Reconstruction of healed elongated anterior tibial and extensor hallucis longus tendons in a young active farmer.

A case of open traumatic lacerations of the anterior tibial tendon (ATT) and extensor hallucis longus (EHL) tendon that both healed in continuity but significantly elongated position following primary repair, resulting in foot drop and sagging of the great toe affecting gait and function, is presented. An adequate immobilization and protection in the early post-operative period is very important and a secure technique for reconstructing a healed elongated anterior tibial tendon is described.

Percutaneous, Minimally Invasive Repair of Traumatic and Simultaneous Rupture of Both Achilles Tendons: A Case Report.

Achilles injury is a common musculoskeletal disorder. Bilateral rupture of the Achilles tendon, however, is much less common and usually occurs spontaneously. Complete, traumatic, and bilateral ruptures are rare and typically require long periods of immobilization before the patient can return to full weightbearing. A 52-year-old male was hospitalized for bilateral traumatic rupture to both Achilles tendons. No risk factors for tendon rupture were found. Blood samples revealed no peripheral blood pathologic features. Both tendons were repaired with percutaneous, minimally invasive surgery usin…

Multiple ruptures of the extensor tendons after volar fixation for distal radius fracture: a case report.

A 62-year-old woman was admitted to our hospital after a bicycle accident with a displaced left (non-dominant) distal radius fracture. After … icit of the extension of the third and fourth left finger was noted secondary to injury of extensor tendons. Ultrasound and CT scan showed protrusion of the angular stability screws in LCP plate that caused a progressive wear resulting in rupture of the extensor tendons. Another tendon transposition surgery was performed with dorsal approach while the pla …

Magic angle magnetic resonance imaging of diode laser induced and naturally occurring lesions in equine tendons.

Magic angle magnetic resonance (MR) imaging consists of imaging tendons at 55° to the magnetic field. In people, magic angle MR imaging is valuable for detection of chronic tendon lesions and allows calculation of tendon T1 values. Increased T1 values occur in people with chronic tendinopathy. The T1 values of normal equine tendons have been reported but there are no available data for abnormal equine tendons. Twelve limbs were studied. Two limbs had diode laser tendon lesions induced postmortem, four limbs had diode laser tendon lesions induced in vivo and six limbs had naturally occurring te…

Congenital ptosis of the upper eyelid corrected by a modified frontalis suspension technique using autogenous tendons.

INTRODUCTION: The treatment of choice for medium to severe blepharoptosis is frontalis suspension. The aim of this study is to describe a new modified surgical technique of frontalis suspension for congenital ptosis using an autogenous tendon. PATIENTS AND METHODS: Frontalis sling with double parallelogram fixation using autogenous tendon was performed on 14 upper lids in 9 children. The clinical and cosmetic improvement was determined by the lid contour, lid height symmetry, and lid creases. Surgical results were compared 12 and 18 months after surgery. RESULTS: All the patients presented n

Carpal tunnel syndrome caused by gouty tophus of the flexor tendons of the fingers: sonographic features.

We report the case of a 23-year-old male presenting with carpal tunnel syndrome and a swelling over the flexor surface of the wrist. MRI findings were initially suggestive of a median nerve schwannoma but sonography (US) showed a heterogenous mass infiltrating the flexor tendons of the fingers and displacing the median nerve in the carpal tunnel. US findings were confirmed by surgical exploration, which revealed a gouty tophus of the flexor tendons of the fingers at the wrist with secondary median nerve displacement and compression.

[Soft-tissue reconstruction of the dorsum of the hand and finger to cover the extender tendons].

Small defects of extensor tendons can mostly be covered by local flaps. For larger defects the groin flaps, pedicled flaps from the forearm and free flaps are indicated. Between January 2001 and December 2005, we performed the following pedicled and free flaps to reconstruct the soft tissue defects of the hand or distal forearm: 23 pedicled flaps: 15 posterior interosseus artery or perforator flaps, 8 radial forearm flaps; 26 free flaps: 4 radial forearm flaps, 12 lateral upper arm flaps, 10 latissimus, serratus fascia or scapula flaps. With this study we want to illustrate the different proce…

Screw impingement on the extensor tendons in distal radius fractures treated by volar plating: sonographic appearance.

OBJECTIVE: The objective of our study was to analyze the sonography examinations of nine consecutive patients with a history of distal radius fracture treated by open reduction and internal fixation of the volar plate who were referred by hand surgeons for sonography of the dorsal aspect of the wrist. CONCLUSION: We postulate that impingement of the extensor tendons in patients with distal radius fracture treated by volar plating starts with local hyperemia and is followed by tenosynovitis and, finally, by partial and complete tendon tears. Sonography is an effective, dynamic, and noninvasive…

In vivo measurement of tissue metabolism in tendons of the rotator cuff: implications for surgical management.

