Rapid Ultrasound for Shock and Hypotension: A Clinical Update for the Advanced Practice Provider: Part 2.

The Rapid Ultrasound for Shock and Hypotension (RUSH) examination is used for patients with hypotension without clear cause or undifferentiated hypotension. In the emergency department setting, clinicians may perform the RUSH examination to supplement the physical assessment and differentiate the diagnosis of hypovolemic, obstructive, cardiogenic, and distributive forms of shock. The key elements of the RUSH examination are the pump, tank, and pipes, meaning potentially causes of the hypotension are examined within the heart, vascular volume and integrity, and the vessels themselves. Anesthetic Management of Elderly Patients With Down Syndrome: A Case Report.

In 1930, the life expectancy of patients with Down syndrome was about 10 years; today, their life expectancy is more than 60 years. With aging, there is an increased need for anesthesia and surgery. There is, however, no published information regarding the anesthetic management of older adults with Down syndrome. In this report, we described the anesthetic management of a 50-year-old woman with Down syndrome undergoing major cervical spine surgery. Components of the anesthetic that we thought would be difficult such as intravenous line placement and endotracheal intubation were accomplished wi

X-STOP surgical implant for the treatment of lumbar spinal stenosis: clinical practice recommendations for neurosurgical nurse practitioners.

Lumbar spinal stenosis is a common condition that affects the aging population because of the natural degenerative changes the spine undergoes during the aging process. The symptoms cause decreased functionality and quality of life. Traditional surgical treatment has been a decompressive laminectomy with or without a fusion. However, a newer less-invasive surgical technique called X-STOP interspinous process device is available for the treatment of neurogenic intermittent claudication because of lumbar spinal stenosis. The main goal of this procedure is to allow the patient to restore function

Alpha1-antitrypsin deficiency: incidence and implications.

Alpha1-antitrypsin (AAT) deficiency is an underrecognized inherited disorder with pulmonary and hepatic implications for both adults and neonates. Clinical expressions of AAT deficiency are seen in the lung, liver, and the skin, with considerable variability in the severity of clinical disease. AAT deficiency accounts for nearly 3% of all cases of chronic obstructive pulmonary disease and is responsible for early-onset emphysema in nonsmokers. Ten to twenty percent of affected neonates develop significant liver disease. Panniculitis, a rare skin complication of AAT deficiency, is characterized

Quality issues concerning genetic counselling for presymptomatic testing: a European Delphi study.

Genetic counselling for presymptomatic testing is complex, bringing both ethical and practical questions. There are protocols for counselling but a scarcity of literature regarding quality assessment of such counselling practice. Generic quality assessment tools for genetic services are not specific to presymptomatic testing (PST). Therefore, the aim of this study was to identify aspects of effective counselling practice in PST for late-onset neurological disorders. We used the Delphi method to ascertain the views of relevant European experts in genetic counselling practice, ascertained via pu

Skin Cancer in Residential Care Facilities: A Scoping Review of Risk, Diagnosis, Psychosocial Impact, and Management.

The older adult population is rising, resulting in a larger number of adults residing in nursing homes. Skin cancer is most commonly diagnosed in older adults. Yet, these individuals often face limited access to advanced dermatologic care, in addition to unique risk factors such as inadequate sun protection and reduced mobility. This review aims to map the current evidence on skin cancer risk and diagnosis, as well as prevention strategies among older adults residing in nursing homes. A search was conducted on PubMed using the algorithm: ((nursing home) OR (assisted living) OR (elderly living

The Symptom Experience of Patients with Advanced Melanoma Undergoing Immune Checkpoint Inhibitor (ICI) Therapy.

OBJECTIVES: The advent of immune checkpoint inhibitor (ICI) therapy has vastly improved outcomes for patients with advanced melanoma. However, the symptom burden and intensity with their impact on quality-of-life (HRQoL) and functionality are heterogeneous and unpredictable. We used descriptive exploratory content analysis from interviews to capture the patient experience after they had completed quantitative data collection of their symptom burden and interference with the use of two patient-reported outcome (PRO) instruments. DATA SOURCES: Participants from a single center with advanced mel

Imprinted Arousal Pattern (IAP): A Transdiagnostic Clinical Reasoning for Compulsive Behaviors.

OBJECTIVE: Compulsive behaviors across psychiatric diagnoses often recur despite diagnostic clarity, pharmacologic stabilization, and structured psychotherapy. Substance use disorders and behavioral addictions demonstrate high relapse rates, particularly under stress, while many patients describe not a pursuit of pleasure but an inability to tolerate stillness, boredom, or emotional deadness. Although the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, Text Revision (DSM-5-TR) provides descriptive reliability and the Research Domain Criteria (RDoC) framework advances dime

Humanizing care in the ICU: how system pressures shape interacting patterns of hurry, blur, burden, and cruelty.

