Absent pulmonary valve with tricuspid atresia/stenosis: literature review with new three long-term cases.

Absent pulmonary valve (APV) syndrome with tricuspid atresia or tricuspid stenosis (TA/TS) is an extremely rare malformation recently reported as a variant of APV with intact ventricular septum (VS). The condition, however, has univentricular physiology and unique structural and clinical features. The purpose of this study was to update the current knowledge about this condition by describing long-term outcomes of three new cases and reviewing the available literatures. A systematic literature search was performed to collect clinical and anatomical data of APV with TA/TS. Institutional medical

Outcomes after the palliative arterial switch operation in neonates with single-ventricle anatomy.

BACKGROUND: Newborns with single-ventricle anatomy, transposition of the great arteries, and systemic outflow obstruction are challenging patients most often managed with a Norwood or Damus-Kaye-Stansel (DKS) procedure. The palliative arterial switch operation (pASO) offers the theoretical advantage of avoiding a systemic-to-pulmonary artery shunt physiology and posterior entrapment of the left pulmonary artery while aligning the single left ventricle with the posterior semilunar valve. Limited outcome data exist for the pASO. The purpose of this study was to examine the clinical course of pat

A Clinical Perspective of the Takotsubo Syndrome.

This article provides a contemporary review of the clinical features of the takotsubo syndrome. The author discusses hallmark elements that distinguish this novel acute cardiac condition from the more common acute coronary syndrome. This review includes relevant clinical detail surrounding findings on ECG, biochemical testing, and cardiac imaging and a discussion of complications, including acute decompensated heart failure, arrhythmias, ventricular thrombi, and left ventricular outflow obstruction. The review concludes with discussion of proper treatment, long-term survival, and recurrence.

Abnormal left ventricular filling after neonatal repair of congenital heart disease: association with increased mortality and morbidity.

BACKGROUND: The presence of mid-diastolic flow reversal on the mitral valve Doppler inflow indicates abnormal left ventricular filling. To determine whether mid-diastolic flow reversal predicts outcome in patients undergoing repair or palliation of neonatal congenital heart disease, we reviewed the echocardiograms and medical records of 40 patients with either left ventricular outflow obstruction or transposition of the great arteries. METHODS: All patients underwent surgical repair; transposition of the great arteries (TGA) = 17, coarctation of the aorta (CoA) = 14, interrupted aortic arch (

Cardiac Sarcoma: A Rare Etiology of Outflow Tract Obstruction.

Cardiac sarcomas are exceedingly rare clinical entities that can require expeditious surgical intervention for relief of symptomatic and hemodynamic sequelae and for improved oncologic outcomes. Here we present 2 cases of primary sarcoma resulting in markedly symptomatic obstruction of the left and right ventricular outflow tracts, thus highlighting the principles of surgical intervention for these rare malignant lesions.

Systemic hypertension in an infant with unrepaired tetralogy of Fallot: case report.

Patients with severe right ventricular outflow tract obstruction in tetralogy of Fallot typically have right-to-left shunting, resulting in low pulmonary blood flow and cyanosis. Here we present the case of an infant with tetralogy of Fallot and severe pulmonary valve stenosis, complicated by systemic hypertension, the presence of which altered flow dynamics and possibly prevented cyanosis.

Paradoxical response to exercise in asymptomatic hypertrophic cardiomyopathy: a new description of outflow tract obstruction dynamics.

OBJECTIVES: The purpose of this study was to analyze left ventricular obstruction in patients with hypertrophic cardiomyopathy (HCM) during exercise echocardiography. BACKGROUND: Despite the association of symptoms with left ventricular outflow tract obstruction in HCM, there exist paradoxical situations in which significant intraventricular gradients (>50 mm Hg) at rest occur in conjunction with excellent exercise tolerance. METHODS: To examine this phenomenon, we performed exercise echocardiography and analyzed the clinical status of 107 HCM patients with and without resting obstruction.

Dynamic mild subaortic left ventricular obstruction caused by an accessory mitral valve attached to the anterior mitral valve in a young pregnant woman.

Accessory mitral valve tissue is an extremely rare congenital cardiac anomaly of embryologic development of the endocardial cushion. This anomaly is often associated with left ventricular outflow tract obstruction (LVOTO). A 26-year-old pregnant female was referred to our Department of Cardiology with exertional shortness of breath and tachycardia. Transthoracic and transesophageal echocardiography revealed a flexible circular (1.3 x 1.4 cm), mobile structure attached to the ventricular side of anterior mitral valve leaflet, with chordal attachments structure from anterior papillary muscle. A rare combination of left ventricular noncompaction, patent ductus arteriosus, and persistent left superior vena cava demonstrated by multidetector computed tomography and echocardiography.

