Clinical patterns of patients with temporal lobe epilepsy and pure amygdalar atrophy.

PURPOSE: MRI volumetric measurements (MRIvol) have been proven reliable in determining mesial temporal atrophy in patients with TLE. We attempted to correlate the clinical features with different patterns of hippocampal formation (HF) and amygdala (AM) atrophy in patients with TLE without foreign tissue lesion. METHODS: We studied 65 patients with refractory TLE. They were divided into five groups according to MRIvol results: pure AM atrophy (n = 11, 10 unilateral and one bilateral), unilateral HF atrophy (n = 16), bilateral HF atrophy (n = 12), unilateral AM + HF atrophy (n = 13), and patien

Lesionectomy in epileptogenic temporal lobe lesions: preoperative seizure course and postoperative outcome.

A series of 54 patients operated on for temporal epileptogenic lesions is reported: 36 had slow growing tumours, 18 supratentorial cavernous angiomas. The patients were divided into two different groups according to the presence of seizures controlled (group 1) or not controlled (group 2) by antiepileptic drugs (AEDs). All the patients underwent preoperative scalp EEG and magnetic resonance imaging (MRI). They were operated on by pure lesionectomy, associated with amygdalo-hippocampectomy in 8 cases of uncontrolled seizures. Postoperatively they underwent MRI examination which revealed an inco

Progressive prosopagnosia associated with selective right temporal lobe atrophy. A new syndrome?

V.H., a 68-year-old right-handed woman, presented with a progressive deterioration in her ability to recognize faces of familiar people, inc … d MRI revealed selective hypoperfusion and atrophy, respectively, of the anterior part of the right temporal lobe. In recent years there have been a number of descriptions of progressive fluent aphasia resulting from atrophy of the left temporal lobe. This case appears to represent a corresponding degenerative process affecting the right temporal lobe.

Traumatic compared to non-traumatic clinical-pathologic associations in temporal lobe epilepsy.

This study determined differences in clinical-pathologic characteristics of intractable temporal lobe epilepsy (TLE) patients whose mechanism of cerebral injury and chronic seizures involved a prior history of cerebral trauma compared to those with non-traumatic initial injuries. TLE patients (n = 120) from a single epilepsy center were retrospectively and blindly catalogued into pathogenic groups (independent variables) based on if there was a significant Birth injury (n = 11) or Cerebral trauma (n = 26). These two 'trauma' categories were compared to TLE patients with non-seizure non-trauma …

Progressive degeneration of the right temporal lobe studied with positron emission tomography.

A 79 year old man with a twelve year progressive history of prosopagnosia and recent naming difficulty, in whom other intellectual skills were preserved, is described. Positron emission tomography (PET) revealed an area of right temporal lobe hypometabolism, with an additional area of less severe hypometabolism at the left temporal pole. This may represent an example of progressive focal cortical degeneration similar to that associated with primary progressive dysphasia, but affecting the right temporal lobe.

Reproductive endocrine disorders in women with partial seizures of temporal lobe origin.

Of 50 consecutive women with partial seizures of temporal lobe origin (temporal lobe epilepsy [TLE]) evaluated for reproductive dysfunction, 28 had menstrual problems. Of those, 19 had reproductive endocrine disorders. Polycystic ovarian syndrome and hypogonadotropic hypogonadism occurred significantly more often in women with TLE than in the general female population. Polycystic ovarian syndrome was associated with predominantly left-sided lateralization of interictal epileptic discharges; hypogonadotropic hypogonadism was more commonly found with right-sided discharges. Hyposexuality occurre…

Multiple personality and related dissociative phenomena in patients with temporal lobe epilepsy.

Many patients with temporal lobe epilepsy also experience dissociative episodes. Three patients with multiple personality exhibited alterations in speech pattern, personality, handedness, and sense of personal identity and claimed amnesia for the dissociative episodes. Another 10 patients identified alternative personalities or demons as motivators of ego-alien behavior. Of clinic patients with temporal lobe epilepsy, 33% exhibited some dissociative phenomena, which had no apparent association with individual seizures but always followed the development of the seizure disorder. The authors pro…

Acquired learning disability for reading after left temporal lobe damage in childhood.

A 6-year-old boy developed hemorrhage in the left temporal lobe and subsequently underwent left temporal lobectomy. Afterwards, he was completely unable to learn to read or write. Psychological tests showed impairment in verbal learning and memory, with right ear and right visual field superiority in verbal perception. A Wada test showed no aphasia after right carotid injection. We conclude that damage to the speech-dominant temporal lobe resulted in the inability to learn to read or write. Despite the lesion, speech remained strongly lateralized to the left hemisphere, accounting for the righ…

Subunit specific altered expression and activity of casein kinase 2 in the brain tissues resected from mesial temporal lobe epilepsy with hippocampal sclerosis patients & rodent temporal lobe epilepsy model.

INTRODUCTION: Mesial temporal lobe epilepsy (MTLE), is associated with dysregulated excitatory-inhibitory balance in the brain. Numerous enzymes, protein kinases, that are modulated through phosphorylation, have been linked with key processes involved in the pathogenesis of epilepsy. Therefore, in this study, we determined the subunit specific expression and activity of multi-subunit casein Kinase 2 (CK2) which influences NMDARs through phosphorylation events, in MTS patients as well as pilocarpine model of TLE. METHODS: mRNA expression of CK2 (α, α', β) & NR2B was measured by real time PCR a…

Differential regulation of excitatory synaptic transmission in the hippocampus and anterior temporal lobe by cyclin dependent kinase 5 (Cdk5) in mesial temporal lobe epilepsy with hippocampal sclerosis (MTLE-HS).

