Chyluria: what does the clinician need to know?

Chyluria is secondary to the presence of chyle in the urine. The classical appearance on inspection is of milky white urine, which is caused by a fistulous communication between the lymphatic system and the urinary tract. Worldwide, it is most commonly associated with the parasite Wuchereria bancrofti, which is prevalent in Asia, most extensively in India but also China and Taiwan. However, in the United Kingdom, Europe and North America, where the condition is rare, non-parasitic aetiologies predominate. Chyluria is occasionally associated with other urinary tract symptoms including infection…

Subcutaneous filariasis: An unusual presentation with an adult gravid worm on aspiration.

Detection of adult gravid filarial worm in fine-needle aspiration cytology (FNAC) smears is unusual despite the high incidence of this parasite in endemic zones. Bancroftian filariasis presenting as subcutaneous nodules is very rare, and only a few cases are reported in literature. In spite of various reports stating the presence of microfilaria using aspiration cytology in various sites, the presence of an adult gravid filarial worm in subcutaneous nodules by FNAC is an extremely rare finding. We hereby report an unusual case of detection of adult gravid worm of Wuchereria bancrofti on FNAC o…

Laboratory diagnosis of infections due to blood and tissue parasites.

Microscopy remains the cornerstone of the laboratory diagnosis of infections due to blood and tissue parasites. Examination of thick and thin peripheral blood smears stained with Giemsa or other appropriate stains is used for detection and identification of species of Plasmodium, Babesia, Trypanosoma, Brugia, Mansonella, and Wuchereria. Even in the hands of well-trained technologists, diagnosis may be hampered by the sparseness of organisms on the slide and by the subjective nature of differentiating similar-appearing organisms. Microscopy and/or culture of ulcer, bone marrow, tissue aspirate,…

Distribution of filarial elephantiasis and hydrocele in Matara district, Sri Lanka, as reported by local leaders, and an immunological survey in areas with relatively high clinical rates.

To eliminate lymphatic filariasis by means of mass drug administration, it is essential to have reliable data on the disease distribution an … ere no endemic filariasis had been known. To confirm active transmission of filariasis in Deniyaya, Wuchereria bancrofti antigen and filaria-specific urinary IgG4 antibody were measured with 2436 subjects. The positive rates for antigen and antibody were 0.6% and 4.3%, respectively. The titer analysis of IgG4 according to age revealed that the youngest IgG4 …

Comparative epidemiological studies on lymphatic filariasis, between tribal and non-tribal populations of Bankura district, West Bengal, India.

There is little information on the epidemiology of lymphatic filariasis, caused by infection with Wuchereria bancrofti, in West Bengal. That which is known mostly relates to Calcutta and its suburbs, and there have been few studies on the disease among the tribal populations of the state. The present study was designed to compare the epidemiology of filariasis in tribal and non-tribal populations living in Bankura district. Blood samples were collected at night from 2076 tribal and 4985 non-tribal subjects and checked, microscopically, for microfilariae (mff). Each subject was also examined cl…

Assessment of immunochromatographic test for rapid lymphatic filariasis diagnosis.

Two rapid immunodiagnostic tests (ICT Filariasis test), developed for the quick diagnosis of Wuchereria bancrofti infection, have been validated in laboratory and field situation. The aim of this study was to assess the performance and usefulness of this antigen capture assay as a diagnostic method in three foci of lymphatic filariasis, located in the South Pacific (Society archipelago, French Polynesia), with different levels of endemicity. A sample of 1,595 patients was tested with this assay in parallel with a reference Og4C3 antigen capture assay and microfilariae detection. A second-gener…

Antigenicity of a protective recombinant filarial protein in human bancroftian filariasis.

A 92-kDa fusion protein that encodes amino acids 1-479 of a 62-kDa Brugia malayi antigen induces resistance to microfilariae in mice. The antigenicity of this recombinant protein was explored in asymptomatic residents of Wuchereria bancrofti-endemic areas of Papua New Guinea and Egypt. Unlike sera from individuals in nonendemic areas, sera from residents of endemic areas contained IgG3 antibodies (up to dilution 1:1280) reactive with the fusion protein. There was little or no recombinant antigen-specific IgG1, IgG2, IgG4, or IgE. The mean level of IgG3 antibodies to the amino acid 1-479 constr…

Mosquitoes of Bali Island, Indonesia: common species in the village environment.

Routine sampling of mosquito populations in rural villages was carried out during 13 months at 4 locations in southern Bali Island, Indonesia. Sampling was by light trapping and early night resting collections around animal stables. Specimens collected were preserved for assay of arthropod-borne viruses; 104,608 specimens representing 20 species were prepared in 2681 pools for viral assay. Anopheles barbirostris and An. subpictus have been shown to be important vectors of Brugia malayi and B. timori and of malaria and Wuchereria bancrofti in other parts of Indonesia but have not been incrimina…

Use of levamisole in parasitic infections.

Levamisole is a drug of choice for treatment of ascariasis. With recommended dosages, it is virtually free of side effects. Single doses of … ies are needed. It has been shown that levamisole has significant activity against microfilariae of Wuchereria bancrofti and Brugia malayi. It is not, however, as effective as diethylcarbamazine ('Hetrazan'), and side reactions are greater. In tolerated doses, levamisole does not have significant action on adult forms or microfilariae of Onchoceea volvulus. …

Some transfusion-induced parasitic infections in Zambia.

