Urethral pressure measurement in stress incontinence: does it help?

INTRODUCTION AND OBJECTIVES: The resting urethral pressure profile (UPP), used for the assessment of women with stress incontinence, is routine in many urodynamic units. It is time- and effort-consuming, and its diagnostic value is controversial, as well as its value in the prediction of outcome of anti-incontinence surgery. Herein, we assessed its value in the prediction of the outcome of surgery. PATIENTS AND METHODS: Sixty women were randomized to fascial sling or TVT. Urodynamics were performed preoperatively, 6 months and annually thereafter. After filling and voiding cystometry, resting

Laparoscopic colposuspension versus vaginal suburethral slingplasty: a randomised prospective trial.

BACKGROUND: This study aimed to determine if laparoscopic colposuspension (LC) was as effective as vaginal suburethral slingplasty (SPARC). METHODS: Ninety-seven women with urodynamic stress incontinence were prospectively randomised to LC (n=48) or SPARC (n=49). Outcome measures were measured at, baseline, six months (n=87) and two years (n=58), and comprised leakage episodes per week and visual analogue scale (VAS) of incontinence severity. The LC and SPARC groups at baseline had similar leaks per week (8.8 vs 9.8) and VAS (5.6 vs 5.9). RESULTS: Laparoscopic colposuspension took longer to

Voiding dysfunction in older men.

PURPOSE OF REVIEW: Voiding dysfunction in older men is an important health issue, with significant morbidity and biosocioeconomic burden. Treatment decisions are increasingly complex as many older men also suffer concurrent comorbidities and polypharmacy. A relatively sparse number of publications specifically examine this relevant topic. RECENT FINDINGS: Common geriatric voiding syndromes include overactive bladder (OAB) and underactive bladder, with nocturia and incontinence often the most bothersome lower urinary tract symptoms, and may represent a falls risk together with OAB and incontin

SlingVR: A Virtual Reality Surgical Training System for Haptic-based "Blind" Surgical Procedures.

STUDY OBJECTIVE: "Blind" procedures are performed without visual feedback and rely on haptics to complete the operation safely "by feel." Current training methods, including static models, cadaver sections, or supervised practice on patients, are expensive, time-consuming, and could impact patient safety. Simulation trainers that provide haptic feedback are still limited in their ability to reproduce realistic feel and accurately provide performance data. This study describes the concept, development, and usability evaluation of the SlingVR Surgical Training System for "blind" procedures. DES

Postoperative Body Weight Changes and Effect on Midurethral Sling Failure.

IMPORTANCE: Little is known about the effect of weight change following surgery on midurethral sling outcomes. OBJECTIVE: The objective of this study was to determine the effect of body weight change after midurethral sling on the 24-month subjective outcome. We hypothesized that body weight gain would increase the chance of subjective failure. STUDY DESIGN: This was a secondary analysis of the Trial of Mid-Urethral Slings. The primary outcome was subjective failure at 24 months postoperatively. Secondary outcomes include objective failure and combined (objective/subjective) failure at 24 mo

What maximal urethral closure pressure threshold predicts failure of mid-urethral sling surgery?

OBJECTIVE: Low Maximal Urethral Closure Pressure (MUCP) is linked to unfavourable outcome of anti-incontinence surgery, however the cut-off value varied within studies. This study aimed to predict the cut-off value of MUCP that contributes to poor outcome of Mid-Urethral Sling (MUS) surgery in Urinary Stress Incontinence (USI) patients. MATERIALS AND METHODS: Records of 729 women underwent MUS procedure from January 2004 to April 2017 reviewed. Patients were divided into four MUCP groups, which were <20 cmH2O (≥20 and < 40) cmH2O (≥40 and ≤ 60) cmH2O and >60 cmH2O. Objective evaluation compri

Complications following retropubic versus transobturator midurethral synthetic sling placement.

INTRODUCTION AND HYPOTHESIS: Transobturator slings (TOS) are inferior to retropubic slings (RPS) based on long-term outcomes; data on complications is critical for patient counseling. We hypothesized rates of urinary retention would be higher for RPS, while pain and repeat sling surgery would be higher for TOS. METHODS: Using the Premier healthcare database we identified encounters for patients undergoing a midurethral sling procedure between 2010 and 2020. Patients were stratified by sling type, either RPS or TOS. The primary outcome was the difference in the composite complication rate betw

Guiding Clinical Decision Making for Surgical Incontinence Treatment After Prostatectomy: A Review of the Literature.

PURPOSE OF REVIEW: Stress urinary incontinence after prostatectomy is a common and debilitating side effect. Immediate post-prostatectomy management emphasizes pelvic floor muscle exercises. Per American Urologic Association guidelines, if incontinence persists for more than 12 months postoperatively, surgical interventions are the mainstay of treatment. Treatment decisions depend on a multitude of factors. The goal of this paper is to review recent literature updates regarding the diagnosis of male SUI to better guide surgical treatment decision-making. RECENT FINDINGS: Patient history is a