We have undertaken an in vivo assessment of the tissue metabolism and cellular activity in torn tendons of the rotator cuff. Cellular oxygen consumption was measured in 13 patients undergoing mini-open repair of small, medium, large and massive full-thickness tears. Measurements were also taken from three control patients who were undergoing open stabilisation of the shoulder with grossly normal tendons. The level of oxygen and nitrous oxide was measured amperometrically using silver needle microelectrodes at the apex of the tear and 1.5 cm from its edge. With nitrous oxide indicating the degr…

Gliding characteristics between flexor tendons and surrounding tissues in the carpal tunnel: a biomechanical cadaver study.

The purpose of this study was to investigate the gliding characteristics of flexor tendons within the carpal tunnel with varied wrist positions and tendon motion styles, which may help us to understand the relationship between carpal tunnel syndrome (CTS) and repetitive hand motion. Eight fresh human cadaveric wrists and hands were used. The peak (PGR) and mean (MGR) gliding resistance of the middle finger flexor digitorum superficialis tendon were measured with the wrist in 0, 30, and 60 degrees of flexion and extension. While moving all three fingers together, the PGR at 60 degrees flexion w…

Management and functional outcomes of combined injuries of flexor tendons, nerves, and vessels at the wrist.

A retrospective review of 42 patients with spaghetti wrist lacerations operated on by the author between June 1997 and May 2005 was completed. A total of 31 males and 11 females, average age of 17.1 years (range, 2-40 years), sustained spaghetti wrist injuries. The most frequent mechanisms of injury were accidental glass lacerations (55%), knife wounds (24%), and electrical saw injuries (11%). An average of 9.16 structures was injured, including 6.95 tendons, 1.4 nerves, and 0.8 arteries. The most frequently injured structures were median nerve (83%), flexor digitorum superficialis 2-4 tendons…

The origin of cells that repopulate patellar tendons used for reconstructing anterior cruciate ligaments in man.

Cryopreserved patellar tendon allografts are often recommended for reconstruction of anterior cruciate ligaments (ACLs) because living donor fibroblasts are thought to promote repair. Animal studies, however, indicate that ligaments regenerate from recipient rather than donor cells. If applicable to man, these observations suggest that allograft cell viability is unimportant. We therefore used short tandem repeat analysis with polymerase chain reaction (PCR) amplification to determine the source of cells in nine human ACLs reconstructed with cryopreserved patellar tendon allografts. PCR amplif

The modified oblique Keller procedure: a technique for dorsal approach interposition arthroplasty sparing the flexor tendons.

A first metatarsophalangeal joint resection arthroplasty that combines a modest metatarsal cheilectomy with an oblique resection of the phalanx base (preserving the flexor hallucis brevis attachment) combined with interposition arthroplasty of the dorsal joint capsule sewn to the plantar soft tissues is presented. Numerous surgical procedures have been described for the treatment of hallux rigidus, including dorsal cheilectomy, resection arthroplasty, joint replacement, and arthrodesis. The Keller procedure has been abandoned by many because of shortening of the great toe and loss of push-off

[Scintigraphy with thallium chloride in a case of clear cell sarcoma of the tendons and aponeurosis].

Clear cell sarcoma of the soft parts (CCSSP) is a rare and highly malignant tumor. This is a case report of a 31 years old woman who presented with a tumor in the internal face of left thigh. The tumor biopsy was suggestive of a possible malignant tumor of the peripheral nerve sheath (malignant schwannoma). Biochemical analyses, computed tomography and magnetic resonance were performed and reported a sarcoma of soft parts (CCSSP) without abnormal inguinal lymph nodes. With this diagnostic suspicion, the patient was sent to the Nuclear Medicine Service of our center where a 201Thallium scintigr

Spontaneous rupture of extensor tendons at the wrist in a patient with mixed connective tissue disease.

Abstract A 54-year-old woman who had been treated for mixed connective tissue disease for 4 years developed spontaneous rupture of extensor tendons in the wrist. The patient was surgically treated by tendon reconstruction. Histopathological examination of the synovial membrane showed lymphocytic inflammatory cellular infiltration around small blood vessels. The tendon ruptures in this case were most likely caused by synovial membrane proliferation in the wrist and mechanical stress generated by the subluxated distal ulna.
