Autoimmune and inflammatory thyroid diseases following vaccination with SARS-CoV-2 vaccines: from etiopathogenesis to clinical management.

Since the Covid-19 pandemic emerged in 2019, several adenoviral-vectored, mRNA-based and inactivated whole-virus vaccines have been developed. A massive vaccination campaign has been undertaken around the world, and an increasing number of SARS-CoV-2 vaccine-induced thyroid diseases have been described in the literature. Subacute thyroiditis has been reported in 52 patients, mean age 45.5 ± 1.8 years, mainly in women (n = 39). Graves' disease is more frequent in women (n = 22) than in men (n = 10), mean age 46.2 ± 2.6 years, reported as new onset, recurrent or exacerbation of well-controlled h

Measurements of an index of echogenicity in some thyroid diseases.

In the preceding article, the measurement of an index of echogenicity has been described. In this article, we present the mean value and standard deviation of this index in normal thyroid and pathological cases. It appears that the index is very significantly lowered in Hashimoto's disease compared to its value in normal thyroids and simple goiters. The advantage of this index in other cases such as Graves' disease, subacute thyroiditis, and thyroid nodules is discussed.

Hyperthyroidism: multiple possibilities in the female patient.

Hyperthyroidism is a clinical syndrome characterized by an excess of thyroid hormone, and its clinical consequences. A suppressed serum TSH concentration is the earliest biochemical manifestation of hyperthyroidism. Subclinical hyperthyroidism, characterized by suppressed serum TSH concentration alone, has important clinical consequences. These include bone loss in postmenopausal women and atrial fibrillation. A twenty-four hour radioiodine uptake and radionuclide scan are indispensable in the differential diagnosis of hyperthyroidism. Graves' Disease, an autoimmune disorder, demonstrates a st

[Iodine deficiency disorder persistance after introduction of iodized salt in a previously endemic goiter area].

OBJECTIVE: Correction of iodine deficiency diminishes the incidence of toxic nodular goitre. The aim of this study was to assess the etiology of thyrotoxicosis in two areas with different goitre prevalence in Galicia, fifteen years after the institutional campaign of salt iodination. Results of the present survey are compared with those from a study performed ten years ago. METHODS: Two hundred and two thyrotoxic patients attended in La Coruña (coastal zone) and Lugo (inland zone) from January 2000 to May 2002 were included. Clinical and exploratory data were recorded. Thyroid hormones, thyro

The species specificity of TSH receptor binding antibodies as measured by radioreceptor study.

The species specificity of TSH binding inhibitory antibodies was compared for patients with untreated Graves' hyperthyroidism, past Graves' hyperthyroidism, active ophthalmopathy with past hyperthyroidism, and subacute thyroiditis, by measuring inhibition of TSH binding to plasma membranes prepared from human, guinea-pig, calf, pig, and dog thyroid glands in a radioreceptor assay. Results were expressed as TSH binding inhibition indices (TBII). Broad species reactivity was demonstrated. This was greatest with pig and least with guinea-pig thyroid membranes. Immunoglobulin (Ig) from patients in

A Literature Review of Painful Hashimoto Thyroiditis: 70 Published Cases in the Past 70 Years.

Painful Hashimoto thyroiditis (pHT) is a rare diagnosis, and optimal treatment remains unclear. To better characterize pHT, PubMed, Embase, Scopus, and Web of Science indexes were searched for case reports or case series reporting pHT, published between 1951 and February 2019. Seventy cases reported in 24 publications were identified. Female predominance (91.4%) and a median age of 39.00 years (interquartile range, 32.50-49.75 years) were observed. Among reported cases, 50.8% had known thyroid disease (including Hashimoto thyroiditis, Graves disease, and seronegative goiters), 83.3% had positi

Calcitonin activity assessed by calcium tolerance test in patients with thyroid disorders.

The calcium tolerance test was performed in 34 healthy subjects and in 103 patients with simple goitre, thyrotoxicosis, hypothyroidism, subacute thyroiditis, medullary carcinoma and in patients after subtotal or total thyroidectomy and after radioiodine therapy. Calcium lowering activity which reflects mainly the activity of clacitonin, was significantly impaired in thyrotoxic and totally thyroidectomized patients. An abnormal calcium tolerance test in a patient with medullary carcinoma corresponded with large amounts of released calcitonin.

Kinetics of the human thyroid trap: experience in normal subjects and in thyroid disease.

Kinetics of the thyroid pertechnetate trap were assessed in 85 studies of 39 normal subjects, in five untreated patients with Graves' disease (two of them both before and after treatment), in two hypothyroid patients, and in one patient each with Hashimoto's thyroiditis of recent onset, subacute thyroiditis, and massive anaplastic carcinoma. In normal subjects, the effects of sex, time of day, and the order of experimental sessions were studied. For the analysis, a three-compartment model was assumed for all studies. Data on thyroidal and neck-background pertechnetate were collected with a mul

[Review and first line thyroid investigations and difficult clinical traps].

This general review will be focused on the evaluation of thyroid function and thyroid disorders, including the following topics: When should thyroid abnormalities be suspected? Which first line laboratory tests should be carried out? What should be done when hyper or hypothyroidism is suspected? Which is the most appropriate medical management for a goitre, for subacute thyroiditis, for pregnancy, related thyroid disorders? When thyroid hormone replacement therapy is initiated in a patient, which are the best diagnostic tools to employ for following-up and adjusting the dosage in such therapy?

