Childhood trauma and personality disorder in 43 women with panic disorder.

Among 43 women with panic disorder (38 with agoraphobia), 23 reported histories of childhood sexual abuse associated with high incidence of other early adverse experiences. The finding that subjects reporting childhood sexual and physical abuse were more likely to receive an Axis II diagnosis than those who did not confirms prior work.

Anxiety disorders and substance use disorders: different associations by anxiety disorder.

Anxiety disorders (ADs) and substance use disorders (SUDs) often occur together, but the strength of this association and their apparent order of onset differ across studies. The goals of this study were to examine: (1) which ADs were associated with which SUDs, and (2) among people who experienced both an AD and a SUD, which disorder had an earlier onset. Lifetime diagnoses from the National Comorbidity Survey-Replication (n=9282) were used. Social phobia, generalized anxiety disorder, panic disorder, and agoraphobia were positively associated with all SUDs. Among people with both an AD and a…

Agoraphobic cognitions in old and young patients with panic disorder.

OBJECTIVES: To compare the agoraphobic cognitions of younger and older patients suffering from panic disorder with agoraphobia by means of existing questionnaires. METHOD: Agoraphobic cognitions were assessed using the Agoraphobic Cognitions Questionnaire (ACQ) in 205 patients confirmed with confirmed panic disorder with agoraphobia (Diagnostic and Statistical Manual, Fourth Edition, Text Revision) and analyzed at the item level applying a Bonferroni correction. RESULTS: The 48 patients who were older than 60 years had a significantly lower mean (SD) ACQ total score than their younger counte…

Open-label support for duloxetine for the treatment of panic disorder.

Panic disorder with or without agoraphobia is a common, often chronic and refractory anxiety disorder. Although a number of pharmacotherapies are now indicated for panic disorder, many patients do not respond to available interventions. We hypothesized that duloxetine, a serotonin-norepinephrine reuptake inhibitor (SNRI) that has greater initial noradrenergic effects than venlafaxine, would have broad efficacy for individuals with panic disorder. Fifteen individuals with panic disorder with or without agoraphobia received 8 weeks of open label duloxetine flexibly dosed from 60 to 120 mg per da…

Effect of maternal panic disorder on mother-child interaction and relation to child anxiety and child self-efficacy.

To determine whether mothers with panic disorder with or without agoraphobia interacted differently with their children than normal control mothers, 86 mothers and their adolescents (aged between 13 and 23 years) were observed during a structured play situation. Maternal as well as adolescent anxiety status was assessed according to a structured diagnostic interview. Results showed that mothers with panic disorder/agoraphobia showed more verbal control, were more criticizing and less sensitive during mother-child interaction than mothers without current mental disorders. Moreover, more conflic…

[The peculiarities of panic disorders determined by gender dimorphism].

In contrast to panic disorder (PD) epidemiology, gender-related etiological and clinical PD peculiarities are not studied well so far. Seventy-five PD patients (27 male, 48 female) with and without agoraphobia diagnosed according to ICD-10 criteria have been studied. No differences were found by gender for the key clinical signs: interrelations in parent family, age at debut, panic attacks frequency, combination with agoraphobia, social maladaptation and response to antidepressant therapy. Along with common features, we revealed gender differences in the clinical picture. In women, neuroendocr…

The N200 abnormalities of auditory event-related potentials in patients with panic disorder.

Cognitive impairment in patients with panic disorder (PD) has been studied using event-related potentials (ERPs). However, previous studies obtained ERP data only at a few scalp sites, and seldom investigated N200 measurements from the difference waveforms. In the present study, auditory ERPs were recorded at 16 scalp sites during an active discrimination task of oddball paradigm. Fourteen PD patients (8 with agoraphobia; 6 without agoraphobia) were compared with 14 sex- and age-matched control subjects. For the nontarget waveforms, P2 amplitude was reduced in PD patients. For the target wavef…

The phenomenological study of 90 patients with panic disorder, Part II.

