Combined local-propofol anesthesia with noninvasive positive pressure ventilation in a vasectomy patient with sleep apnea syndrome.

A vasectomy patient with sleep apnea syndrome, who requested general anesthesia and day surgery, was given sole propofol infusion with airway maintenance by noninvasive positive-pressure ventilation. Surgery was performed with additional local anesthesia. Because this patient was treated successfully, this anesthetic technique may also be applicable to other normal patients.

MULTIMODAL TAP BLOCK WITH KETOPROFEN VERSUS EPIDURAL ANALGESIA AFTER LAPAROSCOPIC AND OPEN ABDOMINAL SURGERY: A RANDOMIZED STUDY.

BACKGROUND/AIM: Effective analgesia is imperative to cover post-operative pain after laparoscopic surgery and laparotomy. While systemic opioid drugs are often used for this purpose, the decrease in opioid administration is highly desirable and the multimodal forms of post-operative anesthesia are preferable. The present study aimed to assess the effectiveness of the TAP block either alone or in combination with systemic NSAID administration compared to epidural anesthesia following laparoscopic surgery and laparotomy. MATERIALS AND METHODS: 180 patients subjected to laparoscopic surgery or l

Isoflurane inhibits hypoxic pulmonary vasoconstriction. An in vivo fluorescence microscopic study in rabbits.

BACKGROUND: Contradictory results have been reported in previous studies investigating the effect of isoflurane on hypoxic pulmonary vasoconstriction by indirect approaches. The current study measured the effects of one-lung ventilation (1LV) and isoflurane 1.5% by direct visual observation of the pulmonary microcirculation. METHODS: Ten New Zealand White rabbits were anesthetized with intravenous thiopental, alpha-chloralose, and piritramid. Arterial, central venous, pulmonary arterial, left atrial, and airway pressures and cardiac output were recorded continuously. 1LV was facilitated by a

Low-dose rocuronium as an adjuvant to remifentanil for laryngeal mask airway-assisted flexible fiberoptic bronchoscopy in children: protocol for a randomized placebo-controlled trial.

BACKGROUND: Flexible fiberoptic bronchoscopy is being increasingly carried out under general anesthesia in pediatric patients. However, the optimal anesthetic protocol still remains unclear. This study aims to test the hypothesis that the addition of low-dose rocuronium during laryngeal mask airway-assisted flexible fiberoptic bronchoscopy in children can enhance procedural outcomes and improve overall management. METHODS: In this randomized, double-blind, placebo-controlled trial, 140 children aged 1-6 years scheduled for flexible fiberoptic bronchoscopy under general anesthesia will be rand

Severe lightning pain after subarachnoid block in a patient with neuropathic pain of central origin: which drug is best to treat the pain?

OBJECTIVE: There have been many reports that spinal anesthesia induces severe lightning pain in the lower limbs of patients with phantom limb pain, tabes dorsalis, or causalgia. We report on a patient with neuropathic pain of central origin who showed newly developed severe lightning pain after therapeutic subarachnoid block (SAB). We performed SAB 16 times in this patient, and he complained of severe pain each time. We investigated which drug was best for treating such induced pain by administering various drugs to the patient. SETTING: The patient was hospitalized for treatment and observat

A pharmacodynamic analysis of factors affecting recovery from anesthesia with propofol-remifentanil target controlled infusion.

AIM: To examine individual patient's demographic parameters and clinical variables related to return of consciousness (ROC) and the pharmacodynamic relationship between propofol effect-site concentration (C(e)) and ROC from propofol-remifentanil anesthesia. METHODS: Ninety-four patients received propofol-remifentanil anesthesia using the effect-site target-controlled infusion (TCI) system. All clinical events were noted, and variables possibly related to propofol C(e) at ROC were examined using linear correlation analyses. Pharmacodynamic modeling incorporating covariates was performed using

Pharmacological Efficacy of Intravenous Magnesium in Attenuating Remifentanil-Induced Postoperative Hyperalgesia: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

Background/Objectives: Remifentanil-based anesthesia is linked to opioid-induced hyperalgesia (OIH), increasing postoperative pain and analgesic requirements. Magnesium, an N-methyl-D-aspartate (NMDA) receptor antagonist, might alleviate OIH. We aimed to assess whether intravenous magnesium reduces postoperative pain, analgesic requirements, and hyperalgesia in adults receiving remifentanil-based anesthesia. Methods: We searched PubMed, Embase, the Cochrane Library, and Web of Science (1 December 2024) for randomized controlled trials (RCTs) comparing intravenous magnesium vs. placebo. Risk of

Preventive intramuscular phenylephrine in elective cesarean section under spinal anesthesia: A randomized controlled trial.

BACKGROUND: Phenylephrine is the first-line vasoactive drug in the cesarean section under spinal anesthesia. The rate of hypotension remains high after intravenous preventive use of phenylephrine. However, few studies have investigated the effect of preventive intramuscular phenylephrine via a longer period of usage on fetal and maternal outcomes. METHODS: A total of 99 healthy parturients undergoing elective cesarean delivery were randomly allocated into three groups: M group (preventive intramuscular use of 5 mg phenylephrine), V group (preventive intravenous use of 100 μg phenylephrine), a

The Impact of Anaesthesia Method on the Use of Anaesthesia Department Resources in Total Knee Arthroplasty.

BACKGROUND: Spinal and general anaesthesia seem to lead to similar health-related outcomes in total knee arthroplasty. However, to produce economically and environmentally sustainable health care, we should also consider other factors. Current guidelines recommend preferring regional anaesthesia and total intravenous anaesthesia to reduce environmental impact. In this study, we analysed time, costs, and disposable materials associated with spinal anaesthesia and total intravenous anaesthesia during total knee arthroplasty. METHODS: A total of 395 patients undergoing total knee arthroplasty we

Safe and effective IV regional anesthesia for use in the emergency department.

The mini-dose Bier block, a technique of intravenous (IV) regional anesthesia that uses low-dose lidocaine and provides safe and effective anesthesia for outpatient closed reductions of upper extremity fractures and dislocations, is presented. This procedure was evaluated in two hospital emergency departments in 105 patients (ages 2 to 86). Ninety-five percent achieved adequate anesthesia (minimal or no pain on closed reduction). No significant complications were noted. Full neurologic function returned in ten minutes in all cases. Both patient and physician satisfaction with the procedure wer

Propofol, the newest induction agent of anesthesia.

Propofol is a rapidly acting intravenous anesthetic agent which has many advantageous kinetic properties explaining its usefulness by bolus dose for induction of anesthesia or for administration by continuous intravenous infusion. It is rapidly distributed in the body with a half-life of only around 2 min and has an efficient hepatic and extrahepatic clearance (total body clearance may exceed liver blood flow). Premedication has little effect on the already good induction characteristics of propofol. The incidence of cardiorespiratory depression appears to be higher than that of other inductio

Euthanasia of mouse fetuses and neonates.

We sought to determine whether any of the common methods of euthanasia for adult rodents would lead to an acceptable death for fetuses or neonates. We wanted to identify a method that was rapid, free of signs of pain or distress, reliable, and minimally distressful to the person performing the procedure and that minimized the amount of handling required to perform the procedure. We evaluated six methods of euthanasia, with and without anesthesia, in three age groups of mice: gravid mice (E14-20) and neonatal pups (P1-P7 and P8-P14). Euthanasia methods included: halothane inhalation, carbon dio
