detectable (spontaneous CSF-leak). Using the 70-degree angular optical device, the dural lacerations were closed by autogenous grafts of conchal mucosa with the help of fibrin glue. The average time of postoperative observation was 17 months. No l

Fractures of the skull base are frequently accompanied by dural injuries. Lyophilized dura mater is very suitable for dural reconstruction at the level of the smooth convex posterior wall of the frontal sinus but surgery in the region of the sphinoid tectum and lateral to the falx cerebri is difficult. The Gore-Tex patch is very soft and comfortable. Vascular prostheses are manufactured from this material and have been implanted successfully for years. This material was subjected to a check of histocompatibility in animal experiments by intracranial implantation as a replacement for lyophilize…

While magnetic resonance imaging (MRI) is no longer a tool of the future, its availability is limited. Most hospitals still use high-resolution computed tomography (CT) scanning as their major imaging modality. Although numerous articles point to the improved diagnostic uses of MRI--as compared to high-resolution CT--a change from high-resolution CT scanning to MRI scanning throughout the United States would cost billions of dollars. We should, therefore, delineate those clinical situations in which magnetic resonance imaging is distinctly superior to high-resolution CT scanning. It is already

A 6-year-old girl had a 7-month history of headaches and painful torticollis. A CT scan of the mastoids showed extensive bone destruction of the base of the skull and C-1. Biopsies of the retropharyngeal area and of the anterior aspect of C-1 were performed: histopathologic findings were suggestive of mycotic infection and cultures were positive for Candida albicans. The child was treated successfully with amphotericin B. The immunologic evaluation demonstrated the absence of myeloperoxidase in the neutrophils.

Within the past quarter of a century, contributions to the surgical literature have steadily reflected an increasing interest in the surgical extirpation of lesions involving the skull base. While optimistic reports demonstrate a variety of operative techniques and clinical experiences, there has been a paucity of discussion regarding the intraoperative and perioperative management of these complex cases. Cognizant of the fact that the care of these patients is of paramount importance, we herein review our rationale and practiced protocol for management of this unique patient population. The r…

Tumours of the skull base become manifest by the nervous structures surrounding them. After the removal of the tumour, the complications are produced by the same disturbed structures. Proceeding under microsurgical conditions will reduce both the risk of subsequent haemorrhages in patients with olfactory--groove meningiomas and central disturbances of the respiratory and circulatory--functions.

The median section of the base of the skull offers many diagnostic problems in radiology. Findings of the plain radiographs and tomographs, of computed tomography and angiography are demonstrated in cases of space-occupying growths of the sella region, lesions of the fossa temporalis and of the region of the pyramid of the medulla oblongata, from a differential diagnostic viewpoint. Sequelae of traumas are discussed separately.

BACKGROUND AND OBJECTIVES: Surgical access to the clival region remains a significant challenge because of its midline location, deep anatomic position, and complex neurovascular relationships. The extended endoscopic endonasal approach (EEA) offers a direct, minimally invasive corridor to this intricate region of the skull base. This study presents a comprehensive review of the clinical presentation, surgical indications, and outcomes in a series of 46 patients who underwent EEA for lesions involving the clivus.
METHODS: This was a retrospective study in which we identified cases of clival p…

INTRODUCTION: Robotic-assisted surgery (RAS) is increasingly used in skull-base procedures due to its enhanced visualization, precision, and ergonomics, particularly in anatomically-constrained regions. The present narrative review explores the current role and emerging frontiers of RAS in anterior and lateral skull-base surgeries within otolaryngology and neurosurgery.
METHODS: We conducted a comprehensive narrative review of studies published throughout the past 2 decades on the PubMed, Scopus, and Web of Science databases using keywords such as robotic skull-base surgery , transoral roboti

Based on the characteristic clinical and radiological findings, a diagnosis of Collet-Sicard syndrome secondary to a skull-base paraganglioma was made. This case underscores the importance of through neurological examination and early …