The risk of acquiring a transfusion-induced infection in Zambia was studied for the first time. Blood slide examination of donors, despite the insensitivity of the method, established malaria as the most serious hazard. The species involved was Plasmodium falciparum, the cause of cerebral malaria, and which could be rapidly fatal in a non-immune host visiting an endemic area. Microfilariae of Dipetalonema perstans and Wuchereria bancrofti were also found in donor populations. While no disease may be induced, allergic reactions due to the breakdown products of dead microfilariae may manifest th…

Mapping of lymphatic filariasis: "to be (in English) or not to be".

Despite lack of supporting evidence from epidemiologic studies and/or confirmed case reports over the last 30 years, maps in non-specialist works on tropical medicine as well as in documents from specialized organizations (e.g. WHO) systematically include Congo and Gabon, two French-speaking countries, within the zone of endemic filariasis due to Wuchereria bancrofti. One certainty is that no cases were found in field studies carried out in these countries in the late 1970s and 1980s but unfortunately published only in French. Studies that we carried out in the Congo between 1982 and 1987 as p…

Correlated prevalence of hydrocele and microfilaremia in Amazon (Belém, 1951-2005).

BACKGROUND & OBJECTIVES: For decades, the city of Belém in Brazil's eastern Amazon was the second city in the country with highest prevalence of cases of filariasis due to Wuchereria bancrofti infection. However, this prevalence decreased over time until reaching null records, concomitantly with a decrease in frequency of recorded hydrocele cases. In this context, we analyzed cross-sectional data to evaluate the degree of correlation between prevalence of positive blood microfilariae results during surveillance screening occurred along 54 years (1951-2005) and prevalence of hydrocele cases rec…

Detection of potential transmission foci of lymphatic filariasis using molecular xenomonitoring in Huahine, French Polynesia.

BACKGROUND: In French Polynesia, substantial progress has been achieved in eliminating lymphatic filariasis (LF) caused by Wuchereria bancrofti var. pacifica, a parasite transmitted by the mosquito vector Aedes polynesiensis. However, despite multiple rounds of Mass Drug Administration (MDA), LF transmission persists on some islands, underscoring the need for robust surveillance to evaluate transmission risks and identify potential transmission foci. METHODOLOGY/PRINCIPAL FINDINGS: An extensive entomological survey combined with a Molecular Xenomonitoring (MX) study was conducted on Huahine I…

Lymphatic filariasis endgame strategies: Using GEOFIL to model mass drug administration and targeted surveillance and treatment strategies in American Samoa.

American Samoa underwent seven rounds of mass drug administration (MDA) for lymphatic filariasis (LF) from 2000-2006, but subsequent surveys found evidence of ongoing transmission. American Samoa has since undergone further rounds of MDA in 2018, 2019, and 2021; however, recent surveys indicate that transmission is still ongoing. GEOFIL, a spatially-explicit agent-based LF model, was used to compare the effectiveness of territory-wide triple-drug MDA (3D-MDA) with targeted surveillance and treatment strategies. Both approaches relied on treatment with ivermectin, diethylcarbamazine, and albend

Semiannual Treatment of Albendazole Alone is Efficacious for Treatment of Lymphatic Filariasis: A Randomized Open-label Trial in Cote d'Ivoire.

BACKGROUND: Ivermectin (IVM) plus albendazole (ALB), or IA, is widely used in mass drug administration (MDA) programs that aim to eliminate lymphatic filariasis (LF) in Africa. However, IVM can cause severe adverse events in persons with heavy Loa loa infections that are common in Central Africa. ALB is safe in loiasis, but more information is needed on its efficacy for LF. This study compared the efficacy and safety of 3 years of semiannual treatment with ALB to annual IA in persons with bancroftian filariasis. METHODS: Adults with Wuchereria bancrofti microfilaremia (Mf) were randomized to …

An investigation of the disparity in estimates of microfilaraemia and antigenaemia in lymphatic filariasis surveys.

BACKGROUND: The diagnosis of lymphatic filariasis (LF) is based typically on either microfilaraemia as assessed by microscopy or filarial antigenaemia using an immuno-chromatographic test. While it is known that estimates of antigenaemia are generally higher than estimates of microfilaraemia, the extent of the difference is not known. METHODS: This paper presents the results of an extensive literature search for surveys that estimated both microfilaraemia and antigenaemia in order to better understand the disparity between the two measures. RESULTS AND CONCLUSIONS: In some settings there was

Lymphatic filariasis: perspectives on lymphatic remodeling and contractile dysfunction in filarial disease pathogenesis.

Lymphatic filariasis, one of the most debilitating diseases associated with the lymphatic system, affects over a hundred million people worldwide and manifests itself in a variety of severe clinical pathologies. The filarial parasites specifically target the lymphatics and impair lymph flow, which is critical for the normal functions of the lymphatic system in maintenance of body fluid balance and physiological interstitial fluid transport. The resultant contractile dysfunction of the lymphatics causes fluid accumulation and lymphedema, one of the major pathologies associated with filarial inf
