A multimethod evaluation of the MMPI in a chronic pain patient sample.

Previous reports on the use of the MMPI with chronic pain patients have produced a variety of results. No single configural feature or scale identifies the chronic pain patient, regardless of the origin or verifiability of the etiology of the pain. Elevations on the neurotic triad occur frequently, but do not have the specificity of more recently reported chronic-pain subtypes on the MMPI. The present nonexperimental study (N = 72) provides an example of a multi-method analysis of a carefully selected sample of chronic pain patients without physical findings. Blind clinical analysis, simple tw

Prevalence of psychiatric diagnoses among frequent users of rural emergency medical services.

OBJECTIVE: This study aimed to determine whether there was an increased prevalence of psychiatric disorders among frequent users of rural emergency medical services. METHODS: In a matched comparison design, I compared frequent users of the emergency departments of 2 rural hospitals, both affiliated with an academic centre, with randomly selected users and with randomly selected users who had the same medical diagnoses. The main outcome measures were psychiatric diagnoses on a structured clinical interview, along with medical diagnoses and number of emergency department visits in the past year

The somatic symptom paradox in DSM-IV anxiety disorders: suggestions for a clinical focus in psychophysiology.

Although DSM-IV criteria for anxiety disorders include physiological symptoms, these symptoms are evaluated exclusively by verbal report. The current review explores the background for this paradox and tries to demonstrate on theoretical and empirical grounds how it could be resolved, providing new insights about the role of psychophysiological measures in the clinic. The three-systems approach to evaluating anxiety argues that somatic measures as well as verbal and behavioral ones are indispensable. However, the low concordance between these domains of measurement impugns their reliability an

Unresolved grief.

This paper has reviewed the literature on bereavement and, with particular emphasis on the authors' own work, describes three syndromes which seem to be related to the nonresolution of distinct phases of the grief process. The possibility of unresolved grief should receive a high index of suspicion for the patient with otherwise unexplainable depression, chronic illness behavior, or symptoms similar to those of a deceased relative or friend. When any of these syndromes are identified, it is useful to ask the patient who he has lost, how he has lost them, how he felt about the loss, whether he

Many studies of social anxiety disorder (SAD) have been conducted because diagnostic criteria are defined as social phobia in DSM-III in the … ent criterion for SAD. However, it is confusing that body dysmorphic disorder is not in category of somatoform disorders but in that of obsessive-compulsive disorder and related disorders and that the Japanese terms of "jikoshu-kyofu" and "shubo-kyofu" are categorized as other specified obsessive-compulsive disorder and related disorders. Psychiatric comorbidity and psychosocial impairment among patients with vertigo and dizziness.

BACKGROUND: Vertigo and dizziness are often not fully explained by an organic illness, but instead are related to psychiatric disorders. This study aimed to evaluate psychiatric comorbidity and assess psychosocial impairment in a large sample of patients with a wide range of unselected organic and non-organic (ie, medically unexplained) vertigo/dizziness syndromes. METHODS: This cross-sectional study involved a sample of 547 patients recruited from a specialised interdisciplinary treatment centre for vertigo/dizziness. Diagnostic evaluation included standardised neurological examinations, str

[The ICD-10 Symptom Rating questionnaire for assessment of psychological comorbidities in chronic tinnitus patients].

BACKGROUND: Psychological comorbidities are frequent in tinnitus patients and their diagnosis is important for both interventions as well as treatment success. The selection of suitable questionnaires is thus crucial. The present study aimed to investigate the ICD-10 Symptom Rating (ISR) questionnaire for the diagnosis of psychological comorbidities. METHODS: The current study investigated tinnitus burden and psychological comorbidities in a sample of n = 311 patients with chronic tinnitus. All participants completed an intensive 7‑day multimodal tinnitus-specific therapy. Tinnitus burden was

A revised and extended systematic review and meta-analysis of the relationship between childhood adversity and adult psychiatric disorder.

This study aimed to systematically review the evidence for an association between adversity experienced in childhood (≤ 17 years old), and the diagnosis of psychiatric disorder in adulthood. Electronic databases (Scopus, Medline (for Ovid), EMBASE, and PsychINFO) were searched for peer-reviewed, longitudinal cohort studies examining child or adolescent exposure to adversity, and adult-diagnosed depression, anxiety, psychotic disorder, eating disorders, substance abuse disorder, illness anxiety disorder, somatoform disorder, or personality disorder. A total of 39 manuscripts were retained. Resu

Posttraumatic stress disorder and comorbidity in Australian Vietnam veterans: risk factors, chronicity and combat.

OBJECTIVE: The objective of this study was to examine the relationship between combat-related posttraumatic stress disorder (PTSD) and comorbid DSM-III-R psychiatric diagnoses to determine commonalities in risk factors, relative onsets and the role of combat exposure. METHOD: An epidemiological cohort study using standardised psychiatric, social and health interviews was undertaken with a national random sample of male Australian Army Vietnam veterans. Interviews and searches of military records yielded risk factors for PTSD, which were examined for association with each psychiatric diagnosis

Electroconvulsive therapy related autobiographical amnesia: a review and case report.

Introduction: While short-term cognitive impairment following electroconvulsive therapy (ECT) is well described and acknowledged, the relationship between ECT and persistent memory impairment, particularly of autobiographical memory, has been controversial. Methods: We describe the case of a 70-year-old consultant neurophysiologist, AW, who developed prominent, selective autobiographical memory loss following two courses of ECT for treatment-resistant depression. Results: His performance on standard measures of IQ, semantic and episodic memory, executive function and mood was normal, while he

A multitrait-multimethod analysis of the MCMI-II clinical syndrome scales.

Multitrait-multimethod procedures and factor analyses were utilized to assess convergent/discriminant validity of the revised version of the Millon Clinical Multiaxial Inventory (MCMI-II) clinical syndrome scales and effects of item overlap on scale performance. Except for the Delusional Disorder Scale (PP), convergent validity was supported, but poor discriminant validity was found for the Alcohol Dependence (B) scale, Drug Dependence (T) scale, and Scale PP. Item overlap did not influence performance for many scales. In fact, results indicate that some scales, such as Bipolar:Manic (N), may

Clinical Correlates of Functional Motor Disorders: An Italian Multicenter Study.

BACKGROUND: Functional motor disorders (FMDs) are abnormal movements that are significantly altered by distractive maneuvers and are incongruent with movement disorders seen in typical neurological diseases. OBJECTIVE: The objectives of this article are to (1) describe the clinical manifestations of FMDs, including nonmotor symptoms and occurrence of other functional neurological disorders (FND); and (2) to report the frequency of isolated and combined FMDs and their relationship with demographic and clinical variables. METHODS: For this multicenter, observational study, we enrolled consecut