[Thyroid ultrasound and its role in the investigation of thyroid disease].

In Europe thyroid ultrasound has been used at outpatient endocrine clinics since many years, and in southern Sweden only during the last years. Ultrasound has a role in the investigation of Graves' disease, subacute thyroiditis, gestational thyrotoxicosis, postpartum thyroiditis, amiodarone thyrotoxicosis and goiter with or without adenoma, but adenomas are usually investigated by endocrine surgeons in Sweden. If widely used the risk for detection of incidentaloma increases. Indications have to be strictly used to avoid further investigations. If an adenoma is localized, the risk for malignanc

The value of shear wave elastography in diffuse thyroid disease.

PURPOSE: The aim of this study is to discuss the value of ultrasound-based shear wave™ elastography (SWE) in diffuse thyroid disease (DTD). METHOD: Thyroid stiffness in 154 patients with DTD and 30 normal subjects was measured by SWE. The serum indicators of all subjects were detected. RESULTS: The area under the receiver operating characteristic (AUROC) curve for DTD by SWE was 0.852. The AUROCs of SWE for differentiating chronic autoimmune thyroiditis (CAT) from Graves' disease (GD) and subacute thyroiditis (SAT) were 0.549 and 0.989, respectively. The AUROCs for distinguishing GD from SAT

Thyroid Swelling and Thyroiditis in the Setting of Recent hCG Injections and Fine Needle Aspiration.

A 60-year-old woman presented with a neck mass and underwent fine needle aspiration of a left thyroid nodule. During this time, she had been injected with hCG for weight loss. Soon after, she developed rapid diffuse thyroid growth with pain. She was ultimately diagnosed with thyrotoxicosis due to postaspiration subacute thyroiditis and subsequently became hypothyroid. This condition is rare in the nonpregnant state in noncystic nodules with a smaller needle gauge approach. The incidence of thyroid nodule discovery and evaluation is increasing. As more procedures are undertaken, understanding o

[Thyroid function and amiodarone. Consequences on dysthyroidism testing].

Amiodarone induces a decrease in serum T3 whereas T4 and rT3 increase. An increase of the thyroid iodine content (TIC) is observed in all patients at the exception of those who develop hypothyroidism under treatment. Actually, no method are available to predict an induced thyroid toxicosis (ITT) and there is no reason to perform systematically thyroid function tests except if past of the patient or clinical or morphological thyroid examinations suggest thyroid abnormality. In case of suspicion of ITT it is necessary to perform T4, T3 determinations and a TRH test. TIC measurement can be useful

["Subacute thyreoiditis" without response on corticosteroid therapy].

A 49-year old employee of a drug store with neck pain, painful thyroid gland, and elevated erythrocyte sedimentation rate (38 mm/h) was diagnosed as subacute thyreoiditis (de Quervain). However, application of oral corticosteroids (prednisone 50 mg/d) during three days did not reduce pain as expected. Therefore, the patient was admitted for further evaluation. Clinical examination showed a female in a pain-relieving posture (forward neck flexion). Further examinations including ultrasound of the thyroid, computertomography of the neck, and ENT examination did not reveal etiology of the pain. F

Lack of increased serum interleukin-6 and soluble IL-6 receptor concentrations in patients with thyroid diseases following recombinant human interferon alpha therapy.

BACKGROUND: Serum interleukin-6 (IL-6) concentrations are frequently elevated in inflammatory thyroid diseases, such as subacute thyroiditis and amiodarone induced thyroiditis. We and others have recently observed that recombinant interferon-alpha (rIFN-alpha) therapy for chronic, active viral hepatitis and malignant disorders may induce thyroid dysfunction, including thyrotoxicosis secondary to thyroiditis. Serum IL-6 and its soluble receptor (sIL-6R) have been measured for the first time in patients with chronic active hepatitis receiving rIFN-alpha therapy. METHODS: Studies were carried ou

COVISHIELD vaccine-induced thyroiditis: a case report.

BACKGROUND: The rapid development of coronavirus disease 2019 vaccines during the pandemic has left their long-term effects largely unknown. Instances of autoimmune and subacute thyroiditis showing features of autoimmune/inflammatory syndrome induced by adjuvants have been reported post-vaccination. This case report aims to highlight the autoimmune/inflammatory syndrome induced by adjuvants syndrome after coronavirus disease 2019 vaccination, drawing attention to a possible connection with thyroid dysfunction and urging for further thorough research. CASE PRESENTATION: We present a case of th

Precipitations of thyrotropin receptor-related peptides by immunoglobulin G of patients with Hashimoto's thyroiditis.

In order to study whether specific binding sites for Hashimoto's IgG exist in TSH receptor, we have examined the TSH receptor binding activity of IgG of patients with Hashimoto's thyroiditis using synthetic peptides corresponding to the amino acid sequences in the extracellular domain of human TSH receptor. Eight different peptides were synthesized, radiolabeled and examined by immunoprecipitation assay. Among the peptides studied, peptide (352-378) was significantly precipitated by IgG of patients with Hashimoto's thyroiditis compared with control subjects. Ten out of twenty patients with Has
