The long-term psychiatric consequences of accidental injury. A longitudinal study of 107 adults.

One hundred and seven accidentally injured adults were studied while in hospital and assessed prospectively twice more in a mean period of 28 months. The patients were studied by means of taped clinical interviews, including the Comprehensive Psychopathological Rating Scale (which includes the Montgomery-Asberg Depression Rating Scale), and several self-report measures of distress (Schedule of Recent Life Events, General Health Questionnaire, Impact of Event Scale and State Anxiety Inventory) at the three assessments. The total incidence of psychiatric disorders considered to be caused by the

Antiepileptic drugs and psychiatric disorders: mechanism involved in manifestation of psychic symptoms of high blood level of antiepileptics.

The adverse psychic effects of antiepileptics embrace all categories of psychiatric symptomatology, including disturbances of consciousness (delirium, confusion), psychotic state (schizophrenia-like psychosis, affective disorder), neurotic state, behavior and character disorder. Antiepileptic intoxication can take the form of a psychotic episode. The lowered level of consciousness due to a high blood level of antiepileptics is expressed as inhibitory symptoms such as a lack of initiative, psychomotor slowing, lowering mood, stuporous state and the like. Another group of manifestation of a high

Factors predicting re-hospitalization for inpatients with bipolar mania--A naturalistic cohort.

Patients with bipolar mania often require re-hospitalization, which can be a social-economic burden. This study aimed to investigate the factors affecting time to re-hospitalization for inpatients with acute bipolar mania. In this 6-year naturalistic study we reviewed the charts of inpatients with acute bipolar mania. Potential dichotomous factors including sex, family history of affective disorder, suicide history, and discharge against medical advice, involuntary admission, psychotic features, substance use disorder, and remission at discharge were recorded. Continuous variables including ag

Catatonia in first-episode psychosis: prevalence and psychopathological association.

BACKGROUND: First-episode psychosis (FEP) is a critical phase in psychotic disorders where early intervention significantly influences long-term outcomes. Catatonia, characterised by motor, behavioural, and psychological abnormalities, is an under-recognised aspect of FEP. AIMS: This study examines catatonia prevalence in affective and non-affective FEP, its role as a severity indicator across psychopathological domains, its correlations with other symptoms and its association with clinical syndromes. METHOD: A cross-sectional study was conducted with 58 FEP patients (38 females, 20 males) a

Mood congruence phenomenon in acutely symptomatic mania bipolar I disorder patients with and without psychotic symptoms.

INTRODUCTION: Bipolar disorder causes substantial morbidity including cognitive impairment. The objective of the study was to evaluate memory performance of acutely mania bipolar I disorder (BD-I) patients with and without psychosis. We also aimed to assess the mood congruence phenomenon upon memory. METHODS: A cross-sectional study was developed with BD-I patients (19 with, and 12 without psychotic symptoms), and 27 age- and education-paired healthy controls. Memory tests were selected to evaluate memory/attention performance. A verbal episodic memory task with affective content (word span)

First-episode psychosis and vocational outcomes: A predictive model.

Most studies on predictors of vocational outcomes are cross-sectional and results are varied. This study aimed to examine the vocational rates of patients with first-episode psychosis (FEP), identify factors predicting a lack of engagement in age-appropriate roles, and evaluate the predictive ability of a model with baseline sociodemographic information and 2-year symptom and functioning trajectories on vocational outcomes. The Singapore Early Psychosis Intervention Program (EPIP) has maintained a standing database on patient clinico-demographic information. The primary outcome, vocational sta

Hypomanic symptoms and first manic episode during ketogenic diet treatment in a patient taking venlafaxine: a case report with longitudinal glucose-ketone index and clinical monitoring.

BACKGROUND: The ketogenic diet (KD) is gaining increasing interest as a metabolic intervention for neuropsychiatric disorders, including mood disorders. However, its potential role in clinically relevant affective changes, such as hypomania or mania, remains insufficiently characterized. METHODS: We present the case of a woman with a previous history of major depressive episode with psychotic symptoms and a posterior history of recurrent episodes of mild depression, who never achieved complete symptom remission. In an effort to ameliorate her chronic depressive symptoms, the patient started a

Mood disorder with mixed, psychotic features due to vitamin b12 deficiency in an adolescent: case report.

Vitamin B12 is one of the essential vitamins affecting various systems of the body. Reports of psychiatric disorders due to its deficiency mostly focus on middle aged and elderly patients. Here we report a case of vitamin B 12 deficiency in a 16-year old, male adolescent who presented with mixed mood disorder symptoms with psychotic features. Chief complaints were "irritability, regressive behavior, apathy, crying and truancy" which lasted for a year. Premorbid personality was unremarkable with no substance use/exposure or infections. No stressors were present. The patient was not vegetarian.

Polygenic risk scores in bipolar disorder subgroups.

