BACKGROUND: Myocardial bridging (MB) may cause myocardial ischemia through impaired diastolic coronary perfusion and coronary vasomotor dysfunction, occasionally presenting as exertional presyncope or syncope.
CASE SUMMARY: We report 2 patients with exertional presyncope/syncope caused by hemodynamically significant MB and concomitant epicardial coronary spasm. Coronary angiography showed no obstructive disease. Acetylcholine provocation induced diffuse spasm in the mid- to distal left anterior descending artery, whereas intracoronary nitroglycerine unmasked systolic compression of a mid-left…

BACKGROUND: It is generally believed that cerebral hypoperfusion in patients with vasovagal syncope (VVS) is secondary to hypotension; however, current evidence suggests that this is not always the case. Thus, this study aimed to analyze the hemodynamic characteristics of patients with VVS and dynamic cerebral autoregulation (CA) dysfunction (referred to as CA-impaired VVS) to improve the diagnosis and treatment of this VVS subtype.
METHODS: This retrospective study included 143 patients with VVS who underwent the head-up tilt test (HUTT) using transcranial Doppler (TCD). Patients were divide…

Spinal anesthesia (SA) is increasingly recognized as a preferred alternative to general anesthesia (GA). Nonetheless, several factors must be carefully considered to ensure safe and practical application. Vasovagal syncope (VVS) or vasovagal reaction, characterized by a sudden decrease in heart rate (HR) and/or blood pressure (BP), is common during pain management procedures and SA administration in patients undergoing surgery. We report the case of a 57-year-old male who underwent SA during penoscrotal inflatable penile prosthesis (IPP) implantation. During the corporotomy closure, the patien…

Sudden cardiac death is defined as an abrupt, unexpected death of cardiovascular cause with loss of consciousness within 1 hour of onset of symptoms. In an effort to prevent these events, clinicians need to recognize symptoms to identify at risk patients. There is often an overlap in symptoms of chest pain, palpitations, and syncope. The workup depends on the characteristics of these symptoms. The history and physical examination often provide adequate information, but additional testing and referral to pediatric cardiology are sometimes indicated.

The Centers for Disease Control and Prevention identified the first case of the novel coronavirus disease 2019 (COVID-19) on January 21, 2020 in the United States. Since its arrival, the virus has caused widespread havoc on the nation as a whole as well as on all individuals. The coronavirus family is not new to the field of medicine. In fact, the viral pathogenicity dates back to the early 1960s, with more information on the respiratory preference and the ability to cause acute respiratory pathology coming later in 2002. The novel coronavirus, severe acute respiratory syndrome coronavirus 2,

AIMS: Tilt testing (TT) is recognized to be a valuable contribution to the diagnosis and the pathophysiology of vasovagal syncope (VVS). This study aimed to assess the influence of age on TT responses by examination of a large patient cohort.
METHODS AND RESULTS: Retrospective data from three experienced European Syncope Units were merged to include 5236 patients investigated for suspected VVS by the Italian TT protocol. Tilt testing-positivity rates and haemodynamics were analysed across age-decade subgroups. Of 5236 investigated patients, 3129 (60%) had a positive TT. Cardioinhibitory respo…

OBJECTIVE: To characterize the quantitative electroencephalographic (QEEG) patterns associated with tilt-induced syncope in youth.
METHODS: Several QEEG parameters were analyzed. Data were calculated for peak or nadir changes with syncope for amplitude-EEG, fast Fourier transform (FFT) power in several frequency ranges, 8-13 Hz/1-4 Hz frequency ratio, and FFT edge.
RESULTS: Changes in QEEG parameters were present among all patients with tilt-induced syncope (n = 76). These changes included increases in the low frequency FFT power (1-4 Hz range), decreases in the power ratio (8-13 Hz/1-4 Hz) …

BACKGROUND: Initial orthostatic hypotension (IOH) is a clinical syndrome of transient orthostatic hypotension that is defined as a drop in blood pressure of >40 mm Hg systolic and/or >20 mm Hg diastolic within 15 s after standing, accompanied by symptoms of cerebral hypoperfusion, but without sustained orthostatic hypotension (blood pressure decrease >20/10 mm Hg after 1-3 min of standing). As the etiology of syncope remains unknown in a large proportion of patients, we hypothesized that IOH is highly prevalent among patients with unexplained syncope.
METHODS: We studied 250 consecutive outpa…

Swallow syncope is a relatively rare syndrome and caused by various foods and drinks. A 76-year-old man was admitted with frequent syncope while eating. Holter electrocardiogram revealed frequent occurrence of atrioventricular block during meals. Both atrioventricular block and sinus arrest were induced by only eating citrus fruits, citrus jelly, and acidic foods but not by other drinks and foods. These arrhythmias were suppressed after administration of atropine. No further episodes of syncope recurred after the implantation of a DDD pacemaker. This case indicated that acidic stimulation of c…

Manipulation of vagal nerve rootlets, whether surgical or through mass effect of a neoplasm, can result in asystole and hypotension, accompanied by ST depression and right bundle branch block. There are few case reports of a neoplasm causing these effects, and this case describes a patient with such a mass presenting with syncopal episodes. A 43-year-old man with a past medical history of HIV, bipolar disorder, and epilepsy was admitted to the neurology service for a video electroencephalogram (vEEG) to characterize syncopal episodes that were felt to be epileptic in origin. During the study,

Patients with syncope, without presence of a structural or primary electrical disease, have a very good prognosis. On the other hand, recurring syncope has a negative impact on quality of life and involves a real risk of physical injuries. Vasovagal syncope usually requires no medical therapy and the most commonly recommended instruction include avoidance of fainting triggers, exercise that mainly provoke leg muscles tension, increase salt consumption and drink indicated amount of fluids. In the case cardioinhibitory type of vasovagal syncope the pacemaker implantation can consider. Unfortunat…

The patient presented with syncope, without chest pain and dyspnea. There was no peripheral edema or nervous system signs. The electrocardiogram (ECG) showed QT-interval prolongation with T-wave inversion in anterior and inferior leads. T-wave inversion in the right-sided precordial leads should prompt consideration of right ventricular overload. The patient underwent computed tomography of the chest that demonstrated pulmonary embolism. Ultrasonography of the veins of the lower limbs revealed an isolated calf vein thrombosis.

PURPOSE: To report a patient who fell unconscious because of vasovagal syncope evoked by needle phobia when he tried to wear contact lenses.
CASE REPORTS: A 16-year-old healthy boy had sometimes experienced dizziness when looking at the tips of pens. When he put his finger and the lens close to his eye to insert a soft contact lens, he felt sick and dizzy and fell unconscious.
CONCLUSION: Our experience suggests that patients with needle phobia may develop vasovagal syncope by concentrating on a fingertip when inserting a contact lens. We need to be aware of unconsciousness because of phobia…
