The relationship between glucocerebrosidase mutations and Parkinson disease.

Parkinson disease (PD) is the second most common neurodegenerative disorder after Alzheimer disease, whereas Gaucher disease (GD) is the mos … is identical to idiopathic PD, with nigral dopamine cell loss, Lewy bodies, and neurites containing alpha-synuclein. The mechanism by which GBA1 mutations increase the risk for PD is still unknown. However, given that clinical manifestation and pathological findings in PD-GBA1 patients are almost identical to those in idiopathic PD individuals, it is likely that, …

Regulatory endorsement for the application of α-synuclein seed amplification assay as a susceptibility and risk biomarker for clinical trials targeting synucleinopathies.

Transformation in the neurosciences in biomarkers is enabling novel therapeutic strategies targeting earlier stages of disease prior to onset of clinical symptoms. Emerging progress in the area of synucleinopathies, including Parkinson's disease (PD) and dementia with Lewy bodies (DLB) have significant implications for clinical trials. Misfolded α-synuclein aggregates (syn-seeds) propagate by templated misfolding of native α-synuclein, driving disease spread. The advent of the α-synuclein seed amplification assay (syn SAA) now enables highly sensitive and specific detection of syn-seeds in cer

Evaluation of Neuroprotective Effect of Gut Microbe in Parkinson's Disease: An In Silico and In Vivo Approach.

Parkinson's disease is a progressive neurodegenerative disorder marked by the death of dopaminergic neurons in the substantia nigra region of the brain. Aggregation of alpha-synuclein (α-synuclein) is a contributing factor to Parkinson's disease pathogenesis. The objective of this study is to investigate the neuroprotective effects of gut microbes on α-synuclein aggregation using both in silico and in vivo approaches. We focussed on the interaction between α-synuclein and metabolites released by gut bacteria that protect from PD. We employed three probiotic microbe strains against α-synuclein …

Discovery of lipid biomarkers for idiopathic Parkinson's disease in red blood cells and plasma.

Parkinson's disease (PD) is a neurodegenerative disorder characterized by dopaminergic neuron loss and lipid-rich Lewy bodies containing misfolded alpha-synuclein, implicating lipid dysregulation in pathogenesis. We performed comprehensive lipidomic profiling using liquid chromatography-ion mobility mass spectrometry on paired red blood cell (RBC) and plasma samples from idiopathic PD (iPD) patients (n = 156) and cognitively normal controls (n = 155). We identified significant alterations in 240 RBC and 113 plasma lipids, with elevated sphingomyelins, ceramides, and phosphatidylcholines in RBC…

Depending on intensity, exercise improved or worsened pathology in a model of prodromal Parkinson's disease.

Exercise is increasingly applied as non-pharmacological intervention in Parkinson's disease (PD). While evidence suggests beneficial effects, the mechanisms, optimal intensity, and potential risks remain unclear. This study investigated the effects of treadmill exercise on motor and non-motor symptoms, alpha-synuclein pathology, neuroinflammation, and gut microbiota composition in Thy1-aSyn transgenic mice, a model of prodromal PD. Moderate (5 m/min) or intensive (12.5-15 m/min) treadmill exercise three times a week for one month delayed motor deficits, with improvements in beam-walking perfor…

Approaches to Disease Modification for Parkinson's Disease: Clinical Trials and Lessons Learned.

Despite many clinical trials over the last three decades, the goal of demonstrating that a treatment slows the progression of Parkinson's disease (PD) remains elusive. Research advances have shed new insight into cellular pathways contributing to PD pathogenesis and offer increasingly compelling therapeutic targets. Here we review recent and ongoing clinical trials employing novel strategies toward disease modification, including those targeting alpha-synuclein and those repurposing drugs approved for other indications. Active and passive immunotherapy approaches are being studied with the goa…

Appendectomy in mid and later life and risk of Parkinson's disease: A population-based study.

INTRODUCTION: Pathogenic movement of alpha-synuclein from the gut to the brain in PD has been proposed. The appendix has a relatively high density of alpha-synuclein deposition in neurologically healthy individuals. We investigated the incidence of PD after appendectomy. METHODS: Using cause-specific hazards regression models, we compared persons over 35 years of age who had undergone appendectomy with two groups of age- and sex-matched individuals having had: (1) a cholecystectomy and (2) neither procedure. Subsequent diagnoses of PD were identified. RESULTS: Among 42,999 individuals underg…

β-Synuclein: An Enigmatic Protein with Diverse Functionality.

α-Synuclein (αS) is a small, unstructured, presynaptic protein expressed in the brain. Its aggregated form is a major component of Lewy bodies, the large proteinaceous deposits in Parkinson's disease. The closely related protein, β-Synuclein (βS), is co-expressed with αS. In vitro, βS acts as a molecular chaperone to inhibit αS aggregation. As a result of this assignation, βS has been largely understudied in comparison to αS. However, recent reports suggest that βS promotes neurotoxicity, implying that βS is involved in other cellular pathways with functions independent of αS. Here, we review

Role of α- and β-Synucleins in the Axonal Pathology of Parkinson's Disease and Related Synucleinopathies.