BACKGROUND: Bipolar disorder (BD) is a genetically and clinically heterogeneous disorder. Current classifications of BD rely on clinical presentations without any validating biomarkers, making homogenous and valid subtypes warranted. This study aims at investigating whether a BD polygenic risk score (PGRS) can validate BD subtypes including diagnostic sub-categories (BD-I versus BD-II), patients with and without psychotic symptoms, polarity of first presenting episode and age at onset based groups. We also wanted to investigate whether illness severity indicators were associated with a higher

Epilepsy, mental health disorder, or both?

Temporal lobe epilepsy (TLE), a subset of the seizure disorder family, represents a complex neuropsychiatric illness, where the neurological presentation may be complemented by varying severity of affective, behavioral, psychotic, or personality abnormalities, which, in turn, may not only lead to misdiagnosis, but also affect the management. This paper outlines a spectrum of mental health presentations, including psychosis, mood, anxiety, panic, and dissociative states, associated with epilepsy that make the correct diagnosis a challenge.

Effectiveness and safety of long-acting aripiprazole 960 mg: preliminary findings from a prospective real-world observational study.

Poor treatment adherence is a major challenge in the management of severe mental disorders, as it is associated with an increased risk of relapse, rehospitalization, symptom worsening, treatment resistance, and all-cause mortality. In 2023 a 2-month 960 mg formulation of aripiprazole 2 months ready-to-use (Ari 2MRTU) was introduced. Compared with monthly long-acting injectable (LAI) drugs, Ari 2MRTU may improve adherence and patient satisfaction by reducing the frequency of healthcare visits. The aim of this prospective longitudinal study was to provide preliminary real-world data on the effec

Use of ECT in patients with organic catatonia.

BACKGROUND: Electroconvulsive therapy (ECT) is considered to be a treatment of choice in patients with catatonia, who do not respond to lorazepam, irrespective of the underlying aetiology. Although, significant data is available for successful use of ECT in patients with catatonia secondary to affective and psychotic disorders, little information is available for use of ECT in organic catatonia. AIM: To assess demographic and clinical profile of patients with organic catatonia receiving ECT. METHODOLOGY: Using a retrospective study design, ECT register of the department was reviewed for the

Multivariate patterns of disrupted sleep longitudinally predict affective vulnerability to psychosis in 22q11.2 Deletion Syndrome.

22q11.2 deletion syndrome (22q11DS) contributes dramatically to increased genetic risk for psychopathology, and in particular schizophrenia. Sleep disorders, including obstructive sleep apnea (OSA), are also highly prevalent, making 22q11DS a unique model to explore their impact on psychosis vulnerability. Still, the contribution of sleep disturbances to psychosis vulnerability remains unclear. We characterized the sleep phenotype of 69 individuals with 22q11DS and 38 healthy controls with actigraphy and sleep questionnaires. Psychiatric symptoms were measured concomitantly with the baseline s

Higher concentration of interleukin 6 - A possible link between major depressive disorder and childhood abuse.

Little is known about the correlation between IL-6 and childhood abuse and neglect which may be risk factors for the development of affective disorders in adulthood. The aim of this study was to analyze differences in serum concentrations of IL-6 between patients with major depressive disorder and healthy controls, and to investigate possible correlations with adverse childhood experiences. Peripheral venous blood samples were obtained from 64 patients who fulfilled DSM-IV-R criteria for a current major depressive episode without psychotic symptoms (MDD) and 53 healthy controls, matched for ag

Efficacy of moclobemide in different patient groups: a meta-analysis of studies.

Whilst tricyclic antidepressants are efficacious in all depressive syndromes, classical MAO-inhibitors differ substantially from them in their action. They are considered less effective in general and not very effective in endogenous depression, but recommended for the treatment of 'atypical' depression. A new class of RIMA (Reversible Inhibitors of MAO-A) represented by moclobemide requires a change in clinical thinking on antidepressants. Moclobemide shows the same efficacy in depression as tricyclics: its effects are similar in unipolar and bipolar affective disorders, and in patients with

Differences in the ICD-10 diagnostic subtype of depression in bipolar disorder compared to recurrent depressive disorder.

BACKGROUND: The aim of the study was to investigate whether patients with bipolar depression and patients with recurrent depressive disorder present with different subtypes of depressive episode as according to ICD-10. SAMPLING AND METHODS: All patients who got a diagnosis of bipolar affective disorder, current episode of depression, or a diagnosis of recurrent depressive disorder, current episode of depression, in a period from 1994 to 2002 at the first outpatient treatment or at the first discharge from psychiatric hospitalization in Denmark were identified in a nationwide register. The dually diagnosed patient with psychotic symptoms.

The dual diagnoses of substance use disorder with other psychiatric disorder is especially problematic in psychotic or other chronically mentally ill patients. Such patients have a more fragile mental status which can be adversely affected by psychoactive substances of abuse. In addition, most such patients need to take potent psychiatric medications which themselves may interact with substances. This article reviews substance induced versus true comorbid major psychiatric disorders and discusses the major classes of psychiatric medications in terms of abuse potential and their abilities to ei
