Protective role of DL-alpha-lipoic acid against mercury-induced neural lipid peroxidation.

Experimental neurotoxicity in rat models was induced by an intramuscular injection of mercuric chloride. dl-alpha-lipoic acid was administered as an antidote in three protocols of experimental design. Two protocols of short-term exposure of mercury was designed, one with prophylactic therapy and the other with curative therapy of lipoic acid. The third protocol was with prophylactic therapy of lipoic acid on long-term exposure of mercury. Enhanced lipid peroxidation, depleted non-enzymic and perturbed enzymic antioxidant status were observed in cerebral cortex, cerebellum and sciatic nerves of

Effect of type and amount of carboxymethylcellulose on in vitro salicylate adsorption by activated charcoal.

The effect of the type of carboxymethylcellulose (CMC) and amount of CMC used in preparing antidotal charcoal formulations on the in vitro kinetics of sodium salicylate adsorption from simulated gastric fluid was assessed in agitated vessels of two designs. Mixtures made with low, medium, and high viscosity CMC were tested. Additionally, the effects of the charcoal and water contents of these mixtures on adsorption kinetics were considered. The results suggest that charcoal mixtures which are fluid enough to be pourable (hence, drinkable) are strongly to be preferred from the standpoint of rat

Advances in the Use of N-Acetylcysteine in Chronic Respiratory Diseases.

N-acetylcysteine (NAC) is widely used because of its mucolytic effects, taking part in the therapeutic protocols of cystic fibrosis. NAC is also administered as an antidote in acetaminophen (paracetamol) overdosing. Thanks to its wide antioxidative and anti-inflammatory effects, NAC may also be of benefit in other chronic inflammatory and fibrotizing respiratory diseases, such as chronic obstructive pulmonary disease, bronchial asthma, idiopathic lung fibrosis, or lung silicosis. In addition, NAC exerts low toxicity and rare adverse effects even in combination with other treatments, and it is

Malnourishment-Associated Acetaminophen Toxicity in Pregnancy.

BACKGROUND: Although acetaminophen is commonly used in pregnancy, it can deplete glutathione concentrations and cause accumulation of 5-oxoproline, with subsequent metabolic acidosis. CASE: A malnourished 25-year-old woman, G2P1001, with chronic acetaminophen use presented with abdominal pain and high anion gap metabolic acidosis. After ruling out other potential causes, her urine 5-oxoproline level was found to be elevated. She received N-acetylcysteine, with resolution of the acidosis. CONCLUSION: Those who care for pregnant patients should remain alert to 5-oxoprolinemia as a cause of met

Poppers-Induced Methemoglobinemia: A Curious Case of the Blues.

Methemoglobinemia (MetHb) can be a deadly condition at certain levels, presenting in a fulminant form of cyanosis or disguising itself with vague symptoms. Methemoglobinemia is an altered state of the body's hemoglobin, which can be congenital or acquired. We report a case of a 62-year-old male who presented with altered mental status and hypoxia after consuming "Jungle Juice", raising concern for methemoglobinemia. A diagnosis of methemoglobinemia was confirmed with arterial blood gas and guidance from New York State poison control. The patient was adequately treated with the antidote methyle

Nitric oxide rescues thalidomide mediated teratogenicity.

Thalidomide, a sedative drug given to pregnant women, unfortunately caused limb deformities in thousands of babies. Recently the drug was revived because of its therapeutic potential; however the search is still ongoing for an antidote against thalidomide induced limb deformities. In the current study we found that nitric oxide (NO) rescues thalidomide affected chick (Gallus gallus) and zebrafish (Danio rerio) embryos. This study confirms that NO reduced the number of thalidomide mediated limb deformities by 94% and 80% in chick and zebrafish embryos respectively. NO prevents limb deformities

[New anticoagulants--are we prepared?].

Rivaroxaban and dabigatran are the new anticoagulant drugs--factor Xa and IIa inhibitors. Recently registered in atrial fibrillation and venous thromboembolic disease became the serious competition to other anticoagulants. Blood coagulation parameters do not need to be monitored which can be seen as a huge advantage. At the same time their effectiveness in thromboembolic incidents prevention is comparable and the risk of serious hemorrhage is even lower comparing to therapy with vitamin K antagonists. Concerns arose around the fact that for the time being there is no effective antidote in case

Clinical hepatotoxicity. Regulation and treatment with inducers of transport and cofactors.

Drug-induced liver injury (DILI) is a rare condition. This review presents a clinical update on the incidence and prevalence of DILI, cellular targets and histopathological features of liver injury, and basic diagnostic approaches. At present, with the exception of acetaminophen poisoning where timely administered N-acetylcysteine serves as the antidote, treatment usually is supportive. Since most biotransformation processes in the liver are under the control of nuclear receptors (NR), treatment modalities aiming to stimulate or inhibit NR expression are at present tested in animal models of A

Toxicology of selected pesticides, drugs, and chemicals. Anticoagulant, cholecalciferol, and bromethalin-based rodenticides.

The control of rodent pests is a continuing goal of mankind. To this end, a multitude of rodenticides have been produced, each designed to kill rodents by exerting their toxic effects on various body systems. As examples, veterinarians have had to manage companion animal poisonings due to anticoagulant, sodium fluoroacetate (compound 1080), thallium, barium carbonate, and zinc phosphide-based rodenticides. Many of these rodenticides were introduced because of their anticipated safety in relation to nontarget species; unfortunately, this has not been the case. Veterinarians must attempt to iden

Cyanide poisoning successfully treated without 'therapeutic methemoglobin levels'.