BACKGROUND: Intensive care units are high-acuity environments characterized by time pressure, medical complexity, and organizational demands. These conditions may influence clinicians' ability to provide patient-centered care and contribute to forms of care perceived as fragmented or dehumanizing. OBJECTIVE: To examine how ICU clinicians describe and make sense of the pressures shaping everyday critical care practice, and how these pressures give rise to interacting patterns of care, including those described as hurry, blur, burden, and cruelty. METHODS: We conducted a qualitative study usin

[Implementation and development of the APN role in primary health care in Germany: A qualitative study].

Implementation and development of the APN role in primary health care in Germany: A qualitative study Abstract: Background: An increasing number of patients with multimorbidity and associated special care needs are facing a declining number of general physicians. This calls for new approaches. Internationally, Advanced Practice Nurses (APNs) are successfully deployed in primary care of multimorbid patients. In Germany, this practice has not yet been established. Successful implementation requires, among other things, specific support. Aim: The aim of the study is to examine the APN perspective

An unintended path: Foley catheter-induced biliary obstruction following gastrostomy tube replacement - A case report.

Percutaneous endoscopic gastrostomy (PEG) tubes are widely used for long-term enteral access. Accidental dislodgement is a relatively common occurrence, and in many settings a Foley catheter is inserted as a temporary measure to preserve tract patency until definitive replacement can be performed. Although this practice is routinely employed, Foley catheters lack an external flange and are therefore susceptible to distal migration and associated complications. We describe a case of a 97-year-old woman with advanced dementia who presented to the emergency department with recurrent vomiting and

Integrative Review of Co-Occurring Symptoms Across Etiologies of Chronic Liver Disease and Implications for Symptom Management Research and Practice.

PURPOSE: Chronic liver disease affects over 30 million people worldwide, and despite advances in medical management that have increased survival, a majority of these individuals report multiple symptoms that severely impair function and quality of life. The purpose of this integrative review was to examine and summarize the current literature focused on self-reported symptoms in individuals with chronic liver disease in order to inform the development and delivery of symptom management strategies in clinical practice. DESIGN AND METHODS: Combinations of search terms related to the symptom exp

A scoping review of "nursing-technology collaboration" in home-based rehabilitation for the elderly under the long-term care insurance system.

BACKGROUND: Under the long-term care insurance system, home-based rehabilitation is a key service model for addressing population aging, but it faces the dual challenges of a shortage of care workers and increasingly complex service needs. The introduction of emerging tools, such as digital health technologies and social assistive robots, has opened up new possibilities for home-based rehabilitation. However, whether technology can truly improve care outcomes depends on the collaborative working model between nurses and technology-known as "nurse-technology collaboration." There is currently a

Proceedings of the United Kingdom Periprosthetic Joint Infection Meeting 2022: Combined and Arthroplasty Sessions.

Considerable variation in practice exists in the prevention, diagnosis, and treatment of periprosthetic joint infection (PJI), which is a devastating complication for patients and surgeons. The consensus principle has been increasingly embraced by the orthopaedic community to help guide practice, especially where high-level evidence remains unavailable. The third United Kingdom Periprosthetic Joint Infection (UK PJI) Meeting was held in Glasgow on April 1, 2022, with more than 180 delegates in attendance, representing orthopaedics, microbiology, infectious diseases, plastic surgery, anesthetic

Lung Ultrasound Guided Management of Pulmonary Congestion in Hemodialysis: A Multicenter Randomized Controlled Trial.

KEY POINTS: A simplified lung ultrasound (LUS)–guided management approach safely reduced pulmonary congestion in patients undergoing hemodialysis. LUS may be performed by nurses and paramedics, making it even easier to integrate it into routine practice. LUS may contribute to better BP control in patients undergoing hemodialysis. BACKGROUND: Pulmonary congestion (PC) is frequent in hemodialysis patients and is associated with deleterious prognosis. Lung ultrasound (LUS) accurately quantifies PC; however, many methods exist to evaluate PC using LUS. Here, we validate a simplified LUS-guided pr

Guidelines for Quality Management of Apallic Syndrome / Vegetative State.

INTRODUCTION: Epidemiology in Europe shows constantly increasing figures for the apallic syndrome (AS)/vegetative state (VS) as a consequence of advanced rescue, emergency services, intensive care treatment after acute brain damage and high-standard activating home nursing for completely dependent end-stage cases secondary to progressive neurological disease. Management of patients in irreversible permanent AS/VS has been the subject of sustained scientific and moral-legal debate over the past decade. METHODS: A task force on guidelines for quality management of AS/VS was set up under the aus

Ethics of Care in Nursing: Protocol for a Qualitative Systematic Review and Best-Fit Framework Synthesis.

BACKGROUND: Ethics of care lies at the heart of the nursing profession. Grounded in the work of Gilligan and Tronto, it emphasizes relationships and responsiveness to the needs of those receiving care. Although Tronto's framework specifies 5 elements of care, no systematic synthesis has yet examined how each element is expressed through observable behavioral indicators in the nursing context. OBJECTIVE: This review aims to identify, appraise, and synthesize the components and indicators of the ethics of care in the nursing profession, drawing on the frameworks of Gilligan and Tronto. It addre