Noncompaction of the ventricular myocardium (NCM) is a disorder characterized by numerous prominent ventricular trabeculations and deep intertrabecular recesses. It may present in an isolated form or in association with other cyanotic heart diseases, obstructions of the ventricular outflow tract, and anomalies of coronary arteries. We report on a rare combination of NCM, patent ductus arteriosus, and persistent left superior vena cava in a 27-year-old man examined via echocardiography and multidetector computed tomography.

High-Precision Hemodynamic and Echocardiographic Assessment of Pacing in Obstructive Hypertrophic Cardiomyopathy.

BACKGROUND: Left ventricular outflow obstruction drives symptoms and outcomes in obstructive hypertrophic cardiomyopathy (oHCM). Right ventricular pacing (RVP) can desynchronize the left ventricle to relieve this and allows control of atrioventricular delay (AVD) but may impair ventricular function. We used high-precision assessment to quantify the hemodynamic and echocardiographic effects of RVP in oHCM. METHODS: Patients with oHCM and implanted dual-chamber pacing devices underwent continuous recording of ECG, beat-by-beat outflow tract continuous wave Doppler, and beat-by-beat, noninvasive

Two-Ventricle repair for hypoplastic left heart syndrome.

Hypoplastic left heart syndrome encompasses a spectrum of structural cardiac malformations characterized by severe underdevelopment of the structures in the left heart-aorta complex. The severe end of the spectrum consists of aortic atresia and mitral atresia, whereas at the mild end patients have aortic valve and mitral valve hypoplasia without intrinsic valve stenosis, recently described as hypoplastic left heart complex. These patients may be candidates for biventricular repair. Since November 1988, 12 of 13 patients (92%) with hypoplastic left heart complex have undergone two-ventricle rep

Seventy Years Managing Hypoplastic Left Heart Syndrome - What has been Learned and What Remains to be Learned.

In this review, we describe the evolution of the surgical management of Hypoplastic Left Heart Syndrome from its initial description through the development of contemporary interventional strategies. Through this account, we highlight lessons learned from prior comparisons and how these apply to the current debate, Norwood vs Hybrid palliations. The Congenital Heart Surgeons Society - Critical Left Ventricular Outflow Tract Obstruction Cohort has played an important role in defining outcomes and shows promise for future understanding.

Apico-pulmonary artery conduit repair of congenitally corrected transposition of the great arteries with ventricular septal defect and pulmonary outflow tract obstruction: a 10-year follow-up.

BACKGROUND: In conventional repair of the congenitally corrected transpositions of the great arteries associated with ventricular septal defect and pulmonary outflow tract obstruction, the placement of the left ventricle-pulmonary artery conduit is at risk owing to probable compression by the sternum, heart block, or injury to the mitral anterior papillary muscle. Apical placement of the left ventriculotomy for the inflow conduit rather than in the midportion or base placement may avoid these complications, although this results in a long and winding extracardiac conduit that may be short-live

Standing position alone or in combination with exercise as a stress test to provoke left ventricular outflow tract gradient in hypertrophic cardiomyopathy and other conditions.

Measuring left ventricular outflow tract (LVOT) gradient by echocardiography in decubitus position, which is used in routine clinical practice, does not reflect the pathophysiology of this dynamic abnormality during daily activities, which trigger the symptoms. LVOT obstruction is dynamic and greatly dependent upon the left ventricular cavity size, geometric configuration of hypertrophy, load, contractility and mitral apparatus abnormalities, including systolic anterior motion of mitral leaflet. Importantly, LVOT gradient may develop not only in hypertrophic cardiomyopathy, but also in other h

[Role of transesophageal echocardiography in the diagnosis of obstructive hypertrophic myocardiopathy. Report of 2 cases and review of the literature].

Two cases of obstructive hypertrophic cardiomyopathy, in which the transthoracic echocardiography was not enough to reach the right diagnosis, are reported. The transesophageal echocardiography revealed to be the most useful technique to reach the exact diagnosis and more information about the mechanism of the left ventricular outflow tract obstruction. Moreover, this technique could be useful to direct to a surgical treatment and to monitorize the mitral valve function and the possible complications during operation.

Prenatal diagnosis of mitral-pulmonary intervalvular fibrosa pseudoaneurysm associated with TGA causing severe pulmonary stenosis: A rare case report.

UNLABELLED: Pseudoaneurysm of the mitral-aortic intervalvular fibrosa is a rare and typically acquired cardiac lesion, with congenital forms being exceptionally uncommon, particularly when identified prenatally. We report the case of a 23-year-old gravida 2 woman with one previous history of miscarriage who was referred at 25 weeks' gestation for a fetal intracardiac mass. Fetal echocardiography revealed a cystic echogenic mass at the crux of the heart, consistent with a pseudoaneurysm arising from the mitral-pulmonary intervalvular fibrosa (MPIVF) associated with transposition of the great ar
