Fever after a cardiac operation is commonly caused by myocardial infarction, postpericardiotomy syndrome, post-myocardial infarction syndrome (Dressler's syndrome), or postperfusion syndrome resulting from cytomegalovirus infection. Postpericardiotomy syndrome and post-myocardial infarction syndrome are autoimmune disorders characterized by eosinophilia, pleuritic chest pain, and pleural effusions. In contrast, the diagnosis of postperfusion syndrome caused by cytomegalovirus is suggested if the patient has a mild sore throat, no pleural component or chest pain, no eosinophilia, and atypical l…

Down syndrome, trisomy of chromosome 21, is well investigated because it is a genetic disease with characteristic mental retardation and precocious dementia of Alzheimer type. Maternal serum markers of human chorionic gonadotrophin unconjugated estriol and amyloid precursor protein, nuchal skinfold on ultrasound and new genetic probes are developed to allow better detection of Down syndrome. The overproduction of A beta 42 because of excessive genes is thought to be a leading factor for early onset of dementia in Down syndrome adults. Animal models and transgenic mice may be helpful in determi…

The authors describe a case of Sheehan's syndrome that followed hemorrhagic complications due to late abortion, and emphasise the infrequency of changes in plasma lipid levels in this disease. They report a particular case of hypopituitarism and point out the main concepts of pathophysiology, diagnosis, therapeutics and complications that follow this increasingly rarer condition.

Rumination is a syndrome characterized by repetitive regurgitation of small amounts of food from the stomach. The food is then partially or completely rechewed, reswallowed, or expelled. This syndrome is relatively common in infants and mentally challenged persons, but it also occurs in adults with normal intelligence. The rumination syndrome is an underappreciated condition in adults who frequently receive a misdiagnosis of vomiting due to gastroparesis or gastroesophageal reflux. Difficulties in establishing the correct diagnosis may be caused by a lack of awareness of the condition among ph…

Hepatorenal syndrome is a common complication in patients with advanced cirrhosis and ascites characterized not only by renal failure but also by marked alterations in systemic hemodynamics and vasoactive systems. Renal failure is due to a marked hypoperfusion of the kidney secondary to renal vasoconstriction. The pathogenesis of hepatorenal syndrome is not completely known but it is thought to be the extreme manifestation of the underfilling of the arterial circulation secondary to an arterial vasodilation, located mainly in the splanchnic circulation. Recently, a new definition and diagnosti…

Boerhaave's syndrome or spontaneous oesophageal perforation, is a potentially lethal and frequently elusive medical condition which presents not only a diagnostic but also a therapeutic challenge. It is insufficiently considered in diagnostic hypotheses, yet may be confirmed or excluded by simple methods such as an erect chest film and a contrast study of the oesophagus. Errors in diagnosis are usually caused by unawareness of its varied and atypical presentations or failure to consider its possibility in acute cardiothoracic and upper gastrointestinal conditions. Early aggressive surgical int…

Felty's syndrome is a complication of rheumatoid arthritis whereby patients develop neutropenia of varying severity. Although the main clinical concern is the development of serious infections, often patients remain asymptomatic or continue with clinical problems related to the rheumatoid arthritis and not to the neutropenia. There is now considerable clinical experience with the use of the recombinant human haemopoietic growth factors granulocyte and granulocyte-macrophage colony-stimulating factors (G-CSF and GM-CSF) in the treatment of patients with Felty's syndrome. The only indication for…

Two cases of Mirizzi's syndrome are reported. The syndrome consists of: external compression of the common bile duct due to a benign lesion (Type I)--presence of a cholecystobiliary fistula with impacted stones and partial or total obstruction of the hepatic duct (Type II). Clinical signs are non-specific and suggest at first sight an obstructive jaundice. Ultrasonography shows dilatation of the upper biliary tract with narrowing of the biliary tract below the dilatation. ERCP often proves mandatory for diagnosis. Therapeutical procedures are reported.

We present 3 cases of Capgras' syndrome-a delusional disorder in which the patient believes that 1 (or more) of his acquaintances has been replaced by an imposter who appears as a double. 2 were schizophrenics and 1 had depression with psychotic features. This syndrome is rare in our practice, but we do not know if this is due to lack of awareness of the condition, or to the possibility that it is a culture-related syndrome. We suggest that although the syndrome has lost some of it's significance, it is still worth making the diagnosis because of the medical and psychological implications this…

The author describes three forms of Marfan's syndrome which have only the skeletal symptoms in common. In all instances also ophthalmological symptoms are present but of different types. A familial case was observed only in one family; in the remaining two frust forms of Marfan's syndrome are involved.

Auditory-pigmentary syndromes are caused by physical absence of melanocytes from the skin, hair, eyes, or the stria vascularis of the cochlea. Dominantly inherited examples with patchy depigmentation are usually labelled Waardenburg syndrome (WS). Type I WS, characterised by dystopia canthorum, is caused by loss of function mutations in the PAX3 gene. Type III WS (Klein-Waardenburg syndrome, with abnormalities of the arms) is an extreme presentation of type I; some but not all patients are homozygotes. Type IV WS (Shah-Waardenburg syndrome with Hirschsprung disease) can be caused by mutations …

Sjogren's syndrome is a chronic inflammatory disease of unknown aethiology. It is characterized by decreased secretion of salivary and lacrimal glands, which induces keratoconjunctivitis sicca and xerostomia. Sjogren's syndrome is a central autoimmune disease, and it has characteristics of both organ-specific and generalized autoimmune diseases. It can exist as a primary disease or is associated with other autoimmune diseases (most freyuently with systemic lupus erythematosus or rheumatoid arthritis) and is classified as a secondary Sjogren's syndrome. The aethiology is multifactorial, and it …

There is presented a case a 42 year old woman, who was admitted to the Department of Internal Diseases, the Institute of Dentistry, Medical Academy in Warsaw, with suspected bacterial endocarditis. Two episodes which indicate a lesion in the central nervous system (as right side hemiparesis and mixed aphasia) in the patients with valvular heart disease, were the basis of this suspicion. Bacterial endocarditis was not confirmed during hospitalisation. Diagnosis of Sneddon's syndrome was established based on skin lesion of livedo reticulatis type with typical picture in skin biopsy and on the le…
