[Diagnosis and treatment of latent tubercular infections].

Despite dramatic advances in diagnosis and treatment, tuberculosis (TB) remains one of the leading causes of morbidity and mortality worldwide. World Health Organization (WHO) reports that one-third of the global population is infected by M. tuberculosis. Direct identification of individuals who are latently infected with live M. tuberculosis without active disease is currently not possible (Latent TB Infection, LTBI). Available immunodiagnostic tests, i.e. Tuberculin Skin Testing and Interferon-gamma Release Assay, ascertain a state of M. tuberculosis specific immune response; they have sever

Anergy in tuberculosis.

Anergy in tuberculosis is of considerable clinical and immunological interest. Although negative skin reactions may be secondary to improper tuberculin testing or to certain diseases, drugs, vaccinations and constitutional factors known to affect cell-mediated immunity, often no underlying explanation is apparent. Unexplained negative reactions occur with a highly variable frequency in a wide variety of tuberculous infections. Anergy in tuberculosis is usually generalized, is often accompanied by other immunologic abnormalities, and frequently disappears as the disease is treated. Most anergic…

Tuberculosis vaccination versus isoniazid preventive therapy: a decision analysis to determine the preferred strategy of tuberculosis prevention in HIV-infected adults in the developing world.

SETTING: The developing world. OBJECTIVE: To compare the strategy of TB vaccination with that of tuberculin skin-testing in conjunction with isoniazid (INH) in preventing tuberculosis in HIV-infected persons. For any clinical scenarios in which immunization would be more effective than INH preventive therapy, to determine the minimum necessary vaccine safety and effectiveness required. DESIGN: Decision analysis. A hypothetical cohort of 10000 HIV-infected persons, 65% of whom were tuberculin positive, living in the developing world, was studied. Probability estimates were based on BCG vaccin

Tuberculin responsiveness as a surrogate prognostic marker in hospitalized HIV-infected patients with tuberculosis in a resource-limited setting.

INTRODUCTION: In resource-limited settings, access to CD4 lymphocyte counts for prognostication in HIV-associated tuberculosis (HIV-TB) remains limited. Tuberculin skin test (TST) reactivity reflects cell-mediated immunity and may serve as a simple surrogate prognostic marker. This study evaluated the association between tuberculin responsiveness and clinical outcomes in hospitalized HIV-infected patients with pulmonary tuberculosis. MATERIALS AND METHODS: A hospital-based observational cohort study was conducted among 107 HIV-positive adults admitted with pulmonary tuberculosis. Tuberculin r

Screening and treatment of latent tuberculosis among healthcare workers at low, moderate, and high risk for tuberculosis exposure: a cost-effectiveness analysis.

OBJECTIVE: To evaluate cost-effective screening and treatment strategies for healthcare workers (HCWs) at risk for tuberculosis exposure. DESIGN: A Markov model was developed to track three hypothetical cohorts of HCWs at low, moderate, and high risk for tuberculosis exposure. For those found to be tuberculin reactors at entry, the choice was for isoniazid treatment or no treatment. For those without tuberculin reactivity, the choice of screening intervals was 6 months, 1 year, 2 years, or 5 years. Outcomes measured were tuberculosis cases, death, life expectancy, and cost. Assumptions were d

IFN-γ-independent immune markers of Mycobacterium tuberculosis exposure.

Exposure to Mycobacterium tuberculosis (Mtb) results in heterogeneous clinical outcomes including primary progressive tuberculosis and latent Mtb infection (LTBI). Mtb infection is identified using the tuberculin skin test and interferon-γ (IFN-γ) release assay IGRA, and a positive result may prompt chemoprophylaxis to prevent progression to tuberculosis. In the present study, we report on a cohort of Ugandan individuals who were household contacts of patients with TB. These individuals were highly exposed to Mtb but tested negative disease by IFN-γ release assay and tuberculin skin test, 'res

Peripheral tuberculous lymphadenitis in pediatrics: 16 years of experience in a tertiary care pediatric hospital of Buenos Aires, Argentina.

Extrapulmonary tuberculosis accounts for 15-20 % of all clinical presentations of tuberculosis. Peripheral tuberculous lymphadenitis is the second most common presentation of extrapulmonary tuberculosis in children, after pleural tuberculosis, in Argentina. We analyzed 92 patients with peripheral tuberculous lymphadenitis seen at the Department of Tisiology of Hospital de Niños "Dr. Ricardo Gutiérrez" between August 2000 and September 2015. The patients' mean age was 8.7 ± 5 years. Nodal sites corresponded to single peripheral (31.5 %), multiple peripheral (20.6 %), and peripheral associated w

A novel assay for screening patients for latent tuberculosis infection prior to anti-TNF therapy.

Screening for latent tuberculosis (TB) infection (LTBI) prior to anti-tumor necrosis factor (TNF) therapy is complicated by the decreased sensitivity of the purified protein derivative tuberculin skin test in immunosuppressed patients, as well as the poor specificity of the tuberculin skin test in bacillus Calmette-Guérin (BCG)-vaccinated patients. Interferon-gamma-release assays that utilize specific TB antigens (e.g. ESAT-6/CFP-10) have greater specificity for LTBI, but their relative sensitivity is unknown in the setting of immune suppression. Dinser et al. evaluated a novel flow-cytometric

[Large-scale outbreak investigation for tuberculosis in Zeist].

Recently a large-scale outbreak investigation was held among customers of a supermarket in the Dutch town of Zeist after an employee was diagnosed with smear-positive pulmonary tuberculosis. About 14,000 people had a tuberculin skin test and an additional 6,000 had a chest X-ray. About 400 positive skin tests and 8 cases of active tuberculosis have been reported so far. Investigation of contacts of infectious tuberculosis cases is an important tool in tuberculosis control in low-prevalence countries. It includes chest X-ray examination for active pulmonary tuberculosis after or instead of tube

[Tuberculin survey of university students and postgraduates in 1998].

