A family of squints: the differing effects of hypermetropia on four siblings.

Four siblings in a family of five were found to be affected with hypermetropia in differing degrees and with differing effects. These were unilateral hypermetropia, producing amblyopia; hypermetropia inducing an accommodative esotropia with minimal amblyopia and fully controlled with glasses; hypermetropia producing an accommodative esotropia with a high AC/A ratio, fully controlled with bifocals; and hypermetropia producing esotropia which was only partially corrected with glasses, therefore partially accommodative, with a residual squint which required surgery. This family of four nicely dem…

Loss and recovery of neurofilament in the cat lateral geniculate nucleus following monocular retinal inactivation.

BACKGROUND: Postnatal development of the mammalian primary visual pathway occurs early in life and is guided by visually-driven afferent activity. Even a short duration of disrupted binocular vision can derail development of neural connections and produce a lasting monocular visual impairment, amblyopia. Temporary inactivation of the dominant retina with tetrodotoxin has emerged as a potential treatment for amblyopia that has exhibited superior potency compared to mainstay therapy in animal models. Notwithstanding its promise as a treatment for amblyopia, little is known about the impact of re…

Investigation of Refractive Errors in Congenital Nasolacrimal Duct Obstruction.

OBJECTIVES: Congenital nasolacrimal duct obstruction (CNLDO) is a condition that can lead to the development of anisometropia and amblyopia. This study aimed to identify refractive errors in children with unilateral and bilateral CNLDO. METHODS: A total of 220 eyes from 110 patients with CNLDO were retrospectively analyzed in this study. All patients underwent at least one surgery, and their most recent refractive errors were compared based on the laterality of the disease. The risk factors for amblyopia were established in accordance with the 2013 and 2021 American Association for Pediatric …

Using visual noise to characterize amblyopic letter identification.

Amblyopia is a much-studied but poorly understood developmental visual disorder that reduces acuity, profoundly reducing contrast sensitivity for small targets. Here we use visual noise to probe the letter identification process and characterize its impairment by amblyopia. We apply five levels of analysis - threshold, threshold in noise, equivalent noise, optical MTF, and noise modeling - to obtain a two-factor model of the amblyopic deficit: substantially reduced efficiency for small letters and negligibly increased cortical noise. Cortical noise, expressed as an equivalent input noise, vari…

Ophthalmic screening of 38,000 children, age 1 to 2 1/2 years, in child welfare clinics.

Screening of 38,000 infants, age 1 to 2 1/2 years, showed a prevalence of 1.3% of strabismus and 0.5% of strabismic amblyopia. Esotropia was more than three times as frequent as exotropia. Approximately half of the cases with esotropia were amblyopic. Eighty-one percent of the cases with exotropia were intermittent, and in 29% the V-pattern was found. Significant ametropia was found in over 50% of the cases with strabismus. Although hypermetropia was the most frequent refraction in children with esotropia, myopia was a frequent finding in both esotropia and exotropia. Anisometroia was particul…

Toxic optic neuropathy.

Toxic optic neuropathy is a complex, multifactorial disease potentially affecting individuals of all ages, races, places, and economic strata. Etiology includes nutritional, environmental, toxicologic, and genetic factors. Most cases of nutritional amblyopia are encountered in disadvantaged countries. However, toxic amblyopia related to drug treatment or alcohol abuse is also encountered in the Western world. Typically, toxic and nutritional optic neuropathy is progressive, with bilateral symmetrical painless visual loss causing central or cecocentral scotoma. There is no specific treatment fo…

BACKGROUND: Congenital cataracts have historically been a major cause of visual impairment in children. Their treatment improved once deprivation amblyopia became better understood. Many children born with congenital cataracts are now able to develop good functional levels of visual acuity. Early detection and surgical removal of the cataracts are only the first steps in the treatment. RESULTS: The optical correction of the resulting aphakia, usually with contact lenses, and the long term treatment of amblyopia when required are invariably more difficult. The pediatric optometrist is particul…
