Functional outcomes related to the prevention of radiation-induced xerostomia: oral pilocarpine versus submandibular salivary gland transfer.

BACKGROUND: Xerostomia has a devastating impact on oral function and quality of life in patients who receive radiation treatment for head and neck cancer. The purpose of this study was to examine functional outcomes related to 2 saliva-sparing treatments: (1) oral pilocarpine during radiotherapy; or (2) the submandibular salivary gland transfer (SGT) before radiotherapy. METHODS: Sixty-nine patients were recruited (SGT = 36; pilocarpine = 33). Speech intelligibility, swallowing outcomes, and quality of life were assessed at 4 points in time (pretreatment, and 1 month, 6 months, and 12 months …

Listener impressions of alaryngeal communication modalities.

Purpose: Following a total laryngectomy in which the larynx is completely removed, individuals in the USA have three primary options for alaryngeal verbal communication including tracheoesophageal speech (TES), oesophageal speech (ES) and electrolarynx (EL). Using a large sample of participants from across the USA, this study investigated listener impressions of each primary type of alaryngeal communication. As these are the individuals more likely to be participating in social interactions and in positions of hiring for employment, the general public's impressions of TES, ES and EL may be a v

[Speech therapy intervention in phonological disorders from the psycholinguistic paradigm of speech processing].

The aim of this study is to present a survey of speech therapy intervention in phonological disorders (PD). We will examine the concepts of normal phonological development and those involved in PD in order to understand how they have been dealt with, historically, in speech therapy intervention. Lastly, we will describe how evaluation and intervention are carried out from the speech processing paradigm. Phonetic phonological skills allow people to decode the phonic strings they hear so as to be able to gain access to their phonological form and meaning. These abilities also enable them to enco

Communication Experiences of Neurological Voice Disorder Patients With Automatic Voice Assistants and Human Listeners.

OBJECTIVES: Automated voice assistants (AVAs) are increasingly important to modern communication. Neurological voice disorders (NVDs) impair voice quality and speech intelligibility, leading to communication challenges. Although dysphonia disrupts communication with human listeners, the communication experience with AVA is unknown for individuals with NVDs. This study aimed to investigate self-reported communication experiences across AVA and human interactions for individuals with NVDs. STUDY DESIGN: Cross-sectional METHODS: The patient group comprised individuals with NVDs who were planning…

Speech and language development and genotype-phenotype correlation in 49 individuals with KAT6A syndrome.

Pathogenic KAT6A variants cause syndromic neurodevelopmental disability. "Speech delay" is reported, yet none have examined specific speech and language features of KAT6A syndrome. Here we phenotype the communication profile of individuals with pathogenic KAT6A variants. Medical and communication data were acquired via standardized surveys and telehealth-assessment. Forty-nine individuals (25 females; aged 1;5-31;10) were recruited, most with truncating variants (44/49). Intellectual disability/developmental delay (42/45) was common, mostly moderate/severe, alongside concerns about vision (37/

Sphincterplasty for Velopharyngeal Insufficiency in the Child Without a Cleft-Palate: Etiologies and Speech Outcomes.

The aim of this study is to report on speech outcomes following surgery for velopharyngeal insufficiency (VPI) on a broad spectrum of patients without a cleft palate. Inclusion criteria included patients without a cleft palate operated on by a single surgeon (JKW) over a 10-year period and postoperative speech evaluation within 1 year. All patients underwent a sphincter pharyngoplasty. The main outcome measures were perceptual speech assessment using a 6-point scale (1 = none or normal, 6 = severe); velopharyngeal function (VPF) (1 = adequate, 2 = marginal, 3 =  dequate); and quantitative nasa

Using Crowdsourced Listeners' Ratings to Measure Speech Changes in Hypokinetic Dysarthria: A Proof-of-Concept Study.

Purpose Interventions for speech disorders aim to produce changes that are not only acoustically measurable or perceptible to trained professionals but are also apparent to naive listeners. Due to challenges associated with obtaining ratings from suitably large listener samples, however, few studies currently evaluate speech interventions by this criterion. Online crowdsourcing technologies could enhance the measurement of intervention effects by making it easier to obtain real-world listeners' ratings. Method Stimuli, drawn from a published study by Sapir et al. ("Effects of intensive voice t

Increased Reporting of Speech in Degraded Stimuli in Schizophrenia: A Case Control Study with Sine-Wave-Speech.