As no recent report has been made concerning the tuberculin survey of healthy young adults in Japan, we checked all the Tohoku University undergraduate and postgraduate students by tuberculin test for the assessment of TB-situation in 1998. The sample included 5,517 students and postgraduates (3,888 males and 1,629 females; 31% of all subjects to be tested) who were evaluated on the basis of the redness of the skin test. The average age of the subjects was 22.3 +/- 3.1 years old (ranging from 18 to 51). As a result of this survey, 5,032 (91.2%) were positive and 485 (8.8%) were negative, and t…

Cost-effectiveness of screening compared to case-finding approaches to tuberculosis in long-term care facilities for the elderly.

BACKGROUND: To determine if the more interventionist approach of screening with the tuberculin test and chemoprophylaxis for high-risk positive reactors to control tuberculosis in long-term care facilities is cost-effective when compared to the case-finding and treatment approach. METHOD: A decision-analysis model was designed wherein systematic screening with the tuberculin skin test of all elderly patients newly admitted to facilities was compared to public health interventions restricted to investigation of cases and contacts with symptoms of tuberculosis after suspected exposure. Differen…

Cavitating pulmonary tuberculosis below age of 2 years.

A prospective study was conducted on 75 consecutive cases of primary cavitary pulmonary tuberculosis in hospitalized children below 2 years to determine the spectrum of the disease. Diagnosis was based on clinical, radiological, tuberculin test and histopathological findings and not on isolation of tubercle bacilli. Peak age was 7-12 months. Ten mothers suffered from pulmonary fibrocavitary disease and 73% of cases were severely malnourished. Presenting symptoms were fever and cough, at times dyspnea and often followed measles. Unlike adults, hemoptysis was seldom encountered. Cavitary lesions…

Immune response to other agents of calves persistently infected with bovine virus diarrhoea virus (BVDV).

The ability of calves persistently infected (PI) with bovine virus diarrhoea virus BVDV to respond immunologically to defined antigens other than BVDV was studied. Five clinically healthy PI calves were studied together with 5 non-PI calves serving as controls. The humoral immune response was tested by measuring the serum antitoxin titre following immunization against tetanus. The cellular immune response was tested by the ability to develop a positive reaction in a cutaneous tuberculin test performed 1 month after immunization against Johne's disease (paratuberculosis). Finally, a skin-sensit…

Suppression of delayed hypersensitivity by the depletion of circulating monocytes.

The intraperitoneal injection of Marcol oil was previously reported to result in the depletion of circulating monocytes. The way in which this depletion affected guinea-pigs' ability to respond to challenge with antigen was tested by challenging with tuberculin after sensitization and examining the ability to respond to DNCB. It was found that the ability to respond to tuberculin was completely abrogated by the injection of Marcol and could be restored by removing the oil from the peritoneal cavity. The ability to respond to a secondary challenge with DNCB was not affected, while the ability t

QuantiFERON-TB Gold and the TST are both useful for latent tuberculosis infection screening in autoimmune diseases.

Screening for active tuberculosis (TB) and latent TB infection (LTBI) is mandatory prior to the initiation of tumour necrosis factor-alpha inhibitor therapy. However, no agreement exists on the best strategy for detecting LTBI in this population. The aim of the present study was to analyse the performance of the tuberculin skin test (TST) and QuantiFERON-TB Gold in-tube (QFT-GIT) on LTBI detection in subjects with immunomediated inflammatory diseases (IMID). The TST and QFT-GIT were prospectively performed in 398 consecutive IMID subjects, 310 (78%) on immunosuppressive therapy and only 16 (4%

BCG vaccination of full-term infants with chronic intrauterine malnutrition: influence of immunization age on development of post-vaccination, delayed tuberculin hypersensitivity.

To determine the effect of intrauterine growth retardation (IUGR) on the response to BCG vaccination, we evaluated the specific delayed tuberculin hypersensitivity of 57 full-term infants with symmetric IUGR (SGA or small for gestational age) and 52 full-term infants with normal intrauterine growth (AGA or appropriate for gestational age). The infants were evaluated using post-vaccination skin tests to tuberculin purified protein derivative (PPD) and tuberculin lymphocyte transformation tests. Using a positive response to the skin test as an indicator of delayed hypersensitivity, we found that

Minimizing tuberculosis risk to hospital personnel and students exposed to unsuspected disease.

A 15-month prospective study of the personnel and students exposed to initially unsuspected active cases of tuberculosis was undertaken to define the risk of their acquiring infection. Eight of 484 (1.65%) personnel exposed to 17 initially unsuspected tuberculosis patients and 5 of 2013 (0.25%) unexposed personnel with similar risk but no known exposures developed positive tuberculin skin tests. This greater than sixfold increase in conversion rate for the exposed group was significant (chi-2=14.83; P=0.0003). Delay in making the diagnosis in these patients was associated with failure to apply

Current tuberculin reactivity of schoolchildren in the Central African Republic.

BACKGROUND: The tuberculin skin test (TST) is the recommended method for screening for Mycobacterium tuberculosis infection in many countries. We used this technique to assess bacillus Calmette-Guérin (BCG) status and to estimate the current prevalence and annual rate of latent tuberculosis infection in schoolchildren in the Central African Republic. METHODS: Two tuberculin units of 0.1 ml purified protein derivative TR23 were injected intradermally into the left forearm of 2710 children attending school in Bangui and Ombella M'Poko. The induration size was interpreted at cut-off points of ≥
