Outcomes of massive transfusion recipients administered ABO-incompatible fresh frozen plasma.

BACKGROUND: The provision of ABO-incompatible fresh frozen plasma (FFP) in massive transfusion (MT) has become accepted to conserve AB FFP stock. There is an evidence gap in non-trauma settings. We compare characteristics of patients who received ABO-compatible or ABO-incompatible FFP during an MT episode due to any cause of critical bleeding, and assess the impact of incompatible FFP transfusion on inhospital mortality. METHODS: Using the Australian and New Zealand Massive Transfusion Registry, data were extracted for patients aged ≥18 years who received an MT (defined as ≥5 red cell units i

New mAb therapies in multiple myeloma: interference with blood transfusion compatibility testing.

PURPOSE OF REVIEW: Immunotherapeutic strategies are emerging as novel therapeutic approaches in multiple myeloma, with several mAbs being in advanced stages of clinical development. Of these, CD38 targeting antibodies appear very promising. In trials with anti-CD38 mAb daratumumab, all patients demonstrated panreactivity in red blood cell (RBC) panel testing, complicating the selection of compatible RBCs for transfusion. This review provides an overview of the interferences and solutions to safely transfuse these patients. RECENT FINDINGS: CD38 is weakly expressed on human erythrocytes. Since

ABO-incompatible blood transfusion and invasive therapeutic approaches during pediatric cardiopulmonary bypass.

Human error has been identified as a major source of ABO-incompatible blood transfusion which most often results from blood being given to the wrong patient. We present a case of inadvertent administration of ABO-incompatible blood to a 6-mo-old child who underwent congenital heart surgery and discuss the use of invasive therapeutic approaches. Invasive techniques included total circulatory arrest and large-volume exchange transfusion, along with conventional ultrafiltration and plasmapheresis, which could all be performed rapidly and effectively. The combination of standard pharmacologic ther

Exchange transfusion during extra corporeal membrane oxygenation used as a bridge to ABO-mismatch cardiac transplantation.

We report a case in which exchange transfusion was performed directly from the extracorporeal membrane oxygenation circuit in a child being bridged to cardiac transplantation. This allowed preparation for ABO-mismatched heart transplant in a child dependent on extracorporeal support, awaiting a suitable ABO-matched donor. Exchange transfusion or plasmapheresis in this setting would normally be performed on cardiopulmonary bypass to allow exsanguinations under hypothermia immediately pre-transplant. This allows little time for depletion of isohemagglutinins, occasionally leading to prolonged by

A new look on neonatal jaundice.

Out of 6586 live born babies, 736 babies with jaundice were studied from 1st July 1996 to 30th June, 1997, in a city based medical college nursery. Physiological jaundice was present in 8.92% of all live born babies and accounted for 79.89% of babies with jaundice. Breast milk jaundice and prematurity were next common causes responsible for 5.29% each of all cases with neonatal jaundice. Septicaemia caused jaundice in 4.75% cases. Among the babies with jaundice appearing between day 4 and day 7 of life, breast milk jaundice was the commonest cause occurring in 49.25% cases. The last entity sur

Clinical outcomes of children receiving ABO-incompatible versus ABO-compatible heart transplantation: a multicentre cohort study.

BACKGROUND: ABO-incompatible heart transplantation increases donor availability in young children and is evolving into standard of care in children younger than 2 years. Previous smaller studies suggest similar outcomes to ABO-compatible heart transplantation, but persisting alterations of the immune system in ABO-incompatible recipients might increase the risk of some infections or benefit the graft owing to reduced HLA reactivity. We aimed to assess long-term outcomes in young children after they received ABO-incompatible or ABO-compatible heart transplantation. METHODS: In this multicentre

Variation of anti-A and anti-B titers in group O potential blood donors: A pilot study.

