A 4-year-old Japanese girl developed a sore throat and high fever. Her tonsils were enlarged, red and covered with a thick white membrane. There was marked leukocytosis (26,600 leukocytes per mm) and elevated C-reactive protein levels (23.3 mg/dL). Rothia dentocariosa was recovered from the throat swab; many Gram-positive cocci were observed in the smear from the pseudomembrane covering the tonsil.

The present study is undertaken on 50 tonsillectomy cases to determine the correlation between surface culture swabs and tonsillar core. Tonsillar disease may stem from the bacteria within the core of the tonsil, rather than the bacteria identified on its surface. Also no consistent pattern of combinations of different pathogenic bacteria was noted. The study proves that surface culture does not reliably predict core pathogens.

BACKGROUND: Acute suppurative tonsillitis is one of the most common pediatric infectious diseases, and has Group A beta-hemolytic streptococci (GABHS) as its most frequent bacterial cause. Acute tonsillectomy has been advocated for severe infectious mononucleosis (IM) pharyngotonsillitis with upper airway obstruction (UAO) and not responding to corticosteroids. This paper reviews and rationalizes our management. A five-year chart review was carried out with a follow-up telephone survey. There were 36 admissions. Kept under continuous medical surveillance were 77 children with streptococcal tonsillitis. In all patients, a pathogenic streptococcus (GABHS) was invariably found in culture. Among other common features of the above disease were decline in the tonsil colony resistance state and lowering of those parameters characterizing immunity. A highly efficient means of treating tonsillitis of streptococcal etiology has been elaborated, relying on the use of macrolides and L. acidophilus according to our methods, which can, we believe, come to be widely used in health care practice.

A total of 100 patients with chronic tonsillitis were selected for this study. Specimens collected immediately before surgery and the operated specimen of tonsils were subjected to appropriate microbiological techniques. Over the past 5 years, bilateral tonsillectomy has been performed in 104 patients (48 male and 56 female) with IgA nephropathy at the Depart … in patients with smaller tonsils. There were no significant differences relative to past history of tonsillitis, age, pus plugs in the lacunae, temporary deterioration of urinary findings after tonsillectomy, and results of provocation tests.

A novel technique of surgical treatment of chronic tonsillitis is presented. It implies the use of Ne-YAG laser in contract regimen. Good results were obtained in experiments on the exposure of distal tonsils to intralacunar laser radiation.

Chronic tonsillitis (CT) was found in 63.1% of 190 obese children aged 3-15 years, 15% of them had no quinsy in the past. Onset of obesity was attributed to frequent quinsy in 18.3% of the examinees. Samples from the fur covering the palatine tonsils of 10 patients with infectious mononucleosis (IM) were subjected to fluorescence microscopy after staining with acridine orange. Bicycle exercise test, load myocardial 99mTc pyrophosphate (99mTc-PYP) scintigraphy and echocardiography were performed in 42 women with chronic tonsillitis between the ages of 18 and 35 years. Of the 21 patients with chronic decompensated tonsillitis combined with ventricular extrasystole and horizontal ST segment depression during the exercise test 18 exhibited diffuse myocardial 99mTc-PYP uptake. Patients with positive scintigraphic findings had reduced index of left-atrium emptying compared with controls and patients with a negative 99mTc-PYP test finding.

Transformation tests of tonsillar and blood lymphocytes with many streptococcal extracellular antigens and streptococcal membrane antigen in patients suffering from chronic tonsillitis were performed. The in-vitro cellular immunoresponse depends on the antigen, the age of the patient, and the origin of the lymphocytes: blood or tonsil. There is good evidence that there is a selective unresponsiveness of the blood lymphocytes in adult patients, stimulated with streptococcal membrane antigen.