A 24-year-old woman ingested an unknown amount of potassium cyanide in a suicide attempt. Coma and metabolic acidosis developed. Administration of the Lilly Cyanide Antidote kit (Eli Lilly and Co, Indianapolis) resulted in prompt resolution of symptoms and full recovery. Whole blood cyanide level was 13 micrograms/mL approximately one hour after ingestion. The highest measured methemoglobin level after sodium nitrite administration was 9.2%, demonstrating that attaining a "therapeutic methemoglobin level" of 25% is unnecessary to insure a satisfactory clinical outcome. Because severe hypotensi

Paracetamol medication and alcohol abuse: a dangerous combination for the liver and the kidney.

The clinical pattern of paracetamol-induced acute liver damage was studied with regard to whether the intoxication was acute or due to continuous use. This condition was found to be caused almost as often by accidental chronic medication as by acute intoxication, with annual incidences per 100,000 of 0.8 and 1.2 cases, respectively. The liver damage found in 18% of the cases of acute intoxication was generally mild, probably due to an early antidote treatment. In contrast, in the chronic intoxicated cases the liver damage was more pronounced and often accompanied by a severe renal dysfunction.

Effect of chlorpromazine on cyanide intoxication.

Previous reports from our laboratory indicated that prophylactic protection against cyanide intoxication in mice can be enhanced by administration of chlorpromazine when it is given with sodium thiosulfate. The mechanism of potentiation of sodium thiosulfate by chlorpromazine was studied alone and in combination with sodium nitrite. Although chlorpromazine was found to induce a hypothermic response, the mechanism of enhancement of the antagonism of cyanide by chlorpromazine does not correlate with the hypothermia produced. Various other possible mechanisms were investigated, such as rate of me

AHAK: a new effective medical countermeasure against percutaneous intoxication by Novichok chemical warfare agents.

Novichoks are the latest known and most toxic class of organophosphorus chemical warfare agents (OP CWAs) to have been developed. Among the documented Novichoks, A-232 is the most toxic and most potent inhibitor of AChE, as well as the most resistant to reactivation by oximes, and thus likely the most challenging to treat. Here we characterize the intoxication following dermal exposure to the Novichok A-232 in swine, and describe a novel countermeasure against it. Our results demonstrate that percutaneous poisoning by a lethal dose of A-232 (2 mg/kg) is characterized by slow appearance and exa

Case report: acute phosphine inhalation poisoning in a patient with Down's syndrome.

BACKGROUND: Phosphine, a highly potent insecticide, can cause damage to multiple physiological systems when inhaled by humans, including the nervous, circulatory, and urinary systems. Currently, there is no effective antidote for phosphine poisoning, which highlights the importance of systematic and effective supportive treatment. CASE PRESENTATION: This paper presents a case of a patient with Down syndrome who had a history of similar gas poisoning and was exposed to high-concentration phosphine gas within 10 min, resulting in inhalation poisoning and shock. Upon admission to our hospital, th

Acute Copper Toxicity: Succimer Makes you Less Blue.

INTRODUCTION: Acute copper toxicity is a rare occurrence, and management guidelines are not based on robust evidence. Treatment recommendations are often extrapolated from those of chronic copper intoxication and the management of Wilson's disease. Although D-penicillamine, EDTA, and dimercaprol have varying success, we describe the use of chelation with succimer monotherapy in a patient with acute copper salt ingestion with subsequent improvement of clinical status and copper concentrations. CASE REPORT: A 38-year-old woman presented to an ED after ingestion of liquid copper fungicide contai

Successful treatment of aluminium phosphide poisoning by dihydroxyacetone: A two-case report study.

WHAT IS KNOWN AND OBJECTIVE: Aluminium phosphide (AlP) is an agricultural fumigant which produces phosphine gas in the presence of moisture. Phosphine inhibits oxidative phosphorylation and causes cell death by inhibiting cytochrome C oxidase. Clinical manifestations of AlP poisoning are refractory hypotension, tachycardia, low oxygen saturation and severe metabolic acidosis. CASE SUMMARY: Two cases received dihydroxyacetone (DHA) in addition to routine management of AlP poisoning. Administration of DHA (7 gr in 50 mL sodium bicarbonate, gavage) 2 times at a 1-hour interval improved the clini

Development of New Biokinetic-Dosimetric Models for the Simulation of Iodine Blockade in the Case of Radioiodine Exposure in Man.

In the case of nuclear incidents, radioiodine may be liberated. After incorporation it accumulates in the thyroid and by internal irradiation enhances the risk of cancer occurrence. By administering a large dose of non-radioactive iodine the uptake of radioiodine into the gland can be inhibited ("iodine blockade"). Biokinetic models using first order kinetics are not suited to simulate iodine blockade, as the uptake into the gland is mediated by a saturable active transport. Therefore, we integrated an uptake mechanism described by a Michaelis-Menten kinetic into a simple ICRP biokinetic model

Case Report of Lethal Toxin Lurking in an Edible Plant.

Cyanide is notoriously known to the public for more than a century now as a weapon of mass destruction (Zyklon B gas - hydrogen cyanide used by Nazis), an agent for chemical warfare during World War I (hydrogen cyanide) and very infamous "Suicide Pill" used in the past by military and espionage organizations during World War II (potassium cyanide). During the modern industrial era, cyanide poisoning is commonly associated with the industrial exposure and domestic fires. But there is little awareness about potentially fatal consequences of cyanide poisoning from common food sources. Here, we pr
