Internal derangement and the accessory ligaments around the temporomandibular joint.

A model that describes the mechanics of jaw opening and the associated movements of the disc in a healthy human temporomandibular joint is used to explain the abnormal movements associated with internal derangement of the joint. A change in the constraints on jaw opening imposed by the accessory ligaments around the jaw can account for, among other things, clicks on opening and closing, disc displacement and replacement, and the conditions of open and closed lock.

Temporomandibular joint dysfunction and facial pain caused by neoplasms. Report of three cases.

The most frequent causes of painful limitation of temporomandibular joint function are myofascial pain dysfunction or anterior disk displacement. A potential problem in dealing with those patients is the risk of misdiagnosing the rare patient who has a neoplastic disease, primary or metastatic to the epipharyngeal region, parotid gland, jaws, or temporomandibular joint. Three cases are presented in which progressively worsening limitation of mandibular movement and increasing pain led to the diagnosis of a neoplasm in the temporomandibular joint region. The article illustrates the necessity of

Oral Submucous Fibrosis and Scleroderma: A Review of the Etiopathogenesis, Clinicopathological Correlation, and Management Aspects.

Oral submucous fibrosis (OSMF) is a chronic, progressive, insidious premalignant disease with multifactorial etiology affecting any part of the oral cavity and sometimes the pharynx by triggering a rapid onset of trismus and dysphagia due to stiffness at the lips, cheek, pharynx, and upper oesophageal region. Submucous fibrosis resembles many auto-immune, dermatological, mucocutaneous, and fibrotic lesions that include scleroderma, amyloidosis, iron deficiency anemia, and systemic or generalized fibromatosis clinically and histologically. Several authors established an association between oral…

Displacement of a Broken Dental Injection Needle Into the Perivertebral Space.

Dental injection needle breakage is an uncommon problem in dental practice. Displacement of the broken fragment into anatomical spaces is, on the other hand, a serious complication that occurs most commonly during inferior alveolar nerve blocks as a result of material wear, incorrect application of the anesthesia technique, or sudden movement of the patient during injection. Further complications such as infection, trismus, and nerve paralysis may exacerbate the condition and, if not treated adequately, life-threatening conditions may develop over time as the fragment dislodges deeper in soft …

Clinical predictors of peritonsillar abscess in adults.

OBJECTIVE: The primary objective was to identify clinical predictors that differentiate a peritonsillar abscess from peritonsillar celluliti … acco smoking behaviour. Clinical signs significantly associated with peritonsillar abscess included trismus (p < .001), uvular deviation (p < .001), and inferior displacement of the superior pole of the tonsil (p < .001) on the affected side. Pain duration was not a significant discriminative factor (p = .069). CONCLUSIONS: The diagnosis of a peritonsill …

Subjective effects of 3,4-methylenedioxymethamphetamine in recreational users.

3,4-Methylenedioxymethamphetamine (MDMA; Ecstasy) is a serotonergic neurotoxin in laboratory animals that has been used for recreational purposes by humans. The subjective effects of this drug were determined in recreational users at a university campus. Of individuals who had admitted to using MDMA recreationally, 100 of 143 agreed to complete a detailed questionnaire concerning the subjective effects of this Schedule I compound. The most common effect of MDMA was a heightened sense of "closeness" with other people (90% of subjects). Tachycardia, dry mouth, bruxism and/or trismus were reporte…

Isolated 'sign of the horns': a simple, pathognomonic, prenatal sonographic marker of Crisponi syndrome.

The case of a patient presenting with Crisponi syndrome recently hospitalized at our institution is described. During pregnancy a diagnosis of this syndrome was hypothesized following sonographic observation of the fetus with the hands showing 'the sign of the horns'. Such a finding, if isolated, as in our case, may represent a simple, pathognomonic sonographic marker of Crisponi syndrome.

Orthodontic treatment of a patient with recessive dystrophic epidermolysis bullosa: a case report.

Epidermolysis bullosa (EB) represents a heterogeneous group of hereditary, chronic, non-inflammatory skin pathologies. Recessive dystrophic epidermolysis bullosa (RDEB) is characterized by sub-lamina dura separation, due to blistering below the lamina densa of the basement membrane zone, and is associated with the absence of Type VII collagen fibers. The mouth, pharyngeal, and esophageal mucosa are compromised by scarring and tissue contraction. The purpose of this article was to present a case report on an orthodontic patient with RDEB.

[Lockjaw caused by induction of anesthesia in a volunteer bone marrow donor].

A 43-year-old healthy female volunteer donor was scheduled for bone marrow harvesting. In the operating room her mouth opening was ascertained to be 5 cm. Anesthesia was induced with thiamylal 250 mg, fentanyl 0.1 mg and vecuronium 6 mg i.v. Her mouth opening was found reduced to 2 cm that was not improved by additional vecuronium 2 mg. A consulted oral surgeon diagnosed temporomandibular disorder. After she was awakened once with resumed mouth opening, anesthesia was reinduced with a bite block placed and with her jaw held open by the oral surgeon, which brought the same results. Laryngoscopy

The recognition of craniomandibular disorders--a comparison between clinical and radiographic findings in eighty-nine subjects.

A comparison has been made between clinical and radiographical findings in a study of eighty-nine patients suffering from craniomandibular disorders. In general it was found that condylar changes were more common in these patients than in earlier studies concerning the severe problems of craniomandibular disorders. A statistically significant correlation between clinical and radiographical findings was found for crepitation with osteophyte formation of the condyle (P less than 0.01). After clinical separation of the patients into two groups based upon the origin of pain, myogenous versus arthr

Severe Generalized Tetanus in a Chimpanzee (Pan troglodytes) Under Human Care: A Case Report from the Republic of Congo.

Tetanus is a life-threatening neurological disease affecting vertebrate species, including primates. Here, we present a case of severe generalized tetanus in a juvenile male chimpanzee (Pan troglodytes) that was rescued from the illegal wildlife trade and admitted to a rehabilitation center in the Republic of Congo. Upon arrival, the chimpanzee presented with deep, contaminated constrictive wounds, trismus, generalized rigidity, and stimulus-induced tonic spasms accompanied by transient apnea, while remaining conscious. A presumptive clinical diagnosis was made, after which integrated care beg…
