Current concepts for the use of platelet-rich plasma in the foot and ankle.

Platelet-rich plasma (PRP) injections have been used and studied since the 1970s. Its use has become more popularized over the last several years in the treatment of foot and ankle injuries. Platelets are a normal product found in the clotting cascade and inflammatory process of healing. They produce granules that release growth factors that promote healing. PRP works by increasing the concentration of platelets, thereby increasing the concentration of growth factors and increasing healing potential. PRP has an advantage over many tissue engineering products in that it is autologous. It has be…

Injuries in elite-level women's football-a two-year prospective study in the Irish Women's National League.

We investigated the pattern of injuries in elite-level women's football in Ireland, during a two-season prospective injury surveillance study in the Women's National League (WNL). Seven out of the eight clubs (271 players) in the WNL were followed prospectively during the 2018 and 2019 seasons. The injury incidence rate in matches (19.2/1000 h) was 7.5 times higher than in training (2.5/1000 h). Players, on average, sustained 0.69 injuries per season (266 injuries/383 player seasons), which equates to 15 time-loss injuries per season for a squad of 22 players. The majority of the injuries sust

The prevalence of overuse injuries in Australian non-elite netballers.

PURPOSE: The aim of this study was to investigate the prevalence of overuse injuries in non-elite netballers and whether these increase over … nee problems accounting for 21.2% (61 cases), and shoulder problems accounting for 5.5% (16 cases). Ankle injuries increased over the study period, while knee and shoulder problems decreased. Injuries were more common in players over 36 years old or those with less than 5 years of playing experience. Players who used a brace were more likely to have an overuse c …

Surgical and nonsurgical treatment of acute ankle sprains.

In the treatment of eversion injuries or tears of the medial structures, exact anatomic restoration is mandatory, and this must be accomplished by either closed reduction or surgery. The treatment of lateral ankle ligament injuries is usually cast immobilization, taping, or a cast-brace. Surgical treatment of lateral ankle ligament sprains is performed only on those highly competitive young individuals who require an almost normally functioning ankle.

Ankle instability.

Acute ankle sprains are common, and if inadequately treated may result in chronic instability. Lateral ankle injuries are most common, with deltoid injuries rare and associated with ankle fractures/dislocation. Medial ankle instability is rare. Functional management of acute lateral ankle sprains is the treatment of choice, with acute ligament repair reserved for athletes. Chronic lateral ankle instability is initially managed conservatively, however, failure of rehabilitation is an indication for surgical management. Nonanatomic tenodesis reconstructions have poor long-term results, sacrifice…

In Trauma Expect the Unexpected - A Case Report on Post-Traumatic Pancreatitis Associated with Superior Mesenteric Artery Pseudoaneurysm in Spinopelvic Fracture.

INTRODUCTION: Bony and ligamentous ankle injuries are some of the most commonly treated injuries by orthopedic surgeons. Open ligamentous ankle injuries without an associated fracture or dislocation are rare and to our knowledge have only sparsely been described in the literature. We present a case and successful treatment of an open lateral ankle injury with capsular rupture and ligamentous damage without fracture or dislocation in a 22-year-old female. Due to the rarity of this injury, there is not a standard treatment protocol. CASE REPORT: A 22-year-old female presented with an open ankle…

Arthroscopic Approach to Deltoid Injuries.

Deltoid ligament injuries may result in medial ankle instability (MAI), recently two subgroups related to an injury of the deep layer have recently been distinguished: dynamic MAI and rotational ankle instability (RAI). Dynamic MAI involves injury to the intermediate part of the deep layer (collicular fibers), usually from unresolved acute trauma. RAI stems from chronic injury of the anterior fibers, often secondary to long-standing lateral ankle instability, and is characterized by abnormal talar rotation and translation. These chronic deep-layer injuries are difficult to detect clinically or

Management in high-risk patients.

Injuries to the foot and ankle are often missed or underestimated in patients with polytrauma and are a source of long-term limitations. Injures below the knee are among the highest causes for unemployment, longer sick leave, more pain, more follow-up appointments, and decreased overall outcome. As mortalities decrease for patients with polytrauma a greater emphasis on timely diagnosis and treatment of foot and ankle injuries is indicated. Geriatric patients represent nearly one-quarter of trauma admissions in the United States. This article discusses perioperative management and complications…

Assessing Compliance With Ottawa Foot and Ankle Rules in the Emergency Department: A Closed-Loop Quality Improvement Project.

Introduction Traumatic foot and ankle injuries are frequent presentations to the emergency department (ED). The Ottawa Foot and Ankle (F&A) Rules are to help clinicians in decision making whether an X-ray of the foot or ankle is justified. Methods A retrospective analysis of ankle and foot X-ray requests from a South London Hospital ED was performed. Consecutive requests for both foot and ankle X-rays were assessed chronologically until n=100 for each cohort. The clinical documentation of each ED clerking and the X-ray request clinical details were then compared to the Ottawa Rules of that res…

[Current diagnostics and treatment of ankle sprains in German].

BACKGROUND: Ankle sprains are one of the most frequent injuries of the musculoskeletal system. The injury pattern determines the treatment and are crucial for the outcome. Nonoperative treatment is commonly recommended for isolated injuries of the lateral ligaments but no standard strategy exists in combined ankle ligament injuries. The goal of this national survey was to achieve an overview about the current diagnostic strategies and common treatment concepts in Germany. MATERIAL AND METHODS: All members of the German Society for Orthopaedics and Trauma Surgery (DGOU) were invited to partici

Sprained ankles in ballet dancers.

Ankle sprains are common in dancers. They result from working in the positions which allow increased risk of sprain on the lateral side of the ankle for many hours a day. One hundred ankle injuries were evaluated. The mechanisms of injury are presented, as well as a classification of these injuries. The recommended treatment and rehabilitation are discussed. Leg muscle strength must be restored before rehabilitation is considered complete.

Rotational Ankle Fracture Dislocation With Associated Lisfranc Fracture.

While ankle fractures most often result from a rotational injury, Lisfranc injuries are more commonly associated with an axial load on a plantarflexed foot. Due to differing mechanisms of injury, rotational ankle fractures with Lisfranc injuries are uncommon and rarely discussed in the literature. Here we present a case of a rotational ankle fracture-dislocation with a concomitant Lisfranc injury. The Lisfranc injury, which was ultimately treated nonoperatively, was discovered seven weeks after operative fixation of the ankle fracture. At the last follow-up nine months after the initial injury

Syndesmotic ankle sprains in athletes.

Ankle sprains are among the most common athletic injuries and represent a significant source of persistent pain and disability. Despite the high incidence of ankle sprains in athletes, syndesmosis injuries have historically been underdiagnosed, and assessment in terms of severity and optimal treatment has not been determined. More recently, a heightened awareness in sports medicine has resulted in more frequent diagnoses of syndesmosis injuries. However, there is a low level of evidence and a paucity of literature on this topic compared with lateral ankle sprains. As a result, no clear guideli