BACKGROUND: Resuscitation with fresh whole blood is vital to preserving life on the battlefield. Transfusing low titer O whole blood (LTOWB), defined as anti-A and anti-B titer levels of <1:256, is safe because LTOWB alleviates the risk for hemolytic transfusion reactions. Because of possible variations in titer levels over time, a study was needed using US Navy and Marine Corps personnel to assess how these titers change across two assessments. METHODS: Retrospective data from group O marines and sailors (M = 25 years of age; range, 19-35 years) stationed in the San Diego region were acquire

Potential approaches to prevent uncommon hemolytic side effects of AB0 antibodies in plasma derivatives.

Since hemolytic reactions in patients after administration of plasma derivatives like immunoglobulins or coagulation factor preparations have been described, titers of anti-A and anti-B-antibodies have to be below defined levels for batch release of these plasma-derived therapeutic products according to the European Pharmacopoeia. We have summarized clinical relevance of AB0 antibodies in plasma derivatives and related legal issues in the European Union, United States of America, and Japan. We have also discussed potential approaches for the prevention of hemolytic side effects with feasible s

Association of type-O blood with neuroendocrine tumors in multiple endocrine neoplasia type 1.

CONTEXT: The ABO blood type system describes the expression of human blood group antigens found on both erythrocytes and normal tissue throughout the body. We recently reported an association between O blood type and the manifestation of pancreatic neuroendocrine tumors in a cohort of patients with Von Hippel-Lindau syndrome. OBJECTIVE: The aim of the study was to determine whether there is an association of ABO blood type with the development of neuroendocrine tumors in patients with multiple endocrine neoplasia, type 1 (MEN-1). DESIGN: A retrospective analysis of 105 patients with MEN-1 wa

Prevalence and Association of Transfusion Transmitted Infections with ABO and Rh Blood Groups among Blood Donors at the National Blood Bank, Amman, Jordan.

Blood screening is considered a compulsory procedure in health care services to reduce the occurrence of transfusion transmitted infections (TTIs). This study estimated the distribution rates of ABO and Rh blood group systems, prevalence rates of TTIs among blood donors and their association with the ABO blood group and Rh system. A retrospective study was conducted at the national blood bank, Amman, Jordan for a period of 6 years (from January 2013 to December 2018). For TTIs analysis, about 5 mL blood sample was collected from each volunteer. A total of 365,029 persons (346,048 (94.8%) males

[Biochemical and hematometric effects of exchange transfusion in isoimmunized neonates].

A hundred twenty full term newborns infants (RN) with ABO or Rh isoimmunization who were submitted to exchange transfusion (ET) because hyperbilirubinemia have been studied and with the procedure were demonstrated: 1. Increase the levels of serum sodium in 2.8% in the RN of ABO group and 3.2% in the RN of Rh group with a recurrence of the original values prior to the procedure within three hours post-ET. 2. The levels of serum sodium equal or superior to 180 mEq/L (mmol/L) in the donor's blood led to hypernatremia after the ET, with a recurrence of the original values prior to the procedures w

Distribution of ABO/Rh Blood Types Across Childhood Leukemia Subtypes: A Retrospective Cohort Study of 540 Pediatric Patients.

IMPORTANCE: Blood type has been proposed as a potential factor in cancer susceptibility; however, its role in childhood leukemia remains unclear. Prior studies have yielded conflicting results, and data from pediatric populations are limited. OBJECTIVE: This study examines whether ABO/Rh blood types are differentially distributed across acute lymphoblastic leukemia (ALL), acute myeloid leukemia (AML), and chronic myeloid leukemia (CML) subtypes in children. DESIGN: A retrospective cohort study was conducted that included 540 children aged 0-18 years diagnosed with leukemia between January 20

[Anti-D prophylaxis after abortions and interruptions].

In almost every pregnancy feto-maternal transfusions occur. From the 6th week of pregnancy, blood containing Rhesus antigens may be infused into the maternal circulation and cause sensitization. In case of miscarriage, abortion, ectopic pregnancy and cystic mole the chance of fetomaternal transfusion followed by sensitization of the mother is significantly increased. Therefore immunoprophylaxis with anti-D-immunoglobins should be performed in all these cases.