Mesial temporal lobe epilepsy with hippocamapal sclerosis (MTLE-HS) is the most common form of drug resistant epilepsy (DRE). MTLE-HS is a distributed network disorder comprising of not only the hippocampus, but other anatomically related extrahippocampal regions. Excitatory synaptic transmission is differentially regulated in the hippocampal and extra-hippocampal regions of patients with MTLE-HS, but its mechanism not understood. Cyclin-dependent kinase 5 (Cdk5) is known to regulate synaptic transmission and plasticity through up-regulation of NMDA receptors by phosphorylating NR2Asubunits. T…

Functional dissociation of language and theory of mind in the developing superior temporal lobe.

Language and theory of mind (ToM; the ability to infer others' mental states) are both crucial for human communication, and yet their developmental origins are unclear. Are their neural substrates distinct within the superior temporal lobe (STL) but with opposing lateralization, as in adults? Or do they emerge from common neural substrates during development, perhaps in homologous regions originally involved in more basic social processing? Here we investigate the development of this functional dissociation, and the dissociation of their underlying connectivity fingerprints in a large cohort o…

Kisspeptin System: A Modulator of Neuroinflammation and Behaviour in Temporal Lobe Epileptic Rats.

Temporal lobe epilepsy (TLE) is the most prevalent form of drug-resistant epilepsy and is driven by persistent neuroinflammatory cascades wherein interactions between inflammatory mediators and hormones play a crucial role. Kisspeptin (Kiss1), a neuropeptide known to modulate synaptic transmission in the hippocampus, has an unclear role in pathophysiology of epilepsy. To investigate this, a lithium-pilocarpine TLE rat model was established. Expression profiles of Kiss1, Kiss1r, and key neuroinflammatory molecules were assessed in the hippocampus, anterior temporal lobe (ATL), and neocortex at …

A vascular component to the selective vulnerability of the medial temporal lobe to tau pathology.

The medial temporal lobe is widely recognized as an early site of pathological tau deposition, a central feature in the development of cognitive symptoms. As of now, the biological mechanisms underlying this selective vulnerability of the medial temporal lobe are not yet understood. Here, we tested whether alterations in medial temporal lobe arterial structure are associated with early tau accumulation and cognitive decline. We first applied a novel imaging methodology (super-selective arterial spin labelling) to map vascular supply to the medial temporal lobe. Next, we cross-sectionally inves…

Brain local structural connectomes and the subtypes of the medial temporal lobe parcellations.

OBJECTIVE: To investigate the quantitative characteristics and major subtypes of local structural connectomes for medial temporal lobe (MTL) parcellations. METHODS: The Q-Space Diffeomorphic Reconstruction (QSDR) method was used to track white matter fibers for the ROIs within MTL based on the integrating high-resolution T1 structural MR imaging and diffusion MR imaging of 100 adult Chinese individuals. Graph theoretical analysis was employed to construct the local structural connectome models for ROIs within MTL and acquire the network parameters. These connectivity matrices of these connect…

Comorbidities in the mouse model of temporal lobe epilepsy induced by intrahippocampal kainate.

OBJECTIVE: Mesial temporal lobe epilepsy (MTLE) is generally associated with cognitive and psychiatric comorbidities. Here we examined whether similar comorbidities are observed in mice injected with kainate in the dorsal hippocampus, a model known to recapitulate many features of human MTLE, and if these comorbidities are sex dependent. METHODS: Male and female C57BL/6 mice were unilaterally injected with kainate in the dorsal hippocampus (KA-MTLE), and hippocampal paroxysmal discharges were recorded over 2 months. Behavioral tests assessed food intake (Novelty Palatable Food Intake test), w…

Equivalence of the FCSRT and RAVLT to detect medial Temporal lobe atrophy and tauopathy.

In AD research, word-learning tests are often used interchangeably despite using distinct learning protocols. This study verified the equivalence of the Rey Auditory Learning Test (RAVLT) and Free and Cued Selective Reminding Test (FCSRT) when investigating medial temporal lobe (MTL) changes and AD-related tau pathology. We obtained the FCSRT and RAVLT immediate and delayed free recalls from 286 participants aged 51+. We segmented MTL regions to obtain the volume and tau-PET signal using the [18F]MK-6240 tracer. Tau-PET Braak stages and plasma p-tau181, p-tau217 and p-tau231 quantifications we…

Robotic assessment of sensorimotor and cognitive deficits in patients with temporal lobe epilepsy.

OBJECTIVE: Individuals with temporal lobe epilepsy (TLE) frequently demonstrate impairments in executive function, working memory, and/or declarative memory. It is recommended that screening for cognitive impairment is undertaken in all people newly diagnosed with epilepsy. However, standard neuropsychological assessments are a limited resource and thus not available to all. Our study investigated the use of robotic technology (the Kinarm robot) for cognitive screening. METHODS: 27 participants with TLE (17 left) underwent both a brief neuropsychological screening and a robotic (Kinarm) asses…

The effect of ferroptosis-related mitochondrial dysfunction in the development of temporal lobe epilepsy.

Temporal lobe epilepsy (TLE) is the most common form of epileptic syndrome. It has been established that due to its complex pathogenesis, a considerable proportion of TLE patients often progress to drug-resistant epilepsy. Ferroptosis has emerged as an important neuronal death mechanism in TLE, which is primarily influenced by lipid accumulation and oxidative stress. In previous studies of ferroptosis, more attention has been focused on the impact of changes in the levels of proteins related to the redox equilibrium and signaling pathways on epileptic seizures. However, it is worth noting that…