This paper examines the nosological and aetiological relationships of panic disorder to the anxiety states and depression. The phenomenology is detailed from an unbiased sample of 90 cases selected, on the basis of meeting positive criteria for panic disorder, from 3 series of consecutive cases. Panic attacks were found to be only quantitatively distinct from non-panic anxiety. Truly spontaneous attacks, not preceded by anxiety-provoking cognitions, were uncommon. No unique association with agoraphobia was seen, other anxiety states and depression being common. Social phobia and generalized an…

Treatment-resistant panic disorder.

A substantial number of patients with panic disorder and agoraphobia may remain symptomatic after standard treatment (including selective serotonin reuptake inhibitors, tricyclic antidepressants, benzodiazepines, or irreversible monamine oxidase inhibitors). In this review, recommendations for the treatment of patients with panic disorder and agoraphobia who do not respond to these drugs are provided. Nonresponse to drug treatment could be defined as a failure to achieve a 50% reduction on a standard rating scale after a minimum of 6 weeks of treatment in adequate dose. When initial treatments…

Transporting an empirically supported treatment for panic disorder to a service clinic setting: a benchmarking strategy.

This work examines the transportability of cognitive-behavioral therapy (CBT) for panic disorder to a community mental health center (CMHC) setting by comparing CMHC treatment outcome data with the results obtained in two controlled efficacy trials. Participants were 110 clients with a primary diagnosis of panic disorder with or without agoraphobia; clients were not excluded on the basis of medication use or changes, severity or frequency of panic attacks, age, or the presence of agoraphobia. Clients completed a 15-session CBT protocol. Despite differences in settings, clients, and treatment p…

Alcoholism and phobic anxiety--a clinical-demographic comparison.

The prevalence of anxious and phobic symptoms in 97 alcohol-dependent and alcohol abuse patients, and that of alcohol abuse and dependence in 90 patients with panic/agoraphobia (PAG), were respectively determined in out-patients attending alcoholism and anxiety clinics in a university psychiatric hospital. The clinical and socio-demographic data of both the phobics and non-phobics of the alcoholic sample, and the alcohol dependents and non-dependent of the PAG sample, were compared. Panic attacks and phobias were associated with increased severity and worse prognosis for alcoholism. The infreq…

[Possibilities in therapy of anxiety disorders in DSM-III-R].

Between 2% and 5% of the population are affected by anxiety disorders. Several studies have shown that imipramine possesses anti-panic and anti-phobic properties. Its response rate is at least 70%. In panic disorder with agoraphobia it is therefore the medication of first choice. Agoraphobia, simple and social phobias are highly responsive to behavioural therapy (desensitization, flooding, social skills training, cognitive restructuring). Drugs have not been found useful in controlled studies. In conjunction with the tricyclic antidepressant clomipramine, behavioural therapy is superior to oth…

Utility of fear survey schedule with Australian samples of anxiety disorder patients.

The Fear Survey Schedule (FSS) has been widely used in clinical settings for the assessment of anxiety disorders yet its reliability and validity is still in doubt. This paper reports the factor structure and the validity of the FSS using 130 agoraphobics with panic attacks, 98 phobics and 108 non-patient controls. The results show that a clear 6-factor solution can be derived from the FSS and this accounts for 46% of the variance. The 6 factors are (1) agoraphobia, (2) fear of small animals, (3) social anxiety, (4) negative social evaluation fears, (5) fear of bodily harm and (6) injury and f…

The use of the anxiety meter to reduce anxiety.

The use of the Anxiety Meter (AM) is taught to clients to reduce anxiety. At first the AM is used as part of the self-control triad. It is then used alone to reduce anxiety. A rationale for the use of the AM is presented together with a description of the procedure. The AM is a self-control strategy for use in treating agoraphobia and panic reaction. A case example is also presented.