Axonal swellings are histological hallmarks of axonopathies in various types of disorders in the central nervous system, including neurodegenerative diseases. Given the pivotal role of axonopathies during the early phase of neurodegenerative process, axonal swellings may be good models which may provide some clues for early pathogenesis of α-synucleinopathies, including Parkinson's disease and dementia with Lewy bodies (DLB). In this mini-review, such a possibility is discussed based on our recent studies as well as other accumulating studies. Consistent with the current view that dysfunction

Function of miRNAs in Conferring Neuroprotection through a Tailored Peptide Derived from Snake Venom Nerve Growth Factor in Response to Paraquat-Induced Toxicity in Caenorhabditis elegans.

Parkinson's disease (PD) is a major global health challenge, shaped by environmental factors and the accumulation of alpha-synuclein. MicroRNAs (miRNAs), key regulators of gene expression, play critical roles in neurodegeneration, and their dysregulation has been linked to PD development. Paraquat (PQ), a widely used herbicide, can induce PD-like symptoms by generating reactive oxygen species, impairing mitochondrial function, promoting alpha-synuclein aggregation, and triggering apoptosis of dopaminergic neurons. These effects ultimately alter the miRNA expression profiles. Our previous resea…

Neuropathology in genetic Parkinson's disease: a focused review of pathological and clinical findings.

Parkinson's disease (PD) is a neurodegenerative disorder with a complex interaction between genetic and environmental causal factors. The role of Lewy body (LB) pathology (Lewy bodies and neurites) in genetic PD remains unclear, as some gene mutations do not consistently present with alpha-synuclein aggregates or LB in postmortem studies. This review aims to evaluate neuropathological data from genetic PD cases, focusing on the prevalence of LB pathology and co-pathologies, as well as the relationship between neuropathological findings and relevant clinical features. Case reports, case series,…

The brain-first vs. body-first model of Parkinson's disease with comparison to alternative models.

The ultimate origin of Lewy body disorders, including Parkinson's disease (PD) and Dementia with Lewy bodies (DLB), is still incompletely understood. Although a large number of pathogenic mechanisms have been implicated, accumulating evidence support that aggregation and neuron-to-neuron propagation of alpha-synuclein may be the core feature of these disorders. The synuclein, origin, and connectome (SOC) disease model of Lewy body disorders was recently introduced. This model is based on the hypothesis that in the majority of patients, the first alpha-synuclein pathology arises in single locat…

PLK2 disrupts autophagic flux to promote SNCA/α-synuclein pathology.

The aggregation and transmission of SNCA/α-synuclein (synuclein, alpha) is a hallmark pathology of Parkinson disease (PD). PLK2 (polo like kinase 2) is an evolutionarily conserved serine/threonine kinase that is more abundant in the brains of all family members, is highly expressed in PD, and is linked to SNCA deposition. However, in addition to its role in phosphorylating SNCA, the role of PLK2 in PD and the mechanisms involved in triggering neurodegeneration remain unclear. Here, we found that PLK2 regulated SNCA pathology independently of S129. Overexpression of PLK2 promoted SNCA preformed

Unraveling neuroprotection in Parkinson's disease: Nrf2-Keap1 pathway's vital role amidst pathogenic pathways.

Parkinson's disease (PD) is an age-related chronic neurological condition characterized by progressive degeneration of dopaminergic neurons and the presence of Lewy bodies, primarily composed of alpha-synuclein and ubiquitin. The pathophysiology of PD encompasses alpha-synuclein aggregation, oxidative stress, neuroinflammation, mitochondrial dysfunction, and impaired autophagy and ubiquitin-proteasome systems. Among these, the Keap1-Nrf2 pathway is a key regulator of antioxidant defense mechanisms. Nrf2 has emerged as a crucial factor in managing oxidative stress and inflammation, and it also …

Causes of Parkinson's disease: genetics of DJ-1.

The identification of Mendelian mutations in rare forms of familial Parkinson's disease (PD) have provided significant insights into the molecular pathogenesis of this common complex disorder. DJ-1 is the third of four genes known to be definitively causal in familial PD, the three others being alpha-synuclein, parkin and the recently identified PINK1. Mutations in the DJ-1 gene were identified in two European families, a Dutch kindred harbouring a large homozygous genomic deletion encompassing exons 1-5 of the gene and an Italian kindred with a homozygous L166P missense mutation. The clinical…

Neuroprotective effect of Bacillus subtilis in haloperidol induced rat model, targeting the microbiota-gut-brain axis.

Functional microbes regulate Parkinson's disease (PD), according to contemporary research. The mechanism by which probiotics (PBT) improve PD was not fully explored yet. We examined the antioxidant impact and mechanism of PBT (Bacillus subtilis) on PD using gut-brain axis regulation. To establish a model of PD, rats were given haloperidol (HAL) intraperitoneally (i.p.) in this study. The standard group received L-DOPA for 21 days. After that, the motor function was assessed using different neurobehavioral tests. Further estimation comprehends the build up of alpha-synuclein, the manifestation …

TNF alpha unmasks enteric malate aspartate shuttle dysfunction bridging Parkinson disease and intestinal inflammation.

Gastrointestinal dysfunction often precedes motor symptoms in Parkinson's disease (PD), suggesting the enteric nervous system (ENS) is central to early pathogenesis. How α-synuclein contributes to ENS dysfunction, and how inflammation modulates this, remains unclear. Here we show that Tumor Necrosis Factor alpha enhances α-synuclein accumulation in induced pluripotent stem cell-derived enteric neurons and glia, and impairs the malate-aspartate shuttle, a key pathway for mitochondrial energy production. This drives a metabolic shift toward glutamine oxidation in patient cells. This metabolic im
