Some familial forms of the dermatological condition vitiligo have recently been linked to polymorphisms in the innate immunity gene, NLRP1. Here, we review what is currently known about the mechanisms that regulate activation of the NLRP1 protein and the downstream effects of NLRP1 on pathways impacting inflammation and apoptosis. How polymorphic variants of the NLRP1 gene contribute to the pathogenesis of vitiligo remains mysterious, requiring further investigation.

Vitiligo and psoriasis are both common diseases. However, coexistence of these two diseases is rare. The pathogenesis of the coexistence of vitiligo and psoriasis is still unknown. Herein, we report three children (11-year-old, 8-year-old, and 7-year-old females) who attended our Department of Dermatology with coexisting vitiligo and psoriasis.

A 52-year-old woman with recently diagnosed vitiligo presented at the Skin Camouflage Clinic of the British Red Cross. She found these hypo-pigmented patches very distressing and wanted to be able to cover them up. She was referred to the clinic by her general practitioner. Two thousand consecutive children were analyzed in a 2-year period to establish a clinical and epidemiologic profile of vitiligo in Jordan; 71 had vitiligo, with the prevalence increasing with age (0.45% <1 yr, 1% aged 1-5 yrs, 2.1% aged 5-12 yrs). Nonsegmental-type vitiligo was seen in 67 of the patients (92.9%).

Refining the position of loci on chromosomes 7 and 9 previously linked with generalized vitiligo or vitiligo-associated autoimmune diseases presenting in families has been performed by high-density single-nucleotide polymorphism (SNP) genotyping. Investigation of the genetic interaction among these loci (and with a previously identified susceptibility gene, NLRP1, on chromosome 17) as risk factors for vitiligo demonstrates the complex nature of this disease.

In a recent genome-wide association study of generalized vitiligo, we identified ten confirmed susceptibility loci. By testing additional loci that showed suggestive association in the genome-wide study, using two replication cohorts of European descent, we observed replicated association of generalized vitiligo with variants at 3p13 encompassing FOXP1 (rs17008723, combined P=1.04x10(-8)) and with variants at 6q27 encompassing CCR6 (rs6902119, combined P=3.94x10(-7)).
