Evaluation of axial and flexural stresses in the vertebral body cortex and trabecular bone in lordosis and two sagittal cervical translation configurations with an elliptical shell model.

BACKGROUND: Osteoarthritis and spinal degeneration are factors in neck and back pain. Calculations of stress in clinically occurring configu … e determined from digitizing. RESULTS: Compared with the normal lordosis, stresses on the anterior vertebral body cortical margins of C5-T1 in the sagittal translated postures are compression rather than tension. At the posterior vertebral bodies in the anteriorly translated position and vertically translated postures, the stresses change from compression to te …

Exploring the impact of body mass index on the accuracy of vertebral bone quality in determining bone mineral density in patients undergoing lumbar fusion surgery.

OBJECTIVE: To investigate whether body mass index (BMI) affects the accuracy of vertebral bone quality (VBQ) in determining bone mineral density (BMD) in patients undergoing lumbar fusion surgery. METHODS: In this retrospective study, patients with preoperative noncontrast T1-weighted MRI were included. Restricted Cubic Spline (RCS) was employed to explore the nonlinear relationship between BMI and VBQ. Then patients were stratified according to the threshold of BMI. Pearson correlation analysis and linear regression were used to analyze the correlation between VBQ and the BMD in different gr

Strain changes on the cortical shell of vertebral bodies due to spine ageing: a parametric study using a finite element model evaluated by strain measurements.

The probability of fractures of the cortical shell of vertebral bodies increases as ageing progresses. Ageing involves all the spinal component changes. However, the effect of the spinal component ageing on the fracture risk of the cortical shell remains poorly understood. In this study, the influence of the ageing of the spinal components on cortical shell strain was investigated. A lumbar spinal specimen (L3-L5) was mechanically tested under a quasi-static axial compressive load. Clinical computed tomography images of the same specimen were used to create a corresponding finite element model

Significant traumatic atrophy of the spinal cord in connection with severe cervical vertebral body hypoplasia in a boy with Larsen syndrome: a case report and review of the literature.

INTRODUCTION: Cervical kyphosis may be potentially the most serious and, indeed, a life-threatening manifestation of Larsen syndrome because of the impingement on the spinal cord at the apex of the kyphosis. Abnormalities of the spine, specifically cervicothoracic kyphosis requires specific attention and management. CASE PRESENTATION: We report on a 3-year-old boy who presented with full clinical and the radiographic features of Larsen syndrome. There was significant vertebral body hypoplasia of C5/7 combined with spina bifida occulta from C1/T2, resulting in congenital cervical instability a…

Relationships between densitometric and morphological parameters as measured by peripheral computed tomography and the compressive behavior of lumbar vertebral bodies from macaques (Macaca fascicularis).

STUDY DESIGN: The measured maximum compressive force and stress of lumbar vertebral bodies from cynomolgus monkeys were compared with peripheral quantitative computed tomography (pQCT) derived densitometric and morphologic vertebral parameters. OBJECTIVES: To determine, which pQCT parameters were predictive of vertebral mechanical behavior, and of these, identify those that were the best predictors of vertebral load capacity. A secondary objective was to test the suitability of a multiple parameter-based approach for predicting vertebral mechanical load response. SUMMARY OF BACKGROUND DATA:

Vascular anatomy of L1 vertebral body in Wistar rat-corrosion-fluorescence, diaphanization and histological analysis, comparison to humans, and importance in blood supply-related investigation.

BACKGROUND: The factors influencing bone healing in complete burst fractures-key to deciding between stabilization or vertebral replacement-remain poorly defined. Arterial vascularization and bone nutrition are known to affect healing or progression to necrosis. However, the role of vascular injury in vertebral fractures is not yet conclusively demonstrated and is not currently factored into the decision-making for ad initium vertebral body replacement surgery. This study aims to analyze the vascularization of the L1 vertebral body in Wistar rats and compare it to human anatomy. The goal is to…

Risk factors of early subsidence of 3D-printed artificial vertebral body following single-level anterior cervical corpectomy and fusion (ACCF): retrospective study of 98 patients.

BACKGROUND: The 3D-printed artificial vertebral body (AVB) was designed with theoretically better biomechanical properties than traditional implants to decrease the incidence of implant subsidence. However, implant subsidence still occurs, with the potential risk factors for AVB subsidence remaining unknown. This study aimed to identify the risk factors for early subsidence of AVB after single-level anterior cervical corpectomy and fusion (ACCF). METHODS: Consecutive patients who underwent single-level ACCF using 3D-printed AVB for cervical spondylotic myelopathy between 2018 and 2022 were in…

L1 vertebral body replacement using 3D-printed polylactic acid bioimplants: in vitro cellular evaluation, in vivo rat model assessment, and histological analysis of implant osseointegration.

BACKGROUND: The vertebral body plays a crucial role in supporting compressive loads and maintaining spinal biomechanics. An ideal biomaterial for total vertebral body replacement should combine biological and mechanical properties, yet no current material fulfills all criteria. This pilot study explores the use of a novel three-dimensional (3D)-printed porous polylactic acid (PLA) implant for total L1 vertebral body replacement. METHODS: This study had four stages: first, design, optimization, and 3D printing of the PLA device; second, in vitro evaluation of biocompatibility and cell growth u…

Mechanical Cavity Creation with Curettage and Vacuum Suction (Q-VAC) in Lytic Vertebral Body Lesions with Posterior Wall Dehiscence and Epidural Mass before Cement Augmentation.

Background and Objectives: We describe a novel technique for percutaneous tumor debulking and cavity creation in patients with extensive lytic lesions of the vertebral body including posterior wall dehiscence prior to vertebral augmentation (VA) procedures. The mechanical cavity is created with a combination of curettage and vacuum suction (Q-VAC). Balloon kyphoplasty and vertebral body stenting are used to treat neoplastic vertebral lesions and might reduce the rate of cement leakage, especially in presence of posterior wall dehiscence. However, these techniques could theoretically lead to in…
