MR arthrography has become an important tool for the assessment of a variety of ankle disorders. MR arthrography may facilitate the evaluation of patients with suspected intra-articular pathology in whom conventional MR imaging is not sufficient for an adequate diagnosis and be useful for therapy planning. Thirty-six consecutive ankle arthroscopies from 1991 to 1996 were reviewed. Indications for arthroscopy were loose bodies, osteochondritis dissecans, and post traumatic and degenerative ankle joints. An alternative to traditional lateral ankle stabilization procedures is presented using freeze-dried fascia lata graft tissue. This procedure is minimally invasive and obviates the complications typically associated with tenodesis procedures. Fifty-five ankle arthroscopies performed for anterolateral ankle impingement between January 1987 and April 1992 were reviewed for functional outcome and patient satisfaction. There were 33 men and 22 women with an average age of 34 years (range 20-67). All patients reported a previous history of ankle injury, and 60% of the patients had at least one ankle sprain. At an average follow up of 2.6 years (range 1.0-4.5), 87% of the patients had good/excellent results, 98% were satisfied, and 84% returned to their previous sports.

We reviewed all ankle fractures over a 6-year period and selected those with greater than 33 per cent subluxation of the talus to assess the early management and the incidence of skin complications. A total of 60 fractures were identified; none were reduced as an emergency procedure on admission to the accident department and seven developed ischaemic necrosis over the medial skin incision. We suggest that early reduction in the accident and emergency department may help to prevent this complication.

Sarcoidosis has been described as a multisystem granulomatous disorder of unknown etiology that usually affects young adults. It can affect the foot and ankle. When erythema nodosum is present, the sarcoidosis usually is characterized by bilateral hilar lymphadenopathy, uveitis, fever, arthralgia, or arthritis. This symptom complex is referred to as Löfgren's syndrome. The author describes a patient who presented with bilateral medial ankle pain secondary to sarcoidosis.

Follow-up checks were performed on 106 patients, between four and 16 years after osteosynthesis on the upper ankle joint.