BACKGROUND AND HYPOTHESIS: People who hear voices may have strong prior expectations of speech, so that noisy auditory signals are resolved as speech. Data in non-clinical voice hearers suggest that voice hearing may involve sensitivity to speech in degraded stimuli. This has yet to be examined in people with schizophrenia (SZ). STUDY DESIGN: In this case-control study, we presented sine-wave-speech (SWS; made by replacing the formants in speech with pure tone whistles) to people with SZ (n = 63) and healthy controls (HC; n = 27). SWS is typically unintelligible on first exposure. However, on

Restrictions in oral functions caused by oral manifestations of epidermolysis bullosa.

Several forms of epidermolysis bullosa (EB) present oral manifestations. Blistering of the (peri)oral mucosa affects the opening of the mouth, the mobility of the tongue and lips, thereby restricting oral functions. We describe the prevalence and characteristics of oral manifestations of EB in relation to loss of oral functions in a cross-sectional study of different types of EB patients using standardized measurement techniques. Twenty-two patients were included. The mobility of the mandible, lips and tongue was measured, the mandibular function impairment questionnaire (MFIQ) was filled out

Phenotype-Related Characteristics of Tongue Motor Function in Amyotrophic Lateral Sclerosis: A Retrospective Comparative Study of Static and Dynamic Acoustic Measures.

This study aimed to characterize lingual motor function in patients with amyotrophic lateral sclerosis (ALS) by analyzing static acoustic indices obtained from sustained vowels and dynamic indices derived from sentence reading. The following aspects were examined: (1) the independence of static and dynamic indices, (2) the relationship between F2 transition duration and F2 excursion, (3) differences in vowel formant frequencies across ALS phenotypes, (4) associations between static indices and speech intelligibility, and (5) acoustic elements contributing to static indices. Sixty-eight patient…

Transforming assessment of speech in children with cleft palate via online crowdsourcing.

OBJECTIVE: Speech intelligibility is fundamental to social interactions and a critical surgical outcome in patients with cleft palate. Online crowdsourcing is a burgeoning technology, with potential to mitigate the burden of limited accessibility to speech-language-pathologists (SLPs). This pilot study investigates the concordance of online crowdsourced evaluations of hypernasality with SLP ratings of children with cleft palate. METHODS: Six audio-phrases each from children with cleft palate were assessed by online crowdsourcing using Amazon Mechanical Turk (MTurk), and compared to SLP's gold…

Impact of SNR, masker type and noise reduction processing on sentence recognition performance and listening effort as indicated by the pupil dilation response.

Recent studies have shown that activating the noise reduction scheme in hearing aids results in a smaller peak pupil dilation (PPD), indicating reduced listening effort, at 50% and 95% correct sentence recognition with a 4-talker masker. The objective of this study was to measure the effect of the noise reduction scheme (on or off) on PPD and sentence recognition across a wide range of signal-to-noise ratios (SNRs) from +16 dB to -12 dB and two masker types (4-talker and stationary noise). Relatively low PPDs were observed at very low (-12 dB) and very high (+16 dB to +8 dB) SNRs presumably du

Digital assessment of speech in Huntington disease.

BACKGROUND: Speech changes are an early symptom of Huntington disease (HD) and may occur prior to other motor and cognitive symptoms. Assessment of HD commonly uses clinician-rated outcome measures, which can be limited by observer variability and episodic administration. Speech symptoms are well suited for evaluation by digital measures which can enable sensitive, frequent, passive, and remote administration. METHODS: We collected audio recordings using an external microphone of 36 (18 HD, 7 prodromal HD, and 11 control) participants completing passage reading, counting forward, and counting

Speech outcome following palatoplasty in primary school children: do lay peer observers agree with speech pathologists?

The aim of this study was twofold: (1) to test the ability of normal children to discriminate the speech of children with repaired cleft palate from the speech of unaffected peers and (2) to compare these naive assessments of speech acceptability with the sophisticated assessments of speech pathologists. The study group (subjects) was composed of 21 children of school age (aged 8 to 12 years) who had undergone palatoplasty at a single cleft center and 16 matched controls. The listening team (student raters) was composed of 20 children who were matched to the subjects for age, sex, and other va

Velopharyngeal insufficiency treated with levator muscle repositioning and unilateral myomucosal buccinator flap.

PURPOSE: Velopharyngeal insufficiency (VPI) is common (20-30%) after cleft palate closure. The myomucosal buccinator flap has become an important treatment option for velopharyngeal insufficiency; however, published studies all use bilateral buccinator flaps. This study assesses outcomes with a unilateral myomucosal buccinator flap that might result in less operating time and might prevent the need of a bite block and an extra procedure for division of the flap pedicle at a later stage. MATERIALS AND METHODS: Forty-two consecutive patients who underwent a unilateral myomucosal buccinator flap
