The pathogenesis of premature craniosynostosis in acrocephalosyndactyly (Apert's syndrome). A reconsideration.

A detailed examination of a stillborn fetus with Apert's syndrome showed several unexpected findings, which prompted a reevaluation of the heretofore generally accepted hypotheses regarding the cause of the dysmorphic craniofacial features in this syndrome. (1) The characteristic clinical features of Apert's syndrome were present at 24 to 26 weeks of gestation, and presumably much earlier. (2) However, there was no evidence, clinical or histological, of premature fusion of any of the cranial sutures or basilar synchondroses in this fetus. From our observations, and from a critical review of th

Inner ear modifications in Dar-es-Soltane II H5 (Morocco): A case of labyrinthitis ossificans.

OBJECTIVE: This paper presents the inner ear modifications in Dar-es-Soltane II H5, an Aterian fossil possibly dated to 100 ka. MATERIAL: The remains consist of a large portion of the cranium including the face, the left frontal and temporal bones, part of the left parietal bone and greater wing of the sphenoid. METHODS: The bony labyrinth anatomy was investigated on existing micro-CT data acquired by the MPI-EVA. RESULTS: The observation of micro-CT sections revealed a partial filling of the semi-circular canals that raises question about its origin. A careful examination of the micro-CT s

A probable case of holoprosencephaly with cyclopia in a full-term fetus from a modern skeletal collection.

OBJECTIVE: This study presents evidence of a probable case of holoprosencephaly with cyclopia, which has been rarely reported in the paleopathological literature. MATERIALS: The skeletal remains of a male fetus between 36 and 40 gestational weeks from the Collezione Antropologica LABANOF (CAL) Milano Cemetery Skeletal Collection were studied. METHODS: The bones were macroscopically examined, and pathological anomalies were recorded and evaluated alongside paleopathological and clinical literature. RESULTS: Developmental anomalies were observed. In particular, a single orbit and optical cana

Severe Thoracic and Spinal Bone Abnormalities in neurofibromatosis type 1.

Neurofibromatosis type 1 (NF1) is an autosomal dominant, multi-system, neurocutaneous disorder that predisposes to the development of benign and malignant tumors. Classical skeletal abnormalities encompass sphenoid wing dysplasia, congenital bowing of the long bones and vertebral osteopathy associated with non-dystrophic or dystrophic scoliosis found in about 10% of NF1 patients. We report a 17-year-old boy affected by NF1 with extreme severe spinal and thoracic malformations affecting bone and lung tissues, including hypoplasia of the right lung, unilateral costal agenesis and severe dystroph

Severe Plagiocephaly Due to Isolated Left Frontosphenoidal Craniosynostosis: An Exceptional Presentation.

Craniosynostosis is a condition characterized by premature fusion of cranial sutures, leading to abnormal head shapes and potential developmental and neurological complications. Although craniosynostosis typically involves major cranial sutures, isolated frontosphenoidal suture fusion represents an exceptionally rare entity. In this case report, we present a 2-year-old girl with a rare occurrence of early closure of the left frontosphenoidal suture, resulting in significant left-sided plagiocephaly. After frontal-orbital advancement surgery with barrel-stave osteotomies, the patient experience

Revisiting a Rare Cause of Plagiocephaly: Frontosphenoidal Craniosynostosis.

Frontosphenoidal craniosynostosis remains a rare cause of anterior plagiocephaly, with only 49 cases reported worldwide. Delayed diagnosis may lead to significant aesthetic and functional consequences. This case series presents 2 recently treated patients, highlighting clinical features, imaging findings, and surgical management. A retrospective review of 2 patients diagnosed and treated at our multidisciplinary head shape clinic was conducted, with parental consent obtained. Both children presented with unilateral brow flattening and preserved brow height. Imaging confirmed frontosphenoidal s

A De Novo Sphenoparietal Dural Arteriovenous Fistula: Unveiling the Deceitful Culprit.

INTRODUCTION: Intracranial dural arteriovenous fistulas (DAVFs) are an uncommon pathology, and a sphenoparietal drainage pattern is certainly rare. We present a case of a de novo sphenoparietal DAVF in a prepubescent patient previously treated for a cerebellar arteriovenous malformation (AVM) 10 years before. CASE DESCRIPTION: A 10-year-old boy presented with worsening headaches for the past few weeks, swelling of the right side of face and eye, and chemosis and proptosis of the right eye. Of note, the patient had been treated successfully for a cerebellar AVM at 6 weeks of age with advanced

Isolated frontosphenoidal synostosis: a rare cause of synostotic frontal plagiocephaly.

OBJECT: Unilateral fusion of the frontoparietal suture is the most common cause of synostotic frontal plagiocephaly. Localized fusion of the frontosphenoidal suture is rare but can lead to a similar, but subtly distinct, phenotype. METHODS: A retrospective chart review of the authors' craniofacial database was performed. Patients with isolated frontosphenoidal synostosis on CT imaging were included. Demographic data, as well as the clinical and radiographic findings, were recorded. RESULTS: Three patients were identified. All patients were female and none had an identifiable syndrome. Head c

Posterior dislocation of mandibular condyle into external auditory canal. A case report.

A case is reported of a 49-year-old edentulous patient who suffered a posterior dislocation of the right fractured mandibular condyle causing a fracture of the tympanic plate. This was associated with a right transverse fracture of the petrous bone and an intact tympanic membrane. Some of the problems related to this condition are discussed.

Attempted validation of a method to locate entry and exit foramens of the branches of the trigeminal nerve; benefits for an original cephalometric analysis technique.

INTRODUCTION: Trigeminal analysis focuses on the skeletal entrance and exit orifices of the sensitive fibers of the trigeminal nerve. The aim of this study was to validate the techniques used to locate these landmarks as described by the creator of trigeminal analysis of the face. MATERIALS AND METHOD: This descriptive study was performed on a dry human skull. Two tin balls forming markers R1 and R2 were fixed at random on the skull in a median sagittal position. Two headfilms of the skull were made. The first showed tin balls fixed at the entrance and exit foramens of the sensitive fibers of

Validation of a grading system for the attachment of the superior turbinate to the sphenoid face.

BACKGROUND: The attachment of the superior turbinate to the sphenoid face may be an important factor in determining the approach for sphenoidotomy. We sought to validate a previously described 4-type grading system for superior turbinate attachment (Type: A, within its medial one-third; B, in its middle one-third; C, to its lateral one-third; and D, directly to the orbit) to the face of the sphenoid sinus and to make recommendations for its use in determining the method of sphenoidotomy (transethmoidal vs transsphenoethmoidal). METHODS: Single-slice images through both sphenoid sinus ostia we

Differential closure of the spheno-occipital synchondrosis in syndromic craniosynostosis.

BACKGROUND: The spheno-occipital synchondrosis is a driver of cranial base and facial growth. Its premature fusion has been associated with midface hypoplasia in animal models. The authors reviewed computed tomographic scans of patients with Apert and Muenke syndrome, craniosynostosis syndromes with midface hypoplasia, to assess premature fusion of the spheno-occipital synchondrosis when compared with normal controls. METHODS: Ninety head computed tomographic scans of Apert syndrome patients and 31 head scans of Muenke syndrome patients were assessed, in addition to an equal number of control

[Assymmetry of the axes of the mandibular condyles in the horizontal plane. Preliminary note].

On X-rays taken in the Hirtz position, the aurthors measure:--the axes of the condyles,--the axes of the petrous temporal bone, --The condylo-pterygoid axes. Variations in these axes are studied. The axes of the petrous temporal bones would appear to be a fixed indicator for the structures of the base of the skull.
